The primary advantage of POCT is the convenience of performing the test close to the patient and the speed at which test results can be obtained, compared to sending a sample to a laboratory and waiting for results to be returned. Thus, a series of clinical applications are possible that can shorten the time for clinical decision-making about additional testing or therapy, as delays are no longer caused by preparation of clinical samples, transport, and central laboratory analysis. Tests in a POC format can now be found for many medical disciplines including endocrinology/diabetes, cardiology, nephrology, critical care, fertility, hematology/coagulation, infectious disease and microbiology, and general health screening. Point-of-care testing (POCT) enables health care personnel to perform clinical laboratory testing near the patient. The idea of conventional and POCT laboratory services presiding within a hospital seems contradictory; yet, they are, in fact, complementary: together POCT and central laboratory are important for the optimal functioning of diagnostic processes. They complement each other, provided that a dedicated POCT coordination integrates the quality assurance of POCT into the overall quality management system of the central laboratory. The motivation of the third edition of the POCT book from Luppa/Junker, which is now also available in English, is to explore and describe clinically relevant analytical techniques, organizational concepts for application and future perspectives of POCT. From descriptions of the opportunities that POCT can provide to the limitations that clinician’s must be cautioned about, this book provides an overview of the many aspects that challenge those who choose to implement POCT. Technologies, clinical applications, networking issues and quality regulations are described as well as a survey of future technologies that are on the future horizon. In a community hospital, a glucose management quality improvement initiative in the inpatient setting has the potential to improve outcomes for patients, improve quality scores, reinforce evidence-based prescribing practices, and increase knowledge about the patient population. An important step to realizing the benefits of a glucose management quality improvement program is to collect and establish baseline data and to provide that data to physicians in a way that will allow them to measure their performance against their peers and against their own historical data trends.In order to provide physicians and members of the glucose management quality improvement initiative with relevant data, a metric was used called "Glucometrics", developed by Goldberg et al. in 2006. This metric uses point-of-care glucose measurements to provide a score that aims to indicate the level of glucose management of a group of patients across the hospital or in a specific unit or department. The metric is achieved by taking the mean of all glucose measurements in a patient-day and determining how many of the patient-day means fall within a certain range. For this project, this metric was applied to a pilot group of hospitalists at one campus of a two-campus community hospital system. The goal of the project was to provide a proof of concept that metrics like Glucometrics could be made available at a physician-specific level, and that this glucose management data could provide useful information to the physician pilot group. As this project was being developed, the hospital system was also undergoing a technology transformation, which included a new system-wide EHR, and a new analytics program. The current state of information availability at the outset of the project was limited, and physicians often didn't have access to summary data that would provide information about trends in their patients and how they compared to patients across the hospital. This project made a case for providing that type of information to physicians and made recommendations to influence the clinical transformation project.The metric was implemented as a tool and the first data was reported to physicians in June 2013. The resulting report was accepted by the pilot physicians. Additionally, they indicated that they would continue using the report going forward in conjunction with education and process improvements to further the glucose management quality initiative at the community hospital. The data did not conclusively show an improvement in scores over the time period after the report was first given to the pilot group. The results may also have been confounded by other simultaneous initiatives including providing an on-site diabetes resource and education efforts as well as the concurrent technology transformation project. As a result, this report could be seen as a tool for measuring the effect of other aspects of the improvement initiative but may not change behavior in itself. In conclusion, the report will be used long term across the hospital system and will serve as a blueprint for similar improvement initiatives where physicians currently don't have access to global data about their patient population.
