Book excerpt: Prior research evidence that has suggested that regional variation and socioeconomic barriers in breast cancer treatment remain substantial problems for patients across the nation. The purpose of our project was to characterize national patterns in the treatment of early invasive breast cancer in older women with incident disease. We specifically sought to characterize disparities in care and regional variation in treatment patterns. Methods. We sought to apply a novel resource, comprehensive national Medicare claims data, to study disparities in care and outcomes in women with breast cancer. We calculated national and state-by-state absolute and standardized utilization rates of radiotherapy (RT) and chemotherapy in patients with early invasive breast cancer treated with conservative surgery (CS); analyzed the modifying effect of race on RT utilization; and evaluated the utility of claims-based covariates in predicting breast cancer stage. Multivariate logistic regression was used to model these outcomes. Findings. Significant regional variation in utilization of breast cancer treatment existed in our cohort of older women diagnosed with invasive disease, even after standardization for patient and disease characteristics. In addition, significant racial disparities in care existed, with non-white women significantly less likely than white women to receive RT after CS, despite this treatment modality generally considered standard treatment. Conclusions. Our research adds to the existing literature by providing the first comprehensive national sample to address these study questions. Our future research will extend on our current findings by determining whether these variations in breast cancer treatment also affect outcomes, such as cancer recurrence, mortality, and costs of cancer care.

Book excerpt: Little is known about what constitutes appropriate care for older women with breast cancer. Extending work begun as part of an National Cancer Institute-funded project, we are examining whether variations in care received by older women affect short-term psychosocial and clinical outcomes. Our specific aims are: (1) To describe patterns of adjuvant hormonal and chemotherapy in older women, and factors associated with receipt of these therapies; (2) To characterize and quantity the breast cancer-related care received by older women during the early years following diagnosis; and (3) To determiine the effects of ongoing breast cancer care on patients' quality of life. We are conducting a longitudinal observational study of a cohort of 350 women> 55 years of age diagnosed with stage I and II breast cancer between October 1992 and December 1995 at five sites in Boston, Massachusetts. Women are interviewed annually to obtain information about health and personal characteristics. Medical record abstracts are performed annually to gather information about treatments received, tests performed, and disease recurrences. Descriptive and multivariate analytic techniques will be used to identify patient and provider characteristics associated with variations in care received and the effects of these variations on patients' quality of life.

Book excerpt: The results of randomized trials evaluating the use of early or adjuvant systemic treatment for patients with resectable breast cancer provide an eloquent rebuttal to those who would argue that we have made no progress in the treatment of cancer. Many of the tumors that we have been most successful in curing with chemotherapy and other newer forms of treatment are relatively uncommon. In contrast, breast cancer continues to be the single most common malignancy among women in the western world, is increasingly a cause of death throughout Asia and Third-World countries, and remains one of the most substantial causes of cancer mortality world wide. The use of mammography as a means of early detection has been shown to reduce breast cancer mortality by 25-35% among those popu lations in which it is utilized. The use of adjuvant systemic treatment in appropriate patients provides a similar (and additional) reduction in breast cancer mortality. Few subjects have been so systematically studied in the history of medicine, and it seems fair to conclude that the value to adjuvant systemic therapy in prolonging the lives of women with breast cancer is more firmly supported by empirical evidence than even the more conventional or primary treatments using various combinations ofsurgery and radiotherapy.

The authors tested the hypothesis that being married results in better physical and mental health outcomes for chronic disease patients (N = 1,817) by increasing social support. They modeled health outcomes one year later, controlling for initial health status. Cross-validation studies of two random halves of the sample supported an indirect effect of marital status on mental health through social support, but did not support a relationship, direct or indirect, of either marital status or social support with physical health outcomes. In addition, specific types of functional support were not differentially predictive of mental health status.
