It is one of the main causes of chronic pain, which makes it a major public health problem. It generates in patients who suffer it a worsening of their quality of life, their psychological well-being, and disability with the consequent incapacity for work. Given that both its etiology and its consequences are complex and multifactorial, postoperative pain must be approached from a multimodal perspective: preventive, pharmacological, nutritional, rehabilitative, nursing intervention, psychological and, in those refractory cases, through surgical approaches or nerve block techniques. This book aims at first to analyze the pathophysiological and psychological bases that underlie its etiology. Next, it aims to carry out an updated review of the multidisciplinary programs for the treatment of postoperative pain that have been carried out and what their results have been. Moreover, the contribution of each of these disciplines in the ambulatory management of postoperative pain will be reviewed. Finally, we want to study the peculiarities of the management of this type of pain in the oncological patient, as well as the personal and socioeconomic consequences and the increase in other comorbidities of this important reason for consultation.

Book excerpt: Section I: Abstract Problem: Thirty percent of patients who have ambulatory surgery describe their pain as moderate to severe on postoperative phone calls in this Northern California Hospital. Patients have expressed dissatisfaction on the topic of subsequent pain on the Outpatient and Ambulatory Surgery-Consumer Assessment of Healthcare Providers and Systems surveys. Patients receive pain management education after recovery via oral and written format, but the content is not standardized. The aim of this project is to decrease the pain experienced by patients from 30% to less than 25% with a secondary goal to increase patient satisfaction with pain education from 10th % to over 20% on OAS-CAHPS. Context: The recovery room has over fifty patient care bays. An average of 25 outpatient surgeries are performed a day. The vast majority of surgeries performed are same day discharge with an average recovery time of ninety minutes. Staff members deliver discharge instructions written by the surgeon which includes pain management. There is variability in content and delivery of pain management education. Unit stakeholders support an educational quality improvement project to address the patient's needs for better pain control and satisfaction with their instructions. Interventions: The American College of Surgeons Safe and Effective Pain Control After Surgery brochure was used to manage the content and delivery of educational material patients received to assess their pain and treat it at home. The brochure meets the Joint Commission requirements of 2018 related to pain education upon discharge (Joint Commission,2017). Inclusion criteria for the intervention group were adult English-speaking patients having scheduled elective surgery with exclusion of eye, ear/nose/throat surgery, add-on cases, and dementia. Staff engagement and buy-in was established with huddles sharing OAS-CAHPS scores and patient postoperative call verbatims. The brochure was presented in the preoperative period for patient to read, and it was reviewed in greater detail in the recovery phase before discharge. A pain score education flyer was added to help patients describe their pain to the nurse on the postoperative call. Measures: Process measures included, increasing the amount of postoperative calls performed, auditing of nursing staff educating with the brochure, and auditing of documentation in electronic medical record. The unit informaticist pulled data from the electronic medical record to support audit accuracy. The two outcome measures included pain statements from the 24- hour postoperative phone calls and the OAS-CAHPS score on the question of subsequent pain delivered by the patient care experience coordinator. Results: The outcome measure results as of June 11,2020 show that although the quantity of patients that complained of moderate to severe pain went down from 30% to 27%. The change was not statistically significant. At the time of this analysis, the OAS-CAHPS score regarding subsequent pain education showed improvement from 10% to 49% for March. Preliminary results for April sit at 46% and May at 99%. Conclusion: There were extenuating circumstances that were in effect at the roll-out of this project. Covid-19 became a global pandemic. The volume of cases went down significantly from the average 25 cases a day to less than 10 a day. Workflows and priorities changed within the unit with a focus on infection control and safety. Based on the results of this project, the pain management educational brochure will be part of every patient's discharge instruction packet pending funding approval. Studies have found that the more education the patient receives about their pain management at home the better they do. They have less complications, report less postoperative pain, and have a better surgical care experience (Sawhney,Wilson,&McGillion,2017).

These numbers have increased considerably for a variety of reasons, and the growing role of prescription drug abuse has been widely recognized. There has also been increasing consciousness of the lack of dedicated and formalized training dedicated to prescription of analgesics, particularly among surgical trainees and practicing surgeons. Recent studies demonstrate that only a small minority of training programs mandates any opioid prescribing education, despite the fact that nearly all programs allow trainee prescription of opioids on an outpatient basis. Therefore, it is not surprising that despite a growing body of literature illustrating the value of opioid alternatives and adjuncts, practicing surgeons in all specialties largely ignore these evidence-based practices and continue to prescribe opioids in situations for which alternatives are proven, safe, and effective. There are many reasons for preparing a concise and practical resource describing evidence-based practices targeting surgeons and surgical subspecialists, including but not limited to: (1) The exponential increase in prescription drug abuse associated with the “opioid epidemic,” as surgeons play an important role in managing perioperative and chronic pain in a variety of practice setting; this issue bears importance from the patient, physician, and policymaker standpoint; (2) With increasing recognition of the role of quality improvement in patient safety initiatives, and literature demonstrating inadequate opioid prescribing education (OPE) in surgical training programs, it is only a matter of time before accreditation bodies including the ACGME incorporate strong recommendations and mandates for OPE; (3) Myriad literature demonstrating the existence of evidence-based alternatives to opioids – All surgeons and surgical subspecialists would benefit from a practical guide concisely laying this literature out. We feel that surgeons of all experience levels may benefit, ranging from the first year surgical intern to an experienced practitioner trying to remain compliant with the evolving opioid regulatory environment. Hence, the appropriate target audience for this text would be for the tens of thousands of current surgical trainees in many specialties, including general surgery, thoracic surgery, otolaryngology, plastic surgery, urology, gynecology, vascular surgery, neurosurgery, and orthopedic surgery. Furthermore, surgeons out of training would be another audience of interest, as there are increasing courses for continuing medical education based on perioperative and chronic pain management. Currently, there are no comparable and competitive guides with these target audiences; all of the existing literature about perioperative and chronic pain is targeted either specifically for patients, anesthesiologists, or pain medicine physicians. This will be the only up to date guide focusing on evidence-based practices for perioperative pain control, and each section will also include information on chronic pain sequelae relevant to each surgical specialty. The editors envision this text being an interdisciplinary endeavor, incorporating surgeons from multiple specialties, anesthesiologists, pain medicine physicians, and palliative physicians as appropriate.

Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
