Book excerpt: Physical activity can help promote long term health benefits in aerobic fitness, blood lipid levels, blood pressure, body composition, glucose, metabolism, skeletal health, and psychological health (Sallis, Prochaska, & Taylor, 1999) to help sustain a healthy lifestyle. Children and adolescents should participate in 60 minutes or more of physical activity [PA] each day (Prevention, 2010). Within this subgroup there has been an increase in obesity and a low enrollment in physical activity, especially in low income communities. This suggests that children and adolescents who participate in organized sport, were more active than those who did not participate (Marques, Ekelund, & Sardinha, 2015). Organized youth sports are an alternative method for preadolescents to reach those 60 minutes or more of physical activity. This project is an overview of organized youth sports and how community program involvement can increase physical activity in preadolescents in low socioeconomic class. This project will identify the exercise barriers in a low socioeconomic class and the reasons behind low enrollment in organized youth sports. After identifying the barriers, studies will then be analyzed to help break down some of these barriers.

Children and adolescents who participate in organized sport have a higher level of physical activity later in life and lead healthier lives in their adulthood. Because physical activity in youth is often executed in an organized manner, organized sport's role in society has become increasingly important over the years, not just for the benefit of an individual but for public health. Despite the many benefits of organized sport, these benefits are not always accessible or equitable to all children and adolescents. These disparities in health, well-being, quality of life, and socioeconomic status (SES) inequities can be a cause of the SDOH. Athletic trainers are health care providers that are uniquely positioned in an optimal public health intersection and can provide direct care for their low SES patient population that is impacted by the SDOH. The primary purposes of this dissertation were to 1) further understanding of youth sport participation experiences from low socioeconomic status families to now of Hispanic/Latinx families, 2) quantify perceptions of clinical management decisions toward low SES student athletes in a group of medical providers and public health advocates, and 3) identify challenges and strategies to upholding a standard of care when providing care to low SES patient population in the secondary school setting. Study 1: 12 Hispanic/Latinx parents and their high school aged children participated in this study. Hispanic/Latinx families described expectations of time and money that were exacerbated by the current youth sport culture. These families met the current youth sport expectations by methods rooted in their culture. When expectations were not met, this led to a misalignment of cultures and resulted in a negative youth sport experience. Study 2: A total of 380 secondary school athletic trainers completed the quantitative aim of this study through a cross-sectional survey regarding their perceptions of the SDOH and SES related to providing patient care. 12 ATs participated in the qualitative aim of this study. ATs perceived health and health care as the most relevant determinant when providing care to low SES patients. When ATs further considered the SES of patients, they identified all SDOHs as barriers they were ill equipped to navigate as they delivered care and engaged in patient referral.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. For many, youth sports have been considered a right of passage to adolescence with organized youth sports programs being encased in communities for a handful of decades. Within that time, a plethora of research has examined various psychological and social outcomes to participation, including perceived enjoyment of sport participation, self-confidence, and social relationships. In addition, the role and contribution of the coach to such outcomes have been well studied suggesting children of trained coaches are more likely to have higher levels of perceived enjoyment of sport participation, self-confidence, and social skills. Despite the breadth of research in the field, much of the knowledge of physical activity has stemmed from the assumption that higher levels of such psychosocial attributes produce higher levels of participation and daily physical activity via sport play. Unfortunately, few studies have gone so far as to measure physical activity levels of children participating in youth sports. The purpose of this study was to explore physical activity levels and contextual factors of physical activity among children participating in organized youth sport practices, and influence of coaching experience and coaching efficacy on levels of physical activity. Physical activity levels were obtained via Actigraph accelerometer and the OSRAC:YS direct observation system, with the latter also used to collect information on the contextual factors of physical activity during youth sport practices. Adult volunteer coaches completed coaching experience and coaching efficacy questionnaires. Results of the study suggest approximately one-third of practice time is spent in recommended levels of physical activity and 40% in sedentary activity. Practice context and coach behavior were shown to be significantly associated with MVPA. Across all teams, coaches spent a significant proportion of practice time providing general instruction and management of players/activities, which were less likely to be associated with MVPA. Coaching efficacy was not significantly associated with children's MVPA. Lastly, end of the season coaching efficacy scores were significantly related to scores measured earlier in the season.
