Download or read book Transanal Minimally Invasive Surgery TAMIS and Transanal Total Mesorectal Excision taTME written by Sam Atallah and published by Springer. This book was released on 2019-04-23 with total page 517 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book captures cornerstone developments in a new body of knowledge and provides an expert resource on a “hot topic” in rectal surgery. Transanal minimally invasive surgery (TAMIS) was designed for local excision of select rectal neoplasms, however soon it became realized that the TAMIS technique could be used for applications beyond local excision, most notably for transanal total mesorectal excision (taTME). This new operative technique has revolutionized our approach to the distal rectum by allowing for improved access, especially in obese male patients with an android pelvis, and by minimizing abdominal wall access trauma. The endpoints of improved oncologic resection, as defined by mesorectal envelope completeness, negative circumferential resection margins, and negative distal margin, are assessed. This book details controversies, pitfalls, and future directions of taTME and TAMIS. Chapters are authored by those on the forefront of innovation with TAMIS and taTME, and each is considered an authority on the topic. Transanal Minimally Invasive Surgery (TAMIS) and Transanal Total Mesorectal Excision (taTME) is a must-have reference for surgeons who are performing this operation and fellows in training who want to completely understand the various nuances of TAMIS and taTME.

Book excerpt: After World War II, Western Europe became closely linked to the United States--economically through a variety of associations within the Atlantic Community, and militarily through NATO. This volume stresses the strategic importance of Western Europe for the United States. It provides detailed surveys of the background and preparedness of the NATO defense forces and the forces of Austria, Switzerland, and other countries of strategic importance. Each chapter provides a general outline of military developments since 1945, including such topics as: the relationship between armed forces and society; recruitment practices; armaments; organization; relations with NATO; and future projections. The authoritative series of descriptive, historical, and analytical essays in this volume makes it an essential resource for defense specialists, policymakers, and scholars of Western Europe.
Download or read book Contract Management written by United States. Government Accountability Office and published by . This book was released on 2009 with total page 41 pages. Available in PDF, EPUB and Kindle. Book excerpt: Federal agencies have used time-and-materials (T & M) contracts to purchase billions of dollars in services. These contracts are risky because the government bears the risk of cost overruns. Effective February 2007, the Federal Acquisition Regulation (FAR) was revised, pursuant to a statutory change, to allow T & M contracts to be used to acquire commercial services under FAR Part 12, which uses a streamlined procurement process. Certain safeguards were included in FAR Part 12, including a requirement that contracting officers prepare a detailed determination and findings (D & F) that no other contract type is suitable. Based on a mandate to review the use of T & M contracts for commercial services, we assessed (1) agencies' reported use of such contracts and what they acquired, (2) the degree to which agencies complied with the new safeguards, and (3) the applicability of the safeguards to General Services Administration (GSA) schedule contracts. GAO reviewed contracts and orders at DOD and civilian agencies and spoke with contracting officials. GAO recommends that OFPP take steps to clarify the FAR regarding labor-hour contracts and to explicitly apply the Part 12 safeguards to the GSA schedules program, and that it provide guidance to contracting officials on the Part 12 D & F. In oral comments, OFPP agreed with our recommendations. The other six agencies in our review also concurred or had no comment.

Download or read book Central Governments Management of Service Contracts written by Great Britain. National Audit Office and published by The Stationery Office. This book was released on 2008 with total page 44 pages. Available in PDF, EPUB and Kindle. Book excerpt: This NAO report (HCP 65, session 2008-09, ISBN 9780102954487), examines how well central government organisations are managing their service contracts, assessed against the good practice framework for contract management. A further examination was done into the effectiveness of the Office of Government Commerce in supporting central government to improve contract management. The NAO has focused on contracts for information and communication technology, facilities management and business process outsourcing, where the contract had been signed and the service was up and running. In the 2007-08 period, central government spent over £12 billion on service contracts primarily in the areas of information and communications technology, facilities management and business process outsourcing. In total the NAO estimates that £240 million was spent on managing service contracts in the period 2007-08. Delivery of public services, protection against service failure and achievement of value for money are all dependent on effective contract management. The NAO has set out a number of findings and recommendations, including: that contract management is not always accorded the priority it deserves; that less than half the organisations surveyed had an individual with overall responsibility for contract management; that some contracts had taken several years before a proper system of management was actually in place, including resources and performance measures; that one-quarter of comercial directors/heads of procurement rated the level of resources allocated to contract management as poor; that central government do not routinely test their service contracts and good practice risk management practices are not being consistently applied. For the Office of Government Commerce the NAO found that: limited guidance is available on contract management; that central government organisations identified a need for better training for their contract managers; that no cross-government contract management community exists and that monitoring and managing major suppliers had focused mainly on the IT sector.
