Chinese immigrant children have higher decayed and filled primary teeth than their US counterparts. There is limited information regarding Chinese American immigrant families' oral health beliefs and practices, which could influence their children's oral health outcomes. This study generated new knowledge about parental oral health beliefs and practices toward early childhood caries among Chinese immigrant parents toward the long-range goal of improving immigrant children's oral health. Aims. The overall goal of this cross-sectional descriptive study was to describe Chinese immigrant parents' Explanatory Model of dental caries (etiology, onset of symptoms, pathophysiology, course of caries, treatment) and the impact of Chinese culture, Chinese health beliefs and behaviors on parents' oral-health care of their children. Methods. Purposive and modified respondent-driven samplings were used to recruit Chinese immigrant parents of children aged 0-6 years through Chinese immigrant agencies and participant referrals in King County, Washington. Participants completed in-person, semi-structured interviews that included perceived influencing factors in order to capture cultural and personal perspectives about early childhood caries and children's oral health care. Interviews were audio-recorded, transcribed verbatim in Chinese and analyzed using content analysis. Results. Forty-six parents participated in the study. Parents believed the causes of dental caries included diet and oral hygiene habits, biological and somatic factors (inheritance, genetic, inborn and transmission issues), children's biting fingers habit, and lack of routine dental visits. They held different understandings on the age of onset for dental caries. Most parents recognized the onset of disease through dental examinations, children's toothaches, and color changes on their children's teeth. Parents were familiar with dental caries through personal experiences and experiences with their friends' children. Parents believed childhood caries had the potential to have a short-term or lifelong negative impact on their children. Most parents would immediately seek professional help for caries treatment, but indicated that information about dental caries and its prevention was needed from dental professional. Furthermore, results from the analysis using a framework derived from a critical analysis of Chinese culture revealed that 11 concepts were reflected in parents' oral-health beliefs and practices. Parents believed that they: (1) had an obligation for their children's oral care; (2) valued their children's training in oral care; (3) expected children to be independent in tooth brushing; (4) allowed children to receive sweets as gifts from relatives; (5) expressed fatalism regarding children's dental caries; (6) believed kinship contributed to children's dental caries; (7) obeyed dentists' authority; (8) avoided confrontation with dentists and family members; (9) demonstrated group orientation related to friends' dental knowledge and advice; (10) valued face related to children's physical facial appearance; and (11) were concerned dental caries would affect their children's school achievement. Results from the analysis using a framework derived from a critical analysis of Chinese health beliefs and behaviors revealed that eight domains were related to their oral-health beliefs and practices, including (1) belief regarding primary teeth; (2) belief regarding tooth worms; (3) belief regarding gum bleeding; (4) belief regarding sugary foods; (5) belief regarding a balanced diet; (6) belief regarding self-care; (7) belief regarding the need for dental care services; and (8) belief regarding tooth extraction. Implications. Explanatory Model of dental caries could be applied to assess parental beliefs and practices towards childhood dental caries. Understanding of Chinese culture, health beliefs and behaviors could be helpful in developing oral-health interventions in relation to family needs. Caries prevention intervention is needed to facilitate parents' daily assistance roles in children's oral hygiene behaviors, as well as healthy diet habits. The knowledge served as a foundation for developing a conceptual framework for designing culturally relevant family-focused interventions to improve immigrant children's oral health.

Book excerpt: The aim of this collection of papers is to provide the reader with a cogent understanding of the role of evidence in the development of social or community-based interventions to promote inclusion oral-health and reduce oral health, health, and psychosocial inequities. In addition, this material will include various methods used for their implementation and evaluation. At the outset, the reader will be offered a working definition of inclusion oral-health, which will be modelled on the work of Luchenski et al. [1]. The interventions described are theoretically underpinned by a pluralistic definition of evidence-based practice [2] and the radical discourse of health promotion as postulated by Laverack and Labonte [3] and others [4,5]. This Special Issue will consist of eight papers, including an introduction. The first three papers will examine the various sources of evidence used to transform top-down into bottom-up community-based interventions for people experiencing homelessness; people in custody and for families residing in areas of high social deprivation. The final four papers will report on the implementation and evaluation of social or community-based interventions. This collection of research papers will highlight the importance of focusing on prevention and the adoption of a common risk factor agenda to tackle oral health, health and psychosocial inequities felt by those most excluded in our societies.

