These will, in some measure, supply the want of proper references in the former impressions of the first volume; though that defect is remedied in this edition. Between the years 1722 and 1739, while I practised in the country, I took notes of all the remarkable cases that occurred in midwifery; but in London, since the year 1740 to the present time, I have been more careful and minute in forming a Collection, with a view to make it public. From a great number of instances, I have selected only the most material, and such as were best adapted to the nature of my plan; for I was unwilling to tire the reader with a succession of parallel histories that contain nothing essentially different from one another. In order to render the performance still more complete, I have taken from authors of the best authority a few extraordinary cases which seldom occur, as well as borrowed some medical transactions from the most approved modern physicians. Von Siebold, and soon developed a strong liking for this department of Medicine. In 1852 he visited Vienna, and afterwards England, Scotland, and Ireland, in order to study the methods of procedure in the Lying-in Institutions of these countries. Shortly after his return to Gottingen he published(1858) an excellent Compendium of Midwifery, and in 1860 was appointed Professor Extraordinarius of Midvifery in the University. The following year he was called to Freiburg as Prof. Ordinarius, and two years afterwards(1864) removed to Konigsberg, which however he again quitted on being appointed(1865) Professor at Breslau. In 1870 in conjunction with Crede, S. started the Archiv fr Gynakologie, and throughout his life was a constant and valuable contributor. Meanwhile he was accumulating a large practice, which however did not prevent him from devoting much time to the great work with which his name is mainly associated, viz. the Lehrbuch der Geburtshulfe, of which the following pages are a translation. The Lehrbuch appeared in 1878, and became so popular that in 1880 and edition was called for, and S. was far advanced with its preparation at the time of his death. Symptoms of failing health appeared in the summer of 1880, but so great was S. 's enthusiasm for his work, that it was with difficulty that he could be persuaded to abandon it for a season. Book excerpt: Excerpt from Lectures on the Theory and Practice of Midwifery: Delivered in the Theatre of St. George's Hospital In other cases labour is rendered difficult and dangerous by the unfavourable position of the child within the uterus: instead of the head presenting, or coming foremost, an arm descends, and the body of the child is placed across the pelvis, so that the longer and more forcibly the uterus contracts upon it, the more firmly are the shoulder and thorax impacted in the pelvis, and the more difficult does the operation of turning become. The placenta, which usually adheres to the upper part of the uterus, is sometimes attached to the cervix or lower part, and then a dangerous flooding ensues, which nature has no power to arrest. The means she usually employs for the suppression of uterine hemorrhage, if exerted in this case, would only aggravate the evil; and if artificial delivery were not performed, death would inevitably follow. The same is the consequence when the placenta has been detached from the upper part of the uterus, and where hemorrhage follows the birth of the child; and not less destructive are the effects produced by the retention of the whole, or portions, of the after-birth in the uterus. There are many other varieties of difficult and complicated labour, in which the unaided powers of nature are insufficient for the completion of delivery, and where the most prompt and energetic and well-directed treatment is required to avert the danger. Even after the most easy and natural labours, when none of these accidents have occurred, and when all danger is apparently at an end, how often does inflammation of the uterus unexpectedly supervene, and run its course with a rapidity and violence equal to that of plague and cholera. That such occurrences are not uncommon, the Reports of public institutions in different countries afford too abundant proof. 219,333 women were delivered in the kingdom of Wurtemberg from the year 1821 to 1825, of whom there died 1248, or one in 175. At Hamburg, 5-57 women died in 1000 who have been pregnant or delivered. Of 448,356 females who died in the Prussian States, in the course of fifteen years, between the ages of fourteen and forty-five, 70,215, or nearly one-sixth, died either immediately in the act of delivery or in child-bed; and of 108 of the infants born, 1 in 108 cost the mother her life. In the mortality table for the city of New York for a period of thirty-two years, viz., from 1805 to 1836, both inclusive, it is stated that the births during this period ranged from 200,000 to 300,000; and that the whole number of deaths from child-bed were only 1038. In the Dublin Lying-in Hospital, from the 8th day of December, 1757, to the 31st day of December, 1832, there were 132,979 women delivered, of whom 1488 died. In the brief interval, however, that has elapsed since then the changes that have taken place in both the theory and practice of obstetrics have made it necessary for me to present to the profession what is essentially a new book. Thus many modifications in the theory have resulted from more careful observations in anatomy and pathology, and from fruitful physio logical investigations. It has been my endeavor to interweave aseptic precautions with all branches of obstetric art without, how ever, insisting upon pedantic measures which experience has shown to be needless. But the noblest conquests of the past years have been the result of the employment of modern surgery for the relief or removal of a host of complications formerly regarded as beyond the pale of human help. In making needed alterations I have not felt it obligatory when new discoveries have destroyed the value of former deductions to retain these in order to maintain a reputation for consistency. I thank the medical press and the medical profession of this country and Great Britain for the flattering welcome they have hitherto extended to this treatise, and trust that in its new form it may command a like degree of favor.
