In patients having ophthalmic surgery, music therapy is effective at reducing patient anxiety compared to the administration of anti-anxiety medications such as Versed. Long wait times, fear of the unknown or pain, fear of disfigurement, and loss of control all contribute to feelings of anxiety. Upon entering into the preoperative area, the patient's body will enter in to the flight or fight phase. Music therapy is implemented while the patient is in the preoperative, intraoperative, and postoperative areas in an effort to reduce anxiety and help the patient's body return back to its baseline state. By increasing the utilization of this intervention by nurses in the surgical setting, the overall surgical experience can be improved for patients.

Book excerpt: The use of music as an intervention in the perioperative setting may be a beneficial tool in decreasing anxiety and pain associated with the surgery. The patient facing a surgical procedure may have anxiety related to the surgery itself, the physical challenges post-surgery, financial concerns, and the prospect of postoperative pain. The implementation of music in the perioperative setting is a cost-effective, non-invasive, non-pharmacologic intervention without adverse effects. The current preoperative procedure does not include the provision of a music intervention. This proposal utilizes the use of portable music players and headphones for patients in the perioperative setting. The staff will receive instruction on the intervention, questions and concerns will be addressed prior to induction of the program. The music intervention will be employed preoperatively, can be used intra-operatively pending the type of surgical procedure, and post-operatively and its effectiveness will be measured by using the Visual Analog Scale (VAS). The use of music has been shown in previous research as beneficial in decreasing anxiety and pain, leading to lessened use of pharmacologic interventions. Both staff and patients will be queried regarding their opinions on the intervention and its benefits or limitations. The collected data will be analyzed and the appropriateness, cost-effectiveness, and impact on patient satisfaction will be determined by upper management, surgery co-management, and the anesthesiology group. Further dissemination of the evidence will be made to additional departments, appropriate organizations, and associations.

Patients experience anxiety related to the uncertainty of any surgical procedure. Increased levels of pain and anxiety can lead to negative effects both physiologically and psychologically. Narcotics and sedatives are most commonly administered to alleviate both of the conditions. When given in large amounts and coupled with anesthetic drugs, adverse effects such as over-sedation can occur. This is an adverse effect that can be problematic and costly for not only the patient, but the nurse and facility as well. Respiratory depression and over sedation go hand-in-hand. When over sedation occurs, a patients respiratory effort can be greatly impacted. Over-sedation and other adverse effects can prolong not only the recovery period, but ultimately delay patient discharge. By adding a non-pharmacologic intervention such as music therapy to the treatment plan, the nurse will be able to reduce pain and anxiety, thus reducing the amount of narcotics and sedatives required. This can be used as a nursing driven intervention to be implemented in the pre-operative phase and follow the patient through the post-anesthesia phase. With the appropriate implementation plan, stakeholder acceptance, and resources, music therapy can be a beneficial influence on physiological parameters and psychological state. Music therapy can provide relaxation, lessen muscle tension, and provide a healing influence for patients. Music therapy can have a great impact on not only patient satisfaction, but anesthesia outcomes as well. Literature has shown that music therapy used as a complementary therapy has been proven as an inexpensive way to reduce perioperative pain and anxiety.

Book excerpt: Patients coming into a facility for invasive procedures often times have a high level of anxiety about the procedure and the outcome, (Ghetti, 2012). Based on research studies, the theory of music therapy intervention can reduce the patient's anxiety before and during an invasive procedure, (Bradt, Dileo and Shim, 2013). Music is a welcome distraction proven to help the patient relax, lower blood pressure, heart rate, respiratory rate and cover up environmental noise, (Comeaux and Steele-Moses, 2013). Data reveals some patients need less sedation and pain medication to lessen base-line vital signs and anxiety levels if music therapy is used, (Matsota, Christodoulopoulou, Smyrnioti, Pandazi, Kanellopoulos, Koursoumi, E., and others, 2013). By implementing music therapy in phases through out the facility, education can be audited through the PDCA model, Plan Do Check Act for process improvement. Starting in the preoperative preparation area for patients having heart catheterizations will allow ample control over the study with less staff to train in the start of the project. This plan allows for the implementer to oversee the study with the assistance of the research committee. Patients will be educated on the trial when checking in for the procedure and given an option to participate. A VAS-A, Visual Analog Scale for Anxiety will be used before and after the procedure, (Abend, Dan, Maoz, Raz, and Bar-Haim, 2014). Patient satisfaction scores will be analyzed before and after the trial. The ultimate goal is to improve patient satisfaction scores by reducing level of anxiety, with the added benefit of possibly lowering cost per case. As evidence of less sedation involved allowing the patient to be dismissed sooner, (Bernatzky, Presch, Anderson and Panksepp, 2011).

