Book excerpt: The Office of Health and Environmental Assessment (OHEA) is primarily responsible for providing EPA with a central capability for evaluating information on the health effects of toxic pollutants and for ensuring the consistency and technical competence of the Agency's risk assessment work. OHEA prepares a variety of documents in response to requests from other EPA offices. These documents are used in support of regulatory and enforcement decisions. Until these decisions are made, many of the documents are available only through the program offices for which the work was performed. Among the examples of documents in this category are Health and Environmental Effects Profiles, Drinking Water Criteria Documents, and some assessments prepared for the Office of Pesticide Programs. A bried description of document types is provided below. The attached list of chemicals is a compilation of assessments prepared, or in preparation, in OHEA and cover the period from 1980 to 1986.

Several key federal agencies-including ATSDR, EPA, NIOSH, NTP, and OSHA-as well as state agencies, assess the toxicity of chemicals. GAO was asked to review chemical toxicity assessment activities. This report (1) describes the chemical toxicity assessment activities selected federal and state agencies undertake; (2) assesses the extent to which these federal agencies' chemical toxicity assessment activities are fragmented, overlapping, or duplicative; and (3) assesses the extent to which these federal and state agencies coordinate their chemical toxicity assessment activities and challenges in doing so. GAO selected five key federal agencies that assess chemicals, and a nonprobability sample of agencies in 10 states that provide a range of assessment activities. GAO reviewed federal agency documentation and compiled summaries of chemical toxicity assessment activities and compared them with one another. GAO interviewed officials from these agencies, representatives from industry, and other stakeholders.

It has been instrumental to the mission of the U.S. Environmental Protection Agency (EPA) as well as other federal agencies in evaluating public health concerns, informing regulatory and technological decisions, prioritizing research needs and funding, and in developing approaches for cost-benefit analysis. However, risk assessment is at a crossroads. Despite advances in the field, risk assessment faces a number of significant challenges including lengthy delays in making complex decisions; lack of data leading to significant uncertainty in risk assessments; and many chemicals in the marketplace that have not been evaluated and emerging agents requiring assessment. Science and Decisions makes practical scientific and technical recommendations to address these challenges. This book is a complement to the widely used 1983 National Academies book, Risk Assessment in the Federal Government (also known as the Red Book). The earlier book established a framework for the concepts and conduct of risk assessment that has been adopted by numerous expert committees, regulatory agencies, and public health institutions. The new book embeds these concepts within a broader framework for risk-based decision-making. Together, these are essential references for those working in the regulatory and public health fields.

EPA's Integrated Risk Information System (IRIS) program is a chemical evaluation program that is a critical component of EPA's capacity to support scientifically sound environmental regulations and policies. The IRIS database contains EPA's scientific position on the potential human health effects of exposure to more than 540 chemicals. This testimony highlights GAO's work on toxic substances, focusing on (1) its March 2008 report, Chemical Assessments: Low Productivity and New Interagency Review Process Limit the Usefulness and Credibility of EPA's Integrated Risk Information System and (2) key changes to the IRIS assessment process EPA included in its revised IRIS assessment process released on April 10, 2008. It also highlights the findings of two GAO reports on EPA's regulation of toxic chemicals. For the IRIS report, GAO analyzed EPA data and interviewed officials at relevant agencies, including the Office of Management and Budget (OMB). For this testimony, GAO supplemented the IRIS report with a review of EPA's revised IRIS assessment process announced earlier this month. Given the importance of the IRIS program to EPA's ability to protect public health and the environment, Congress should consider requiring EPA to suspend its new process and develop one that is responsive to GAO's recommendations.

However, given the significant yet controversial nature of risk assessments, it is important that policymakers understand how they are conducted, the extent to which risk estimates produced by different agencies and programs are comparable, and the reasons for differences in agencies' risk assessment approaches and results. GAO studied the human health and safety risk assessment procedures of the Environmental Protection Agency, the Food and Drug Administration, the Occupational Safety and Health Administration, and the Department of Transportation's Research and Special Programs Administration. This report describes (1) the agencies' chemical risk assessment activities, (2) the agencies primary procedures for conducting risk assessments, (3) major assumptions or methodological choices in their risk assessment procedures, and (4) the agencies' procedures or policies for characterizing the results of risk assessments.
