Book excerpt: La dernière Conférence de Consensus sur la bronchiolite aiguë du nourrisson, réalisée par la HAS en 2000, préconisait comme traitement la prescription d'une désobstruction rhino-pharyngée associée à des mesures hygiéno-diététiques et à de la kinésithérapie respiratoire. Notre étude avait pour objectif de dresser un état des lieux de cette prise en charge thérapeutique auprès des médecins généralistes de la Sarthe quinze ans après ces recommandations. Les autres objectifs étaient d'évaluer la connaissance des critères diagnostiques et d'hospitalisation par les médecins généralistes, et de comprendre les raisons d'éventuelles discordances. Il s'agissait donc d'une étude observationnelle descriptive pour l'objectif principal, avec des variables qualitatives pour les objectifs secondaires. L'étude était réalisée à partir d'un questionnaire adressé par téléphone ou internet aux médecins généralistes de la Sarthe. Parmi les médecins généralistes de la Sarthe, 14,39% ont répondu à notre étude. Nos résultats ont retrouvé 35,71% des prescriptions en accord avec les recommandations. 53,57% des médecins interrogés prescrivaient un traitement médicamenteux autre que celui figurant dans les recommandations, La kinésithérapie respiratoire, bien que controversée, était prescrite dans 64,29% des cas. Par ailleurs, les médecins connaissaient en moyenne 3,48 critères diagnostics et 2,96 critères d'hospitalisation. Ces résultats sont à mettre en perspective avec le fait qu'il semble y avoir un lien entre le nombre de critères d'hospitalisation rapportés et la connaissance du diagnostic. Enfin, les principales difficultés dans la prise en charge de la bronchiolite rapportées par les médecins interrogés étaient l'examen clinique difficile de l'enfant, la pression des parents pour un traitement rapide et efficace, ainsi que le manque d'option thérapeutique pour cette pathologie. En conclusion, on peut dire que, par rapport aux recommandations sur le traitement de la bronchiolite, le taux de prise en charge concordante reste assez faible, mais que les habitudes de prescription des médecins généralistes s'améliorent.
