Book excerpt: The World Health Organization has updated its 1988 publication on the partograph by addressing the findings of a multicenter trial conducted during 1990-91. The second part of the updated publication, "Preventing Prolonged Labour: A Practical Guide--The Partograph, Part II: User's Manual," provides guidelines on the use of the partograph in the management of labor. It focuses on the practical aspects of using the partograph as a managerial tool in labor and does not address theoretical aspects. The partograph helps identify early women in slow labor and prevent and manage any resultant complications. It should be used only for women who have no pregnancy complications that require immediate action and who have actually started labor. The partograph allows health workers to plot the progress of labor (i.e., cervical dilatation, descent of the fetal head, and uterine contractions), the fetal condition (i.e., fetal heart rate, membranes and amniotic fluid, and molding of the fetal skull), and the maternal condition (i.e., pulse, blood pressure, temperature, urine [volume, protein, acetone], drugs and IV fluids, and oxytocin regime). The manual addresses what needs to be done in the case of abnormal progress of labor which incorporates prolonged latent phase (>8 hours) and prolonged active phase. It then moves on to discussing management of labor by focusing on normal latent and active phases, between alert and action lines, at or beyond active phase action line, prolonged latent phase, and further discussion on oxytocin, membranes, and fetal distress. The manual provides two exercises and answers to test knowledge on correct partograph use. Annex I is an aid cutout to be reproduced in plywood to help health workers accurately measure cervical dilatation.

Part IV lists the four OR protocol objectives as regards to the partograph and hypotheses behind OR protocol. The OR methodology consists of several phases: collection of data prior to introduction of the partograph, introduction of the partograph by means of a training program, evaluation of training, collection of data after introduction of the partograph, and data analysis. OR should state whether or not health workers can be trained in correct use of the partograph, partograph reduces prolonged labor and maternal morbidity and perinatal morbidity and mortality, and referrals of women in labor based on abnormal partograph are appropriate.
