Il a été largement attribué à l'utilisation croissante de fluor. Ce déclin a aussi été observé en France et dans le Massif Central. En effet, deux études épidémiologiques descriptives (1987-1991) sur la santé bucco-dentaire des enfants âgés de 6 à 15 ans scolarisés dans le massif Central ont révélé une réduction significative de la prévalence carieuse. En 1991, une étude épidémiologique a visée étiologique conduite sur un échantillon d'adolescents âgés de 14 et 15 ans scolarisés dans le Massif Central a permis de clarifier l'influence actuelle des facteurs de risque liés à la carie dentaire. L'analyse multidimensionnelle des données indique que le grignotage, l'hygiène mais surtout le niveau d'apport en fluor influencent significativement et indépendamment le niveau de l'atteinte carieuse. La Caisse Maladie régionale des professions indépendantes a décidé à titre expérimental de mettre en place un programme de prévention bucco-dentaire en Auvergne. Il s'agit de tenir compte des données épidémiologiques récentes en matière de santé dentaire et d'évoluer d'un système orienté vers les soins curatifs vers un système orienté vers la santé. Les enfants du régime âgés de 4 à 15 ans se voient proposer une visite dentaire annuelle chez un dentiste librement choisi et une prise en charge à 100% des soins en rapport avec la prévention et le traitement de la carie dentaire. Le bilan après un an est encourageant puisque 50% des enfants de 4-5 ans concernés ont réalisé la consultation prévue. Dans l'avenir, seule une évaluation rigoureuse permettra de connaître l'efficacité d'une telle action surla santé dentaire des participants.

Book excerpt: This study explores the policy options a provincial government might consider in extending health care coverage to the purchase of prescription drugs and dental care. It examines the major public policy objectives involved, such as spreading risk, redistributing wealth, and reducing the barriers to care, and evaluates alternative programs in terms of their costs and efficiency as well as their realization of the basic social objectives of health care. Using varied statistics, some drawn from schemes in other provinces, it estimates what different packages of pharmacare and denticare would have cost in Ontario in 1975. The results indicate that universal coverage may be one of the most costly and least effective options. Based on current modes of service delivery, a universal pharmacare and denticare program would transfer wealth to upper income groups without significantly improving the utilization of health care services. A study of drug manufacturing and retailing systems in Canada and of the structure of dental services suggests that wasteful methods of service delivery could lower per capita costs by 30 to 40 per cent. Potential annual savings in pharmacy and dentistry together in Ontario run into the hundreds of millions of dollars. The authors show how a combination of competitive pressures and selective public intervention can be used to rationalize the delivery system. They caution, however, that such potential savings will be forever unrealized if a public-insurance type of program is introduced which freezes the existing system in place and forecloses the options of either public provision or private market competition.