Abstract: The last oral health survey conducted by the government of Hong Kong in 2001 found that dental caries was common among the kindergarten children and that most of their dental caries was untreated. The overall aims of this research project were to find out the oral health status of the kindergarten children in Hong Kong and the outcomes of providing a private practice based dental service programme to these children. To achieve the first aim, an oral health survey was conducted in 2007. Through stratified simple random sampling, 12 kindergartens in Hong Kong Island were selected. All children attending grades 1 to 2, aged 3 to 5 years, in the kindergartens were invited to participate. Children with parental consent were clinically examined by one of two calibrated examiners. A questionnaire on the child's demographic background and oral health related behaviours was completed by the parents. Response rate of the survey was 89% and 1,343 children were examined. Their mean age was 52 months (SD=7) and 51.5% were boys. Around one third (35.3%) of the children had experienced dental caries. The mean dmft score was 1.5, increasing from 1.2 at age 3 to 2.0 at age 5 (ANOVA, p=0.016). Soft, cavitated caries (dt) accounted for 90% of the dmft score. The prevalence and severity of dental caries in the children were associated with the children's socio-demographic background, and their dietary and tooth-brushing habits. To find out the feasibility of implementing a private clinic based dental service program for the preschool children a total of 245 children attending grade 1 in 9 kindergartens were invited to participate. Baseline information of the children was collected through a clinical examination and a parental questionnaire at April 2007. After baseline data collection, the study children were randomly allocated into the test or the control group. Children in the test group were offered free dental services at regular intervals in 3 private dental clinics. Evaluation took place in July 2009 after the completion of the 24-month intervention programme through conducting clinical examinations and a parental questionnaire. After 24 months, 88% of the control and test group students remained in the study. The mean dental appointment attendance rate of the test group children was 53%. Individualized instructions on oral hygiene were provided to 82% of the children and 52% of the children had received topical fluoride application. Treatments were provided to more than 30% of the children and the mean number of dental fillings placed is 1.1. The mean total running cost per child was around HK$100 a year. The kindergarten children who were covered by the study dental service programme had better oral health than that of the children who were not. The study programme was effective in preventing development of new dental caries and in reducing the amount of untreated caries among the test group children. However, it was not effective in improving the oral health related behaviours and oral hygiene status of the children. The study programme was acceptable to the kindergartens, the parents and the children by the satisfaction study. Consumer satisfaction was reasonable high. It is considered as a major public health problem affecting pre-school children and it is the most common dental disease which affects 28% of children within two to five years of age. ECC not only induces pain and discomfort, but can also affect communication, nutrition status, learning abilities, speech and quality of life, which may progress into adulthood and pose a heavy burden on the healthcare system in long-term. In Hong Kong, over 50% of children were affected by dental caries in 2001 but over 70% of children had never been to a dentist for a regular check up at age five. Thus, much of the tooth decay was remained undetected and untreated. Dental caries can be preventable and achievable. Collaboration between families, early care and health care professionals is required to promote effective oral health care. Numerous studies have found that educational programs and workshops are effective in promoting oral health and can provide children a lifelong opportunity to be free from preventable oral disease. In order to promote oral health to achieve the mission of the Department of Health in Hong Kong- at least 65% of 5 years old children are free from caries by the year 2020, an evidence-based guideline for a community-based programme in oral health education was developed in the proposed setting after a critical appraisal of the reviewed evidence. The comprehensive intervention plan, including communication plan with stakeholders, training of staffs and pilot testing will be carried out to facilitate the implementation of the innovation. The oral health programme will be evaluated for its effectiveness in achieving the patient outcomes, health providers' outcomes and system outcomes in the proposed settings. Abstract: Introduction: Oral diseases such as dental caries and periodontal diseases are among the most common diseases in Hong Kong. Family as a basic societal unit has an important role to play in shaping the individual member's behaviours and health status. However, most dental research studies focus on modifiable risk factors of individuals rather than those of a family. Objectives: 1. to describe the oral health behaviours and oral health status of family members in a selected sample of families in Hong Kong; 2. to assess the relationships between oral health behaviours and oral health status amongst the family members; 3. to assess the influence of socio-economic factors, lifestyle factors and oral health knowledge, attitudes, and behavioural factors on oral health status of the spouses; and 4. to assess the influence of parental factors on the child's oral health behaviours and status. Methods: A cross-sectional survey using a combination of a random household and a purposive sampling was conducted. The study population was 5 to 7-year-old children and their parents in Hong Kong. A clinical examination and a questionnaire survey were conducted on the core family members (parents and children) of the recruited families. Structural Equation Modeling (SEM) was employed to test the hypothesized multivariate models which tried to investigate the complex relationships among different risk factors and oral health status. Results: A total of 432 families with targeted children, 373 fathers and 424 mothers were recruited. The mean DMFT/dmft score of the father, the mother, and their children were 7.2, 6.2, and 2.6, respectively. Around half (52%) of the fathers and one-third (35%) of the mothers had periodontal probing pocket(s) deeper than 3mm. In the structural equation models, strong positive correlations were found between the oral health behaviours of fathers and mothers (∅=0.98, pConclusion: Oral health behaviours and status are correlated among family members. Children's oral health status is affected by their oral health behaviours, which may be affected by parents' socio-economic status, oral health knowledge, attitudes, and behaviours. It has become necessary due to the unacceptable incidence of dental caries (cavities), particularly in infants and young children that seems to continue unabated from one generation to the next. Our greatest concern is the relatively high incidence of nursing bottle caries in the inner city which appears to be directly related to the feeding practices of mothers who are ignorant of dental disease processes. Moreover, the prevailing view that baby teeth are unimportant has caused many parents to neglect this primary dentition and hope for better prevention measures when the permanent teeth take their place. Unfortunately, the bad dental habits learned during these years tends to remain even after the teeth change. Additionally, too many adults see the dental office as a repair shop, only to be searched out when there is a crisis in the mouth. The goal of this publication is to increase awareness of the importance of baby teeth and to provide a literary resource on the role of expecting mothers in dental disease prevention. Furthermore, proper application of this information will ensure a better quality of life for their children, while reducing the financial strain on public health services. It is my intention to make all future editions more pictorial as I gather photographs from my practice.
