It reduces the overall quality of life and therefore requires specific care systematic and effective solution coping with everyday commands, prohibitions, restrictions, obligations and necessities. As the treatment of this disease with many adverse consequences and strict restrictions is increasingly concentrated in the domestic sphere, features professional diabetes education, counselling and health upbringing and the desired inherent part of personalized care in a spirit of evidence-based medicine. It plays an important role in the prevention of diabetes mellitus in the population, other health problems of diabetics, complications of diabetes mellitus and socio-pathological phenomena at any age.Aim: In this work we are presenting education, counselling and health upbringing as an inherent part of personalized care in a spirit of evidence-based medicine. In detailed we present diabetes education, counselling and health upbringing as a form of primary, secondary and tertiary prevention. By survey we found the need, scope and importance of professional education, counselling and health upbringing of diabetic patients, patients with pre-diabetic syndromes and risk population.Method: Education as a system of the transmission of specific knowledge and associated intellectual and manual skills and habits directs moral, aesthetic and physical development in accordance with the objectives and role in society. Diabetes education eliminates the deficit of knowledge and skills that have arisen and could persist due to the presence of diabetes. Specialized social counselling is focused to the specific needs and problems of diabetics whose solution requires a comprehensive approach. Health upbringing across the entire spectrum of activities and using modern methods of providing and expanding the theoretical knowledge of health, strengthens the attitudes and habits of prevention, protection and support of health not only hazardous but also population groups and significantly promotes positive and eliminates the negative elements relating to health.Discussion: Diabetes education, counselling and health upbringing play a crucial role in a specific diabetes care as a risk groups of individuals. Treatment in the spirit of personalized evidence-based medicine and the development of chronic and organ-specific complications changes. Therefore it requires constant updating and complementing theoretical knowledge, intellectual and manual habits and skills, as well as systematic guidance in eliminating the negative effects of the disease. Important role to play is effective communication with diabetological team. Our long experience has raised the need for lifelong systematic diabetes education, which would be managing university educated professionals. Because diabetic patients are living with disease and its difficulties all life, diabetes education and counselling undertaken by volunteers working off time in civil jobs is not enough to cover all needs and requirements. Therefore there is a need for the implementation of diabetes education and specialized social counselling in regular working time and in health care facilities for 345,475 (2015) diabetic patients in Slovakia and other individuals in the pre-diabetic stage.Results: The main objective of diabetes education is to train, educate and inform diabetic patients and with this improve compensation, extend and improve the quality of life, reduce the financial cost of a personalized treatment in a spirit of evidence-based medicine, the consequences of acute, chronic and organ-specific complications, but also safeguard the rights of diabetic patients as citizens, patients and consumers and thus to achieve their full integration into society. The aim of diabetes education is to get proper theoretical knowledge, practical skills and a good ability to implement them at home. Health problems and the resulting social consequences represent for most diabetic patients problem of social integration, therefore, a specialized social counselling is focused on coping with health problems and their consequences, improvement of adaptation to the disease, the area of support and social support, guidance, orientation in legislation and claims, if necessary for distribution to another entity. In providing different methods and techniques are used, as well as own instrument DIASSQL 23/20 (Diabetes Social Support Quality of Life 23/20) created in Diabetes Educational Centre DIAMART to measure and early detection of psychosocial problems of diabetics, which significantly decompensate diabetes mellitus and thereby reducing the quality of life.

Other elements of importance to the delivery of diabetes care, in addition to team care, such as creating a patient registry, assessing practice needs, implementing processes of care, connecting to community resources, and evaluating outcomes are presented in detail on www.BetterDiabetesCare.nih.gov. This website provides tools and resources to help health care professionals implement systems changes. Early identification and management of pre-diabetes can delay or prevent the onset of type 2 diabetes. In people with type 1 and type 2 diabetes, ongoing comprehensive diabetes care, including the ABCs of diabetes (A1C for glucose, Blood pressure, and Cholesterol), can prevent or control diabetes-related microvascular and macrovascular complications. With proper medical management, education, self-care, and attention to behavior, social, and environmental factors, people with diabetes and pre-diabetes can live long, active, and productive lives.
