C'est pourquoi leur prise en charge à l'officine doit être optimale afin d'aider le patient à éviter le rejet de l'organe transplanté, les infections et d'autres effets indésirables potentiels des médicaments. Cette prise en charge à l'officine peut débuter relativement tôt par le soutien du patient lorsque celui-ci est inscrit sur liste d'attente de transplantation. Le rôle du pharmacien sera alors de veiller au maintien : de l'état de santé du patient notamment en veillant à une observance médicamenteuse rigoureuse des traitements en cours. Suite à la transplantation, le pharmacien aura une place privilégiée dans la prise en charge médicamenteuse et également dans l'éducation thérapeutique de son patient. Il doit s'assurer de la bonne observance par son patient du traitement immunosuppresseur. Le pharmacien doit l'informer de tous les éléments essentiels directement liés à son traitement : conditions de prise des médicaments, conduite à tenir en cas d'oubli d'une prise ou en cas de vomissement après la prise d'un comprimé, quels sont les aliments à éviter ou à privilégier. Le pharmacien peut remettre une fiche de conseils lors de la délivrance du traitement afin que le patient puisse se remémorer le message oral qui lui aura été dispensé. Le pharmacien ne doit pas omettre de prodiguer des règles d'hygiène ainsi que des mesures diététiques à respecter. En outre, le pharmacien doit insister sur les examens de suivi du patient : dosage des taux d'immunosuppresseurs, surveillance de la fonction rénale et hépatique par exemple. Enfin, le pharmacien par son écoute joue un rôle dans le soutien psychologique du patient. Il peut également lui conseiller de s'orienter vers des associations de patients si cela est nécessaire.

Book excerpt: La transplantation d'organe est actuellement l'une des principales thérapeutiques pour des malades atteints de pathologies conduisant à une perte irréversible de la fonction d'organes vitaux. Parmi les six organes pouvant être transplantés, le rein est le plus fréquemment greffé. Si la technique est maîtrisée, un des principaux obstacles est le rejet. Pour éviter ce phénomène, les patients prennent quotidiennement un traitement immunosuppresseur complexe aux nombreux effets indésirables et dont les taux plasmatiques sont parfois difficiles à équilibrer. Le suivi et l'accompagnement du patient semble donc être primordial pour assurer son adhésion au traitement à long terme. Le pharmacien, en dispensant chaque mois leur traitement aux patients, est un véritable professionnel de santé de proximité. Il semblait donc important qu'il intervienne dans la démarche d'appropriation par le patient de son traitement et des règles hygiéno-diététiques à adopter. Notre étude avait pour but de connaître la vision de chacune des parties afin de proposer des solutions d'amélioration de la prise en charge à l'officine de ces patients. Deux questionnaires ont donc été conçus. Le premier était destiné aux pharmaciens d'officine pour évaluer ses difficultés lors de la dispensation des traitements anti-rejet (gestion du stock, des interactions médicamenteuses, utilisation du dossier pharmaceutique...) ainsi que son ressenti par rapport à l'éducation thérapeutique du patient. Le deuxième, quant à lui, était destiné aux patients greffés et a montré que certains patients ont des lacunes dans les connaissances vis-à-vis de leur traitement (gestion des prises des médicaments et des oublis, mesures hygiéno-diététiques à respecter, suivi biologique et clinique à respecter, gestion de situations problématiques et des effets indésirables, vaccins contre-indiqués, mesures de protection vis-à-vis du soleil et des infections...). Ils peuvent compléter les connaissances acquises lors de séances d'éducation thérapeutique par des entretiens pharmaceutiques à l'officine avec leur pharmacien. Nous avons donc réalisé une trame pour le déroulement d'entretiens pharmaceutiques, un jeu de cartes sur les situations de la vie courante et des supports éducatifs à partir des besoins émanant despersonnes interrogées. En manipulant ces outils pédagogiques, le patient devient acteur de son apprentissage et il peut emporter les fiches conseils pour compléter les informations reçues à l'oral. De plus, ces outils faciliteront le déroulement de l'entretien pour le pharmacien. L'objectif du travail transversal entre professionnels est de rendre acteur de leur santé des malades qui ont des traitements à prendre quotidiennement.
