Book Biomechanics in Dentistry Evaluation of Different Surgical Approaches to Treat Atrophic Maxilla Patients

Download or read book Biomechanics in Dentistry Evaluation of Different Surgical Approaches to Treat Atrophic Maxilla Patients written by Muhammad Ikman Ishak and published by Springer Science & Business Media. This book was released on 2012-08-31 with total page 103 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book shows computational finite element simulations to analyse the strength of implant anchorage for intrasinus and extramaxillary approaches under various occlusal loading locations and directions. Three-dimensional model of the craniofacial area surrounding the region of interest, soft tissue and framework are developed using computed tomography image datasets. The zygomatic and standard dental implants are modeled using a conventional computer-aided design software and placed at the appropriate location. Material properties are assigned appropriately for the cortical, cancellous bones and implants with Masseter forces applied at the zygomatic arch and occlusal loadings applied on the framework surface.

Book Reconstruction of Severely Atrophic Edentulous Ridges with Autogenous Onlay Bone Grafts

Download or read book Reconstruction of Severely Atrophic Edentulous Ridges with Autogenous Onlay Bone Grafts written by Scarnu00f2 Dario and published by . This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Reconstruction of severely atrophic edentulous ridges with autogenous onlay bone graftsMatteo Chiapasco, Dario Scarnu00f2, David Palombo, Grazia Tommasato*Unit of Oral Surgery - Department of Biomedical, Surgical, and Dental Sciences - University of Milan, Milan, Italy; (*Head and Chairman: Prof. M. Chiapasco)Background: Severe bone deficiencies derived from atrophy, periodontal disease, and trauma sequelae may hamper the rehabilitation of edentulous ridges with oral implants. Placement of implants can be hindered both by the absence of sufficient bone volume and the proximity of anatomical structures, such as the inferior alveolar nerve, the maxillary sinus, and the nasal floor. Therefore, reconstructive procedures are requested to allow implant placement and adequate implant-supported prosthetic restorations.Aim: To evaluate the long-term outcome of implant rehabilitations of patients affected by severely resorbed edentulous ridges reconstructed with autogenous bone blocks. Evaluated outcomes were: a) peri-implant bone resorption; b) success and survival rate of implants; and c) patientu2019s satisfaction.Materials and Methods: In a 15-year period (1997-2015), 263 patients were reconstructed with autogenous bone grafts harvested from the calvarium (n=75), the ilium (n=123) and the mandibular ramus (n=65). u2028Six to nine months afterwards, 1137 implants were placed in the reconstructed areas. After a further waiting period, patients were rehabilitated with implant-supported prostheses. Follow-up ranged from 1 to 20 years (mean: 9 years) after the start of prosthetic loading. Peri-implant bone resorption before and after the start of prosthetic loading was evaluated with a dedicated software (ImageJu00ae), success and survival rate following Albrektsson's criteria, patientu2019s satisfaction with a questionnaire. Results: Success and survival rates of implants were: 86.1% and 99.2% with calvarium, respectively; 77.4% and 97.8% with iliac crest; 89.1% and 97.8% with mandibular ramus. The majority of patients (245) were fully satisfied, while only 18 would retrospectively had chosen less invasive solutions. Conclusions: On the basis of these results, bone reconstruction with autogenous grafts appears to be a reliable solution to allow the placement of implants in severely resorbed ridges. Success and survival rates of implants were consistent with those obtained in case of implants placed in native bone, in particular when patients were reconstructed with calvarium and mandibular ramus, in which the cortical component was dominant. The vast majority of patients were fully satisfied as regards the final results.Key word: bone graft, bone atrophy, autogenous bone, onlay bone graft, dental implants, patients satisfaction, quality of life.

Current manufacturers offer a variety of implant lengths and widths, allowing surgeons and restorative dentists the ability to select the best implant for each clinical circumstance. Cutting edge information is provided on the research and clinical results achieved utilizing a range of implants, specifically those developed by Nobel Biocare, Straumann, Jack Hahn, and Bicon. Readers will also find an extensive description of the role of ultra-short implants involving reconstruction in both cleft patients and cancer patients who have lost portions of their mandible and/or maxilla. This book is a must-have for those interested in learning how the use of short and ultra-short implants offers both surgeons and restorative dentists an opportunity to stand out from those that use only the traditional length implants.
