Available in PDF, EPUB and Kindle. Book excerpt: Since coronary angioplasty was first practiced by Andreas Gruntzig in 1977, it has rapidly developed into a technique widely used on patients with chronic and acute coronary heart disease. The meeting described in this book was held under the auspices of the working group of our national society, chaired by Prof. Kaltenbach, Frankfurt, and by Prof. Meyer, Mainz, in cooperation with Stanford University. It is an attempt to present various cardiologists' appraisals of the current position ofPTCA in clinical medicine. PTCA is far from easy, as its mechanism is critically balanced between success and fail ure. Therefore the experience of more than one hundred cases is generally regarded as ne cessary for cutting down complications and achieving a high rate of sli'ccess. Furthermore, success and complications are a result not only of technical expertise, but also of patient selection. The emphasis of the workshop was on discussion of some unsolved problems and open questions such as: - What are the reliable indication guide-lines? - What are the established guide-lines for deciding, once a complication occurs, between operative and non-operative treatment? - What are the indications and limitations for the combined use of thrombolysis with PTCA in acute myocardial infarction? - When and at what intervals should PTCA be applied after successful lysis? - Who are the candidates for dilatation of main stem stenosis? - When should one attempt to open occluded arteries and which technique shows the most promise?

Book Transluminal Coronary Angioplasty and Intracoronary Thrombolysis

This book was released on 2012-12-06 with total page 447 pages. Available in PDF, EPUB and Kindle. Book excerpt: This volume contains the proceedings of the Fourth International Symposium on Coronary Heart Disease, held in Frankfurt/Federal Republic of Germany. In 1970, when the fIrst symposium was held, the whole range of diagnostic and therapeutic measures was covered, but since then interest in coronary heart disease has increased so considerably that the fourth symposium was restricted to only two therapeutic aspects, angioplasty and intracoronary thrombolysis. It can be noted with pleasure that European cardiologists, namely Andreas Griintzig and Peter Rentrop, have made essential contributions to the applicability of these modes of treatment. When angioplasty was fIrst discussed at the Third International Symposium (Coronary Heart Disease, Stuttgart, 1978) a great deal of astonishment was expressed. Today the procedure has gained an important place in the treatment of angina pecto ris. The proportion of candidates for coronary surgery in whom angioplasty can be attempted instead of operation is around 10%, in certain subgroups even higher. The most remarkable fmding of our 5-year experience with angioplasty are the extre mely good long-term results. Recurrences happen in about 15%-20% of cases within the fIrst 3 months; after this period of time, recurrences are very unusual and mostly due to progression of the disease at other sites. Angioplasty has thus also contributed to our understanding of the dynamics of the atherosclerotic process.

Available in PDF, EPUB and Kindle. Book excerpt: Percutaneous Transluminal Coronary Angioplasty is presently the widest used non-surgi cal method for the treatment of stenotic lesions in coronary artery disease. Continuous development of the procedure and equipment has made complex and multi-vessel-inter ventions possible and has led to other techniques of intravascular angioplasty. In reviewing current results, this volume pays particular attention to causes, incidence, circumstances, recognition, treatment and outcome of complications of PTCA; it also considers restenosis, which still occurs at an unsatisfactory high rate after primarily successful procedures. To deal with these topics, experts on PTCA met to discuss their experiences and insights into peculiar aspects of complications in PTCA. The discussion was subdivided into: Recognition of complications; Complications in new interventional techniques; Management of complications; and Restenosis after angioplasty. The question of whether angiographic and pathological characteristics of sclerotic lesions allow the identification of complication-prone lesions in advance is discussed in the first chapter. One paper deals with the state of the art of digital cardiac imaging, which enables the operator to have an early knowledge of and adequate reaction to possible complications occurring during angioplasty. Special attention is directed to the so-called "high-risk"-PTCA, detailing the clinical variables, in order to have better knowledge about patients with a higher probability of unsuccessful or complicated procedures.

