Book excerpt: Although the results of fracture repair have markedly improved over the past century, a significant proportion of orthopedic surgery still involves the reconstruction of bony defects and the stimulation of bone healing through the application of different bone graft materials. With advancements in the production of exogenous bone morphogenic proteins, mesenchymal stem cell harvest, and osteoconductive scaffolds, the requirements for vascularized bone have been reduced but not supplanted. Vascularized bone graft still remains an ideal source of bone replacement and more recent technical advancements have allowed for the transfer of viable growth plates and the restoration of blood flood to areas injured by avascular necrosis. Within this issue many of the pioneers of free microvascular bone transport provide their own historical perspective and long-term results in the treatment of difficult bony problems. In addition, more recent applications of vascularized bone grafts for the treatment of problems within the wrist, spine, and long bones are reviewed. Finally, we come full circle and examine Meekren's xenografting procedure again and the recent developments within the frontier of vascularized xenograft and allograft bone transfer.

The biological behavior of the bone-implant interface is crucial for the long term survival of the artificial joint. Basic scientists and the industry are continuously searching for new implant fixation mechanisms and improved materials. There is an accumulation of a great amount of basic science data (both biological, material and mechanical) related to the incorporation or loosening of the bone-implant interface. However, basic science data does not always translate to satisfactory clinical application, and orthopaedic practitioners often wonder which piece of information is clinically useful. A further problem is that basic scientists often speak their own scientific language and may not fully appreciate common clinical practice needs. In this textbook the biological and mechanical mechanisms of implant incorporation and loosening will be presented. All new data concerning materials and methods for incorporation enhancement will be critically analyzed. Data useful for clinical application will be stressed. J. Kingma. Amongst other problems, reconstructive surgery with the aid of bone-grafting captured the interest of both of us. In vol. XII, fasc. 3 (1960) of the Archivum of calf bone preserved by refrigeration was Chirurgicum Neerlandicum Kingma wrote: 'The use introduced into surgery in the Netherlands by Meiss in 1951. After the appearance of Meiss' reports on his favourable results, many other surgeons wished to employ this method and in order to cope with the numerous requests for material, a national bone transplantation service for calf bone was in stituted in 1952 under the patronage of the Netherlands Red Cross with Kingma as its medical direc tor'. In 1953 we considered homogenous graft to provide a better 'biological intermedium' than calf bone graft. Consequently we developed a bone-bank for homogenous grafts in our department from that date. After 18 years of experience we have been able to demonstrate clinically and radiologically that in some cases a 'biological fixation' can be maintained with the homogenous graft during the recon struction by 'graduated' function. Kingma controlled the results and arranged the data."