Book excerpt: Preoperative patient anxiety is an issue that, over the years, has not been adequately addressed as evidenced by the lack of available research in this area. Research that was found shows a link between patients with high anxiety experiencing increased pain and analgesic requirements in the recovery area, increased infection rates, and prolong healing times. This paper will focus on the adult orthopedic patients, who are about to undergo surgery and who are experiencing an increase in their anxiety in the preoperative area. Can relaxation interventions, such as music, television or a distracting game such as playing cards, decrease the amount of stress they are experiencing prior to them going into the operating room? The proposed solution would include upon entering the preoperative area, patients would be offered the option of borrowing a CD player, headphones and a choice of music, headphones for the TV, or a deck of cards to play with, during their preoperative stay. These patients will be given a questionnaire to fill out to rate their anxiety level before and after the interventional therapy, in order to track the success of the interventions. Music and distraction therapy are noninvasive interventions, as well as viable coping mechanisms for the patient. These interventions have the potential, according to several research studies, to reduce a patients negative emotional manifestations lowering their preoperative anxiety. Hospitals are reimbursed within the parameters of today's payer system which is based on the patient's perception of how he/she was treated while in the hospital. Therefore, it seems prudent to provide the patient with a holistic, healing and anxiety relieving approach to their health care.

One of these therapies include the use of music, as literature has found that it reduces anxiety in patients undergoing surgical procedures that require sedative0́ ̄medications. In patients undergoing cataract surgery, the use of music during the procedure has been found to have a positive impact on patients, as their anxiety levels have been reduced. As a result, this quality improvement project focused on the reduction of anxiety with the use of music therapy, in patients undergoing cataract surgery in an outpatient surgical facility. Using the Hamilton Anxiety (Ham-A) scale, the anxiety level of0́ ̄104 patients was measured pre-and post-operatively. Overall, findings indicated that most of the Ham-A scores of the patients had decreased, and these patients also documented in the "Yes/No 1-Question Survey" that music therapy was therapeutic in reducing their anxiety0́ ̄from the pre-operative period to the time of discharge from the post-anesthesia care unit. However, 10 patients had no change in their anxiety level pre- and post-operatively. Book excerpt: Pre-procedural anxiety experienced by patients is a phenomena which is widely recognized, and in some cases treated with sedative medications by mouth. The physical implications of stress and anxiety have been recognized as having a negative influence on intra procedure vital signs and even recovery time (Bringman and others, 2009). These facts play a major role in the holistic view of a patient, and result in stressors described in the Neuman Nursing Theory as an imbalance in the patient09́s internal line of defense (Gonzalo, 2011). Several research studies have indicated a clear correlation between music intervention and a reduction in pre-procedural anxiety (Nilsson, 2008), which is used as the basis for this research study . The experimental group is subjected to instrumental, calming music for half hour prior to their procedure, and natal signs and anxiety levels are measured via the Beck Anxiety Inventory (BAI) questionnaire before and after music. The control group is likewise filling out the BAI questionnaire and having their vital signs measured, but without music therapy. The testing and the results are thoroughly discussed with implicated stakeholders, as this inexpensive yet effective measure to reduce pre-procedural anxiety can be easily implemented in nursing practice.
