Book excerpt: Contexte : Les personnes âgées fragiles, notamment celles de plus de 75 ans, sont les plus à risque de perte d'autonomie. Le médecin généraliste a un rôle essentiel dans la prescription d'activités physiques adaptées (APA) aux personnes âgées fragiles, afin de rendre ces personnes plus actives et moins sédentaires. Notre étude, observationnelle, quantitative, avait pour but de décrire la prescription d'APA des médecins généralistes d'Indre et Loire, à leurs patients âgées fragiles de plus de 75 ans. Résultats : 211 médecins généralistes ont répondu à l'enquête (taux de participation = 41,1%). Parmi les 203 (93,2%) médecins qui déclaraient inciter leurs patients âgés fragiles à pratiquer des APA, 166 (81.8%) donnaient des conseils généraux, 63 (31%) précisaient la durée et la fréquence des activités physiques, et 15 (7,4%) donnaient un programme détaillé d'activités physiques adaptées (type d'APA, durée, fréquence, intensité). Les prescriptions d'APA étaient essentiellement orales (93,1%). 8,9% des médecins effectuaient une prescription écrite et 12,3% remettaient une brochure d'information sur l'AP. Les types d'APA les plus cités étaient: l'équilibre et la lutte contre les chutes (70,9%) et la souplesse et le maintien des amplitudes articulaires (53,2%) pour les activités encadrées par un professionnel. La marche (97%), la gymnastique douce (73,9%) et le maintien des activités de la vie quotidienne (70,9%) étaient les plus citées parmi les activités libres ou en structure associative. Conclusion : La prescription d'APA chez les personnes âgées fragiles, par les médecins généralistes d'Indre et Loire, consiste essentiellement en des recommandations générales, à l'oral. Les APA proposées visent le maintien de l'autonomie et la lutte contre les chutes. Une meilleure information sur les modalités de prescription d'APA et sur les structures qui en proposent permettrait sans doute d'améliorer le nombre et l'efficience des prescriptions d'APA.

Pour les personnes âgées souvent fragiles et en perte d'autonomie, les médecins généralistes ont une place centrale dans la mise en place de l'activité physique adaptée aux personnes âgées, pour qu'elles soient moins sédentaires et pratiquent une activité physique. Notre étude qualitative avait pour but de d'évaluer les connaissances de l'activité physique adaptée chez la personne âgée et de sa mise en place par les médecins généralistes. Résultats : Treize médecins généralistes ont été interrogés pour la réalisation de cette étude qualitative. L'ensemble des médecins déclaraient conseiller la pratique de l'activité physique à leurs patients âgés. Cependant la prescription de l'activité physique adaptée est souvent mal connue par les médecins généralistes qui rencontrent plusieurs freins à sa mise en place comme le manque de connaissances ou d'informations sur les parcours de soins et les structures existantes. De plus un manque de temps à consacrer à sa mise en place est également souligné. Pour ces raisons, peu de médecins indiquaient la fréquence et la durée de l'activité physique avec un programme précis. Et la prescription de l'APA chez les personnes âgées était plutôt à base de conseils oraux et généraux. Conclusion : La prescription de l'activité physique adaptée chez les personnes âgées par les médecins repose essentiellement sur des conseils généraux et oraux afin de limiter la perte d'autonomie et ses complications. Cependant, pour améliorer la mise en place de l'activité physique adaptée chez la personne âgée, devant le manque de connaissances et d'informations que soulignent les médecins généralistes, des éclaircissements sur la prescription de l'APA et des structures proposant de l'APA pourraient améliorer la mise en place de cette dernière.
