Book excerpt: Public discussion of global healthcare issues is dominated by those who believe that top-down, government-driven interventions are the solution to the myriad health problems suffered by people in less developed countries. This thinking is responsible for a plethora of harmful policies, ranging from a drive towards socialized healthcare systems, to calls for the centralization and semi-nationalization of pharmaceutical research and development, to impractical but grandiose UN-sponsored schemes for tackling HIV/AIDS and malaria.In spite of the abysmal track record of top-down approaches, non-governmental organizations and UN agencies continue to promote them, to the detriment of the private sector, economic development, and human health. The resulting politicization of diseases such as HIV/AIDS has led to a diversion of resources away from more easily treatable diseases that affect more people. Meanwhile, cost-effective and simple interventions such as vaccination are being subordinated to other more politically correct diseases.This centralizing mindset has also resulted in many governments in less developed countries attempting to plan and control universal healthcare systems, which has encouraged rationing, inequitable access, and entrenched corruption. It has also seriously undermined the effectiveness of overseas development aid. Moreover, the politicization of diseases such as HIV/AIDS has led to a diversion of resources away from more easily treatable diseases that affect more people. As a result, cost-effective and simple interventions are neglected by donors.There has to date been little public discussion of the role of markets and their underlying institutions--property rights and the rule of law--in improving human health. Thus, this essay explores the extent at which decentralisation has contributed to fighting corruption in the sub-Sahara. Overwhelmed by bad governance, an eyesore of poverty and disease, the relics of prolong corruption and misrule have pushed developing countries in sub-Saharan Africa and their counterparts, the World Bank (WB) and International Monetary Fund (IMF) into political, administrative and economic reforms. Since the 1990s, decentralisation has been a key policy instrument advocated and favoured by governments, donor countries, civil society and international institutions to engender good governance. Many countries in Africa have speedily implemented political, administrative and fiscal decentralisation within the last three decades (Conyers 2007; Dickovick and Wunsch 2014). The primary or perhaps the profound motivation for the wave of decentralisation around the world, particularly in sub-Saharan, is based on the conceptual argument that it offers potential benefits. According to proponents, in a decentralised governance system where power and resources are devolved, services will increase alongside efficiency. Productive and economic growth will inhibit rent-seeking, encouraging downward accountability to promote civic participation in decision-making. This will eventually alleviate poverty and reduce corruption. It is expected that where these goals are achieved, the level of human development index will rise in sub-Saharan Africa. In spite of the hypothetical rationale for adopting decentralised policy, there are several scholars and academics that are pessimistic and cynical about the concept. Tulchin et al. (2004) for instance, argues that due to the complex and fluid nature of decentralisation, it is highly unlikely to determine the actual outcome against expectation. To qualify this statement, Wunsch (2008) writes that evidence of decentralisation across Africa over the years has been frustrating. However, some evidence suggests that there have been improvement in service delivery in certain countries within this region. Although Conyers (2007:27) caution that is it hard to ascertain whether decentralisation contributed to the progress.

Thus we must turn to the overly neglected theoretical work on the political economy of Africa in order to determine how the poverty of governance in Africa is firmly anchored both in Africa’s domestic socioeconomic reality, as well as in the region’s role in the international economic order. Instead of focusing on increased monitoring, enforcement and formal democratic procedures, this book combines economic analysis with political theory in order to arrive at a better understanding of the political-economic roots of corruption in Sub-Saharan Africa.

Several socio-economic factors have been identified as the principal causes of malnutrition; some of these include poverty, lack of education, poor government policies, poor access to healthcare, poor access to hygienic water supply and environmental sanitation, lack of toilet facilities as well as gender inequality and corruption. However, it is important to note that all of these factors are related to each other in one way or another. Thus, in order to mitigate the continued rise in the level of malnutrition in sub-Saharan Africa, a multifocal approach is needed. These factors should not be approached individually in the process of finding a viable solution to the scourge of malnutrition, rather they should be approached as a collective entity in the planning and implementation of government policies.

Viewed as a continent of wars, famines and entrenched poverty in the late 1990s, there is now a focus on “Africa rising†? and an “African 21st century.†? Two decades of unprecedented economic growth in Africa should have brought substantial improvements in well-being. Whether or not they did, remains unclear given the poor quality of the data, the nature of the growth process (especially the role of natural resources), conflicts that affect part of the region, and high population growth. Poverty in a Rising Africa documents the data challenges and systematically reviews the evidence on poverty from monetary and nonmonetary perspectives, as well as a focus on dimensions of inequality. Chapter 1 maps out the availability and quality of the data needed to track monetary poverty, reflects on the governance and political processes that underpin the current situation with respect to data production, and describes some approaches to addressing the data gaps. Chapter 2 evaluates the robustness of the estimates of poverty in Africa. It concludes that poverty reduction in Africa may be slightly greater than traditional estimates suggest, although even the most optimistic estimates of poverty reduction imply that more people lived in poverty in 2012 than in 1990. A broad-stroke profile of poverty and trends in poverty in the region is presented. Chapter 3 broadens the view of poverty by considering nonmonetary dimensions of well-being, such as education, health, and freedom, using Sen's (1985) capabilities and functioning approach. While progress has been made in a number of these areas, levels remain stubbornly low. Chapter 4 reviews the evidence on inequality in Africa. It looks not only at patterns of monetary inequality in Africa but also other dimensions, including inequality of opportunity, intergenerational mobility in occupation and education, and extreme wealth in Africa.

Chapter 1 argues that pervasive poverty and low agricultural p- ductivity are important factors in understanding food insecurity in the region, and broader global processes are examined. This chapter maintains that while poverty undermines individual and household access to suf?cient food through market p- chase, land inequalities, corruption, structural adjustment programs, civil con?ict, HIV/AIDS and the role of the World Trade Organization Agreement on Agric- ture are decisive. The authors argue that achieving food security in sub-Saharan Africa requires policies and actions that are integrated with efforts to reduce poverty, enhance livelihoods and incomes and increase agricultural output, while also paying attention to underlying structural factors that bear on agriculture in the region.
