Book excerpt: The Stressed Heart is truly unique in concept and will provide an eXCItmg adventure to the reader no matter what his or her field of expertise and interest. The title, although quite appropriate, does not adequately indicate the range of topics considered or the rational interrelationships among them. Indeed, perhaps the most important point to be learned from the book is that a serious consideration of the response of the heart to mechanical overload, ischemia, or excessive humoral stimuli must include evaluation of each of the topics in the table of contents. The heart responds to stress through alterations in both structure and function. How these changes are brought about is the subject of the initial chapters. These consider first the normal regulation of gene expression in the heart, the rapid response to mechanical overload that leads to both quantitative and qualitative changes in the contractile proteins, and our current understand ing of the signals that might be elicited by stress and alter gene expression. One chapter emphasizes the fact that, regardless of the nature of the stress, the common denominator is a discrepancy between energy requirements and expenditure. The central role of cellular acidosis in initiating the sequence of responses to stress and the possible roles of peptide regulators of transcription and protein regulators of translation are considered in detail.

Although much basic research in virology and immunology can be accomplished within the biomedical domain, biobehavioral disciplines such as behavioral medicine offer more opportunities for the comprehensive approach necessary to confront the AIDS/HIV problem. The editors of this groundbreaking volume suggest that the very nature of this constantly evolving problem encourages an approach to research and intervention/prevention efforts that emphasizes flexibility of response to changing knowledge, patterns of the pandemic, new treatments, and shifts in public opinion and behavior. A major triumph in dealing with this phenomenon would include a bridging of the gap between research and applied efforts, which has been the largest obstacle for progress to date. In this book, such previously uncharted territory is explored, opening a host of new possibilities for dealing with the very real threat of AIDS.

Inspired by the work of the French phenomenologist Maurice Merleau-Ponty, a phenomenological theory of psychosomatics is worked out as an alternative to traditional, biomedical thinking. The patient who presents somatic symptoms with no clearly discernible lesion or dysfunction presents a problem to the traditional health care system. These symptoms are medically unexplainable, constituting an anomaly for the materialistic understanding of ill health that underlies the practice of modern medicine. The traditional biomedical model is not appropriate for understanding a number of health issues that we call “psychosomatic” and for this reason, biomedical theory and practice must be complemented by another theoretical understanding in order to adequately grasp the psychosomatic problematic. This book establishes a complementary understanding of psychosomatic ill health in terms of a non-reductionistic model allowing for the (psychosomatic) expression of the lived body. A thorough presentation of the work Merleau-Ponty is followed by the author’s application of his thinking to the phenomenon of psychosomatic pathology.
