Book excerpt: Face à l'augmentation de la prévalence de l'insuffisance cardiaque et sa lourde morbimortalité, la lutte contre l'inertie thérapeutique est capitale. Différentes études ont révélé des prescriptions insuffisantes en terme d'association de molécules et de dosage. En conséquence, l'objectif de notre étude était de comprendre les réticences des médecins généralistes à prescrire et optimiser les thérapeutiques recommandées. Étude observationnelle par méthode qualitative avec approche par théorisation ancrée à partir de 19 entretiens semi-dirigés. Par défaut de connaissances et d'adhésion aux recommandations, il s'est avéré que l'optimisation des traitements n'était pas effectuée. Le concept de titration, souvent obscur pour les médecins, venait accroître la complexité de la prise en charge de cette pathologie. L'absence d'habitude, le manque de directives et l'accès complexe au cardiologue étaient soulignés. Le rôle des généralistes s'avérait mal défini et certains renvoyaient la responsabilité de la gestion du traitement au spécialiste. Les principaux freins relatés dans la littérature semblaient être liés au patient et ses comorbidités, mais notre étude a montré que de nombreux autres facteurs participaient à l'inertie, qui était alors justifiée par une stabilité clinique, une crainte de la iatrogénie chez des patients fragiles, et parfois un manque de temps. Pour assurer un suivi optimal pour les patients, les médecins auront à prendre conscience de l'importance de l'optimisation et du rôle qu'ils ont à jouer. Pour cela, une bonne connaissance et une adhésion totale aux recommandations, avec l'aide des directives des cardiologues, seront un socle essentiel. De plus, le développement des réseaux d'insuffisants cardiaques nécessitera l'inclusion des généralistes, peu avisés à l'heure actuelle. Et donc, devant l'enjeu majeur de santé publique que représente l'insuffisance cardiaque, une meilleure communication dans la population générale et auprès des médecins sera nécessaire.

Le traitement de l'Insuffisance cardiaque à fraction d'éjection altérée (ICFEA) a bénéficié de nombreuses avancées avec l'apparition de recommandations récentes. Cependant de nombreux patients restent sous traités. L'objectif de cette étude était de mettre en avant les freins gênant l'optimisation des traitements médicamenteux de fond de l'ICFEA chez les Médecins généralistes (MG). Matériel et méthode : Il s'agit d'une étude qualitative sous forme d'entretiens individuels semi-dirigés réalisés parmi les médecins généralistes du bassin d'Annecy principalement. Un Focus group (FG) a été réalisé afin de vérifier notre saturation. Les MG étaient recrutés en variation raisonnée par une technique de recrutement inspirée de la boule de neige et complétée par un recrutement de proximité. Les critères d'exclusions étaient : moins de 2 ans d'exercice, et activité particulière exclusive. Aucune rétroaction n'était prévue. Résultats : 18 entretiens individuels ont été réalisés entre le mars 2016 et juin 2017, ainsi qu'un FG comprenant six MG permettant d'obtenir une saturation pragmatique. L'ensemble des entretiens a été retranscrit et codé en analyse thématique puis ont été triangulés par les deux chercheurs. Différentes familles de freins ont été retrouvées : des freins liés au médecin, au patient, au réseau de soin, au traitement, et techniques. Conclusion : Cette étude a permis de mettre en avant les différents freins à l'optimisation des traitements de fond de l'ICFEA chez les MG. Elle permet d'entrevoir des solutions possibles (amélioration du réseau, adaptation des formations...).
