Four implants of each design (big and small platform) were randomizedinstalled.After 12-week of healing period, histomorphometric analyses of the specimens were carried out to measure the crestal bone level values and the tissue thickness in theimplant shoulderportion. Book excerpt: In the past, osseointegration was regarded to be a mode of implant anchorage that simulated a simple wound healing phenomenon. Today, we have evidence that osseointegration is, in fact, a foreign body reaction that involves an immunologically derived bony demarcation of an implant to shield it off from the tissues. Marginal bone resorption around an oral implant cannot be properly understood without realizing the foreign body nature of the implant itself. Whereas the immunological response as such is positive for implant longevity, adverse immunological reactions may cause marginal bone loss in combination with combined factors. Combined factors include the hardware, clinical handling as well as patient characteristics that, even if each one of these factors only produce subliminal trauma, when acting together they may result in loss of marginal bone. The role of bacteria in the process of marginal bone loss is smaller than previously believed due to combined defense mechanisms of inflammation and immunological reactions, but if the defense is failing we may see bacterially induced marginal bone loss as well. However, problems with loss of marginal bone threatening implant survival remains relatively uncommon; we have today 10 years of clinical documentation of five different types of implant displaying a failure rate in the range of only 1 to 4 %.

Book excerpt: The purpose of this study was to compare two approaches for the management of experimental peri-implantitis induced by cotton ligatures placed around porous-surfaced dental implants. Thirty implants were placed in 5 beagle dogs, 3 fixtures on each side of mandible, 4 months after extraction of the mandibular premolars and first molars. After re-entry and abutment connection, a cotton ligature was placed around the neck of each implant and forced apically to disrupt the soft tissue seal of the peri-implant space. Oral hygiene was withdrawn and administration of a soft diet allowed gross plaque accumulation. Periodic radiographic and microbiologic exams were performed. After 6-8 weeks the ligatures were removed and regular hygiene re-instituted. Four weeks later and after commencement of systemic antibiotics, the "rescue" surgical procedures were performed. These were based on GBR principles and done using a split-mouth model design. On one side of the jaw Dynagraft® Putty was used as a graft while on the other side, the same graft material was used along with Capset as a barrier. Results from microbiologic, radiographic, clinical and microscopic assessments revealed that: (i) 'Porphyromonas gingivalis' was detected at all peri-implantitis sites as the disease progressed and at the time of rescue surgery, up to 3 mm of the porous surface was no longer submerged in bone because of the bone loss that occurred; (ii) Assessment of prepared sections using BSEM and statistical treatment of the data indicated that both treatment approaches i.e., Dynagraft + Capset and Dynagraft only, were effective in terms of inducing new bone formation and allowing for re-osseointegration to varying degrees; (iii) Dynagraft + Capset treatment group had significantly better values for the regained bone height and bone ingrowth fraction for defect segment, parameters that reflect the level of re-osseointegration induced by the treatment.

Book excerpt: Autografts (ABG) are the gold standard for alveolar ridge augmentation (ARA), but involve second-site surgical morbidity. The novel BMP bioimplant URISTTM has been designed to be an effective alternative to autografts. The efficacy of URISTTM with 0.5(D1) or 1(D2) mg BMP/unit was compared to ABG or Bio-Oss®. A split mouth design was employed in Beagle dogs. Caliper and Cone Beam CT analyses were utilized to compare dimensional changes in the alveolus. After 12 weeks, dental implant were placed and bone cores analyzed by histology. Dogs that did not have dental implant placement were analyzed for the effect of prolonged healing time on dimensional bony changes. URISTTM had comparable dimensional changes in ARA to the gold standard autograft, with some statistically significant increases in width. All grafted sites had successful primary stability of dental implant placement. Histology showed no clear differences in the character of new trabecular bone deposited among grafted sites.

Book excerpt: In their contribution to the first edition of this Handbook, entitled "The Teeth," LEHNER and PLENK (1936) discussed the tissues constituting the "perio dontium" rather briefly. In contrast to the detailed paragraphs dealing with, for example, enamel and dentine, the section (about 40 pages and 20 illustra tions, mostly drawings) devoted to periodontal tissues failed to provide a factual review and summary of the contemporary knowledge and latest developments in research on the various components of the periodontium. Instead, much of the text was an attempt to arrive at conclusions from often purely semantic speculations, playing the various schools of thought against each other, provid ing arguments in favor of the authors' views and arguments for the feasibility and probability of accepting or rejecting the often diverse opinions, while the reader was referred to the already existing literature for factual details. Since 1936, however, factual details ofthe structural biology of the periodon tal tissues, i. e. their development, structure, function, and physiology, have been greatly extended and have been internationally accepted. With much less opin ionated belief to cope with, this knowledge has formed the solid foundation upon which diagnosis, prevention, and treatment in the fields of clinical perio dontology, modem orthodontics, and re- and transplantation procedures of teeth have been built.