This book was released on 1986-10-31 with total page 218 pages. Available in PDF, EPUB and Kindle. Book excerpt: Since the introduction of coronary angioplasty in 1977, this procedure has gained a steadily increasing position in the treatment of coronary artery obstmction. From the available evidence it can be estimated, that this thera peutic tool will get even more additional momentum of many ten-thousands of patients to be treated in the next few years, due to a growing fraction of patients who are candidates for this intervention. Information about the indications, benefits and risks of coronary angioplasty is accumulating rapidly in addition to publications about refinements of the technique itself. Recently, a number of investigators have realized that coronary angioplasty is not only a therapeutic tool, but can, during the procedure, be used as a source of diagnostic information. When the catheter is placed in a coronary artery obstruction, inflation of the balloon produces transient myocardial ischemia. Before, during, and after this period of severe ischemia, studies of the perfor mance of the myocardium at risk can be carried out. The fact that therapeutic coronary angioplasty is carried out in a cardiac catheterization laboratory which is by definition optimally equipped for the measurement of hemodynamic parameters, has probably also contributed to the effectuation of these investigations. The combination of hemodynamic and biochemical parameters with morphological information from the coronary angiogram can be utilized for the quantification of myocardial involvement and the success of coronary dilatation with angioplasty. Studies of interactions with pharmacological substances are also feasible and informative.

Book The Role of Percutaneous Transluminal Coronary Angioplasty

Download or read book The Role of Percutaneous Transluminal Coronary Angioplasty written by G. Goodman and published by . This book was released on 1992 with total page 118 pages. Available in PDF, EPUB and Kindle. Book excerpt: PTCA ia a prime example of rapid technological advances affecting the diagnosis and treatment of cardiovascular disease. The main clinical problems for which PTCA can be performed are: chronic stable angina; unstable angina; acute myocardial infarction; asymptomatic disease (where the only evidence of ischemia is determined by diagnostic testing); repeat PTCA for restenosed lesions; stenosed CABG grafts. Since it was first tried in 1977, the use of PTCA has increased dramatically, especially in western nations. The use of CABG, which was introduced a decade earlier, has continued to rise substantially. The large and costly increases in the use of these procedures has occurred despite limited understanding about their relative health, economic, and social merits. Although some well-founded evidence is available concerning the relative value of the alternative treatments for coronary artery disease, much of the decision making concerning the use of these treatments for particular types of patients is only empirically based.

Book excerpt: The third essay addresses the substitutability of percutaneous transluminal coronary angioplasty (PTCA) for coronary artery bypass graft surgery (CABG) across the population of Pennsylvania from 1994--2004. This study builds on previous work (Cutler and Huckman, 2003) and suggests that PTCA is a substitute for CABG in the marginal population (measured by severity) and that the introduction of a follow on technology, the coronary stent, may have been a driver of much of the diffusion of the use of PTCA as an alternative to CABG.

The Hong Kong Hospital Authority reported nearly 7000 patients suffered from acute myocardial infarction (AMI) in year 2010 to 2011. Percutaneous transluminal coronary angioplasty (PTCA) is a minimal access surgical treatment for coronary artery disease but studies have shown that patients experienced different levels of anxiety before and during PTCA which led negative impact to the patients. Music can be regarded as a safe, cost-effective therapy to reduce one's anxiety level. It can be carried out by nurses without any specific technique. However, there was no systemic review for using music therapy to patients undergoing PTCA. Therefore, this dissertation aims to evaluate the best available evidence on using music therapy for patients undergoing PTCA. Four electronic databases, Medline, CINAHL, Embase & PsycINFO, were searched for studies to investigate the efficacy of music therapy in patients undergoing PTCA. There were seven studies were eligible with data extracted and quality assessment performed by the critical appraisal skill programme (CASP) checklist. Four studies were graded as high quality, which consistently demonstrated a statistically significant more reduction in the anxiety level of patients who received music therapy either before and/or during PTCA than those who had no music therapy. Consequently, an evidence-based guideline of using music therapy for patients undergoing PTCA was developed according to the guideline development process of SIGN (2010). The use of music therapy is transferable and feasibly in the local setting with little manpower concern. The total cost estimated for running the innovation for a year was HKD 83,775 which was considered as cost-effective to reduce patients' anxiety level and avoid associated adverse events. A 14-months programme including communication with the stakeholders, pilot testing, staff training and clinical application of the proposed music therapy was designed. Qualitative and quantitative data on patient, healthcare provider and system outcomes would be accounted throughout the programme. The effectiveness of the guideline would be determined by the reduction of patient's anxiety level by using the Chinese version of State-Trait Anxiety Inventory (STAI) (Shek, 1993). Moreover, healthcare provider outcome would be evaluated by questionnaire and the expenditure of the proposed music therapy would be monitored closely.
