Book excerpt: La fibrillation atriale (FA) est le type d'arythmie cardiaque le plus fréquent en Europe et aux Etats Unis. La survenue d'un ou plusieurs accidents vasculaires cérébraux (AVC) constitue l'une des complications les plus graves de cette pathologie. La part majeure des patients atteints de FA bénéficient d'un traitement préventif antithrombotique, ayant pour objectif premier de minimiser le risque d'AVC. Depuis des décennies, les traitements traditionnellement prescrits sont les antiagrégants plaquettaires, malgré une efficacité limitée, ou les antagonistes de la vitamine K (AVK) qui, malgré une efficacité reconnue, restent utilisés avec grande précaution tant leur utilisation est complexe. Récemment, de nouveaux anticoagulants oraux (NACO) innovants ont obtenu leur autorisation de mise sur le marché (AMM) pour l'indication thérapeutique de prévention des AVC chez les patients atteints de FA : dabigatran, rivaroxaban, apixaban...Les dossiers d'études cliniques de ces nouvelles molécules ont apporté des résultats très convaincants : une efficacité renforcée, une tolérance majorée ou encore une maniabilité également augmentée. Néanmoins, ces traitements présentent également quelques points de faiblesse. Cette thèse aura notamment pour but de mesurer l'impact de ces nouvelles alternatives thérapeutiques dans les habitudes de prescription dans la prise en charge de la FA, par une comparaison des avantages et les inconvénients de ces molécules face aux traitements traditionnels.

Problème majeur de santé publique, l'accident vasculaire cérébral reste méconnu du grand public en particulier ses traitements possibles et le facteur de risque que constitue la fibrillation auriculaire. Les AVC touchent chaque année environ 130000 nouveaux patients en France et représente un poids financier considérable d'environ 8.3 milliards d'euros. Le plan AVC du gouvernement qui fait suite au rapport du Pr Fery-Lemonnier donne une part nouvelle à la prévention. Boehringer Ingelheim, laboratoire pharmaceutique, très impliqué dans ce domaine, a mis en place, dans le cadre du plan AVC, plusieurs expérimentations dont une à l'officine avec le groupe PHR et une à l'hôpital : la filière AVC. Ce travail reprend le cheminement de ces 2 expérimentations ainsi qu'une mise au point sur cette pathologie (étiologie, épidémiologie, facteurs de risque, prévention et traitements pharmacologiques). Le pharmacien, professionnel de santé reconnu par la loi HPST, a été trop souvent oublié, ce travail montre que le pharmacien, au-delà de son simple rôle de dispensation, peut intervenir dans une démarche généralisée de coopération dans la prise en charge du patient.
