Book excerpt: Accurate assessment of environmental hazards and related risks is a primary prerequisite for effective environmental health protection, at both the individual and collective level. National and regional policies on environmental health need to be guided by knowledge about the risks to the populations involved; as the Environmental Action Plan for Europe notes, 'priority setting requires the comparative assessment of risks to health of different environmental factors against the cost of controlling them.' In recent years this has assumed particular importance, for with the encouragement of the World Health Organisation (WHO), all countries in Europe are committed to producing National Environmental Health Action Plans (NEHAPs), which will define priorities and targets for environmental health and the actions needed to achieve them. Reliable information on risks is clearly fundamantal to this process. Individual risk assessment is no less important in this context. Much of the responsibility and capacity to improve public health lies ultimately in the choices (e.g. about diet, smoking, alcohol consumption, sexual activities, sporting activities, travel mode, place of residence and occupation) which we make as individuals. If we are to improve and protect our own health, therefore, and in so doing play our personal role in achieving the targets set by these Plans, we need to be guided by a clear understanding of the risks involved.

Book excerpt: Given that for centuries, the standard tool to understand diseases in tissues was the microscope and that its major limitation was that only excised tissue could be used, recent technology now permits the examination of diseased tissue in vivo. Optical coherence tomography (OCT) has promising potential when applied to coronary artery disease. OCT has the capability to identify coronary plaque and to distinguish between plaques that are stable and unstable. If the plaques are stable then OCT can direct percutaneous intervention (angioplasty or stenting). Optical coherence tomography is a light-based imaging technology that allows for very high resolution imaging in biological tissues. It has been first applied in ophthalmology, where it soon became the golden standard for the assessment of (epi-) retinal processes. The unique imaging capabilities have raised the interest of researchers and clinicians in the field of cardiovascular disease, since OCT offers unique possibilities to study atherosclerosis pathophysiology in vivo. With over 1.1M Americans having a heart attack this year because of unstable plaque rupture, OCT may have an increasingly important role in the early diagnosis of coronary artery disease. Many of these important initiatives involve the US Department of Energy (DOE) and the Ministry of the Russian Federation for Atomic Energy (MINA TOM). Currently, collaboration is under way which involves reactor safety, the disposition of fissile materials from the weapons program, radioactive waste disposal, and the safety of nuclear warheads. Another fruitful area of interchange resulted from the radiochemical storage tank accident at the site of the Siberian Chemical Compound at Tomsk-7 in 1993. DOE and MINATOM agreed to meet and exchange information about the accident for the purposes of improving safety. A meeting on the Tomsk tank accident was held in Hanford, Washington in 1993, followed by a second meeting in st. Petersburg, Russia in 1994 in which the agenda expanded to include radiochemical processing safety. A third exchange took place in 1995 in Los Alamos, New Mexico, and additional papers were presented on nonreactor nuclear safety. Following a planning session in 1996 in Seattle, Washington, it was decided to hold a fourth technical exchange on the broader subject of nuclear materials safety management. Through a grant from the North Atlantic Treaty Organization (NATO) Disarmament Programme, the meeting took place on March 17- 21, 1997, in Amarillo, Texas as a NATO Advanced Research Workshop (ARW) through grant no. DISRM 961315.
