Traditional and non-traditional medical and public health stakeholders from across federal, state, and local government health care coalitions, community organizations, school districts, child care providers, hospitals, private health care providers, insurers, academia, and other partners in municipal planning met to review existing tools and frameworks that can be modified to include children's needs; identify child-serving partners and organizations that can be leveraged in planning to improve outcomes for children; highlight best practices in resilience and recovery strategies for children; and raise awareness of the need to integrate children's considerations throughout local and state emergency plans. Communities across the United States face the threat of emergencies and disasters almost every day, natural and man-made, urban and rural, large and small. Although children represent nearly 25 percent of the U.S. population, current state and local disaster preparedness plans often do not include specific considerations for children and families. The preparedness and resilience of communities related to children will require a systems framework for disaster preparedness across traditional and non-traditional medical and public health stakeholders, including community organizations, schools, and other partners in municipal planning. This report examines resilience strategies that lead to successful recovery in children after a disaster and discusses current approaches and interventions to improve recovery in children.

After the catastrophic 2004 tsunami in the Indian Ocean, legislation was passed to expand U.S. tsunami warning capabilities. Since then, the nation has made progress in several related areas on both the federal and state levels. At the federal level, NOAA has improved the ability to detect and forecast tsunamis by expanding the sensor network. Other federal and state activities to increase tsunami safety include: improvements to tsunami hazard and evacuation maps for many coastal communities; vulnerability assessments of some coastal populations in several states; and new efforts to increase public awareness of the hazard and how to respond. Tsunami Warning and Preparedness explores the advances made in tsunami detection and preparedness, and identifies the challenges that still remain. The book describes areas of research and development that would improve tsunami education, preparation, and detection, especially with tsunamis that arrive less than an hour after the triggering event. It asserts that seamless coordination between the two Tsunami Warning Centers and clear communications to local officials and the public could create a timely and effective response to coastal communities facing a pending tsuanami. According to Tsunami Warning and Preparedness, minimizing future losses to the nation from tsunamis requires persistent progress across the broad spectrum of efforts including: risk assessment, public education, government coordination, detection and forecasting, and warning-center operations. The book also suggests designing effective interagency exercises, using professional emergency-management standards to prepare communities, and prioritizing funding based on tsunami risk.

Book excerpt: This study examines the perceptions of school personnel regarding emergency preparedness at schools and the mental health needs of students. More specifically, this study examines school personnel’s awareness of emergency preparedness plans at their sites and the mental health services that are available at their schools. A survey was administered to 40 school personnel at three campuses located in various geographical areas within Stanislaus County. The findings reveal that almost all participants had some awareness of their school emergency preparedness plan. In addition, the majority of participants acknowledged understanding procedures related to their sites’ emergency preparedness plans. Although most participants were aware that their sites had emergency preparedness plans, the results revealed that disparities of knowledge existed between administrators and teachers and that knowledge differed based on school grade level. Overall, administrators were more familiar with emergency plans than teachers and participants from elementary schools were more familiar with emergency preparedness plans than those at higher grade levels. Differences in perceptions regarding risk and the presence of law enforcement on campuses also differed based on school grade level. Participants from junior high and high school campuses were more supportive of law enforcement on campus than those from elementary campuses. The findings also revealed differences in awareness of the availability of mental health services for students based on the geographical location of the schools. Participants in urban areas had more knowledge than those in non-urban or rural areas. Further examination of inconsistencies in knowledge and perceptions about emergency preparedness plans among school personnel and further attention to availability of mental health services for students irrespective of the geographical location of the school are needed.

Despite important advances in our country's ability to respond effectively to chemical, biological, or nuclear terrorism, there continues to be inadequate development of pediatric protocols that could be implemented by the local, State, and Federal agencies charged with preparation and consequence management. Emergency preparedness plans have evolved over recent years to include not only intentional (terrorist) disasters but also unintentional public health emergencies such as natural disasters (e.g., earthquakes or floods; chemical incidents such as hazardous materials releases; and emerging infections such as Severe Acute Respiratory Syndrome (SARS), West Nile virus, and pandemic influenza). Under principles of dual functionality, emergency response plans must now take the approach of creating response plans that integrate intentional and unintentional disasters. Children differ from adults in many ways that are of great importance in building public health emergency response plans. Their greater susceptibilities result from differences in breathing rate, skin permeability, innate immunity, fluid reserve, communication skills, and self-preservation instincts. These differences and others require that disaster response plans be modified for such a priority population. Children also spend as much as 70-80 percent of their waking hours away from their parents in school. Schools, therefore, have a vital role in assuring that children are cared for and proper interventions are delivered after a public health emergency. When this project was undertaken in 2004, there was no national model for school-based public health preparedness. Consequently, school districts across the Nation had rudimentary, fragmented, or non-existent emergency preparedness programs. Since 2006, there has been a marked increase in awareness of the vulnerability of schools and the challenging logistics involved in protecting children in schools during unexpected events. However, there continue to be obstacles for many school districts in creating a practical, comprehensive, and practiced school-based emergency response plan. Among these obstacles are evacuation, accommodations for children with special health care needs, and inclusion of after-school programs in emergency response plans. Under a contract from AHRQ, the Center for Biopreparedness at Children's Hospital Boston conducted an analysis of emergency response plans from school districts in Massachusetts, Florida, Wisconsin, Colorado, and California. Using these findings in conjunction with existing recommendations on the development of school-based preparedness programs, we developed a template that provides an overview, including “best practices” for school districts to use in their development of a comprehensive emergency response plan. Finally, in cooperation with the Brookline, Massachusetts, public schools, we designed a roadmap for the development of school-based plans for each of the eight elementary schools, high schools, preschools, and after-school programs in Brookline. This monograph provides guidelines for use by school districts of all sizes. Our goal in creating this monograph is to describe to readers a practical approach to creating a school-based all-hazards emergency response plan from the national literature in combination with “lessons learned” in the field.

These stories ranged from actions of protecting students during tornadoes to active shooter incidents that resulted in loss of life. It was surprising to find that in Illinois there is no specific training for K-12 personnel in emergency preparedness and the mitigating actions they can take before, during and after disasters and emergency situations. This study researches the currently available training at federal and state level and proposes comprehensive disaster preparedness, awareness, and workplace violence training units specifically tailored for K-12 school administrations, staffs and faculties to prepare them for the unexpected in the school environment.
