Available in PDF, EPUB and Kindle. Book excerpt: Cardiac surgery has developed dramatically since the first open-he art operations were performed in the mid 1950s. Although the improvement of surgical technique, extracorporeal circulation, and postoperative management has contributed to a marked reduction of morbidity and mortality, the development of cardiac surgery to its present state would not have been possible without blood substitution by homologous donor blood. Only 20 years ago, open-heart operations required an average of 8 units of blood preserves. The excessive need of donor blood in those early days was mainly due to premature surgical technique, insecure control of anticoagulation, severe blood trauma by extracorporeal circulation, and the lack of retransfusion technologies that would have allowed the reuse of shed mediastinal blood. The introduction of new technologies, such as normovolemic hemodilution, in traoperative autotransfusion, postoperative return of shed mediastinal blood, and predonation of autologous blood has greatly reduced donor blood requirements. At present the majority of routine coronary artery surgical procedures can be performed without any blood transfusion. Blood loss, however, may be considerable in patients undergoing complex valve surgery or reoperations, as they often require several units of transfused blood. Blood conservation has now become an area of major interest for the cardiac surgeon. This increased concern is caused by infectious complications of blood transfusion, in particular hepatitis and, more recently, AIDS.

Book Hemostasis in Cardiac Surgery

Download or read book Hemostasis in Cardiac Surgery written by Safuh Attar and published by Wiley-Blackwell. This book was released on 1999-09-01 with total page 262 pages. Available in PDF, EPUB and Kindle. Book excerpt: Medical knowledge about blood, its use in transfusions, and blood products has grown exponentially, as well as awareness about its safety, risks and costs. In a discipline where risks are proportional to benefits, medical specialists involved in open heart surgery need to have the latest information to make informed decisions. Dr. Attar's work reflects the contributors' direct interest and experience in blood and cardiac surgery introducing the latest information on blood and its use, hemostasis, blood conservation techniques and safety, the risks involved, alternatives to blood transfusions, the diagnosis and therapy of coagulopathy associated with cardiac surgery, and medicolegal aspects. Medical advances include the latest intraoperative diagnostic measures for platelet functions and therapy strategies for hemostasis abnormalities with cardiac surgery. This work is unique for its extensive and critical description of instruments and tests used intraoperatively, especially for the evaluation of the coagulation factors and platelets, the algorithms for transfusion guidelines and the detailed application of APROTININ and other antifibrinolytic agents in minimizing bleeding in cardiac surgery. Written for cardiac and transplant specialists -- from surgeons, anesthesiologists, surgical nurses to perfusionists -- this valuable and timely reference thoroughly explores the subject of blood transfusions in open heart surgery, its clinical applications, the current status of blood substitutes and important legal aspects.

Book Autologous Blood Donation Decision Making Factors Used by Preoperative Patients

Download or read book Autologous Blood Donation Decision Making Factors Used by Preoperative Patients written by Betty A. Frost and published by . This book was released on 2001 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this descriptive study was to gain insight into factors used by preoperative patients to make a decision to donate autologous blood. Twelve patients who donated autologous blood preoperatively participated in the study. Qualitative analysis revealed four major categories that described the patient perspective and experience: preparing for surgery: the likelihood of requiring a tr nsfusion; dealing with the anticipated blood loss during surgery; the autologous blood donation experience; and the autologous blood donation outcomes and future considerations. Demographic findings were consistent with the literature that a major benefit of autologous blood donation is the reduced risk for homologous blood transfusion. Other factors such as choosing autologous blood donation from multiple options presented, amount of anticipated blood loss during surgery and cost-effectiveness of autologous blood donation were not revealed as motivators used in decision-making. The physician played a key role in recommending autologous blood donation. The nurse played a key role as a resource prior to and during the autologous blood donation. (Abstract shortened by UMI.).

Book A Comparative Study Autologous Blood Donation with Normovolemic Haemodilution in a Tertiary Cardiac Unit UK

This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Title:tA comparative study: autologous blood donation with normovolemic haemodilution in a tertiary cardiac unit, UKAuthor(s):tKalamkar Y.1, Jeeji R.1, Mehta V.1, Butterworth G.1, Balacumaraswami L.2Institute(s):t1University Hospitals of North Midlands, Dept of Anaesthesiology & Intensive Care, Stoke on Trent, United Kingdom, 2University Hospitals of North Midlands, Cadiothoracic Surgery, Stoke on Trent, United KingdomText:tBackground: It is estimated that cardiac surgical units utilise about 4% of all red cell transfusions. [1.] The national blood transfusion audit (NBA) reveals wide variation in transfusion rates and product usage between units. [2] Autologous Blood Donation (ABD) with normovolemic haemodilution is an established strategy for blood conservation.Method: This is a retrospective study of blood transfusion practice in our adult cardiac unit. Two groups, 78 patients in each, undergoing elective and emergency cardiac surgery were included. Normovolaemic haemodilution was performed with one unit of autologous blood (500ml) collected via a PA sheath under all asepsis before skin incision in ABD group. Autologous blood was reinfused after heparin reversal. Anonymised data were collected from a theatre management system, patient's anaesthetic notes and transfusion databases. Comparison was made with the NBA.Results: A total of 156 patients were analysed. The largest procedure groups were coronary artery bypass grafts (CABG) on-pump and off pump, followed by aortic valve surgery and single valve with CABG procedures. The gross transfusion rates for packed red cells (PRC), fresh frozen plasma (FFP), cryoprecipitate (Cryo) and platelets (Plt) were 15.3%, 6.4%, 5.1% and 2.5% in ABD group and 69.2%, 47.4%, 62.8% and 56.4% in Non-Autologous transfusion group respectively.National Comparative Audit of blood transfusion 2011 shows that the average rate transfusion rate of 43% for Packed red cells, 13% for FFP and 15% Platelets. Blood transfusion rate in all patients on-pump and off pump, elective and non-elective CABG is 42% in our centre. Conclusion: The overall transfusion rates are significantly less in patients with autologous blood donor group.Homologous blood transfusion can have adverse effects on clinical outcomes. Patients undergoing urgent or emergency coronary artery bypass grafting (CABG) are usually on single or dual antiplatelet agents this is associated with increased need for homologous blood transfusion perioperatively. Autologous blood donation significantly reduces allogeneic blood requirement in cardiac surgery. We recommend Autologous blood donation should be considered as a cost-effective and safe alternative to reduce allogeneic blood consumption.References: 1. Tinegate et al. Book Patient Blood Management in Cardiac Surgery

Download or read book Patient Blood Management in Cardiac Surgery written by Christian von Heymann and published by Springer. This book was released on 2019-06-03 with total page 219 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book provides a multidisciplinary approach to the maintenance of hemostasis and minimisation of blood loss in patients undergoing cardiac surgery. All aspects of patient blood management are covered that may contribute to a reduction in perioperative bleeding and transfusion requirements in cardiac surgery. This is achieved through practical cases and a theoretical background that gives a better understanding of patient hemostasis and the occurrence of bleeding complications. This book is relevant to cardiac surgeons, anesthesiologists, clinical perfusionists, hematologists and intensivists.
