Book excerpt: Beating heart surgery, or off-pump coronary artery bypass (OPCAB), is a less invasive alternative to conventional coronary artery bypass graft surgery (CABG) and aims to avoid the surgical trauma caused by cardiopulmonary bypass. For the purpose of this review OPCAB was defined as off-pump CABG with full merdian sternotomy. Seven randomised controlled trials and twenty-five non-randomised controlled trials comparing OPCAB with CABG were available for review. The methodological inaccuracies and poor evidence quality of many of the studies meant that only very general conclusions could be deawn from the data. Perioperative morbidity and mortality was generally similar or lower during OPCAB, in comparison to CABG. Overall, there was no discernible difference between difference in short-term safety outcomes between OPCAB and CABG, while major complications appeared less likely to occur following OPCAB. However, the lack of long-term follow-up precluded an assessment of the long-term safety of OPCAB. The limited data indicated that perioperative effectiveness outcomes were similar for both OPCAB and CABG. However, the number of grafts performed per patient was significatly lower during OPCAB.

Book excerpt: MIDCAB (minimally invasive direct coromary artery bypass) is a coronary artery bypass procedure done on a beating heart, through a minimal access incision. For the purpose of this review MIDCAB is defined as a off-pump coronary artery bypass (OPCAB) grafting undertaken a full median sternotomy. No differences in mortality rates could be detected between MIDCAB and CCABG groups and there was some limited evidence that MIDCSB may be assiciated with less perioperative and postoperative morbidity (less need for transfusion, lower rates of atrial fibrillation and myocardial injury, and reduced inflammatory response) than for CCABG. However, the evidence came from mostly small randomised controlled trials and non-randomised studies. There was insufficient evidence to etsablish whether MIDCAB reduced postoperative pain. No difference in patency rates could be detected between MIDCAB and CCABG groups. The MIDCAB procedure may be associated with shorter operating times and shorter hospital stays than for CCABG procedures. However, there may be more need for re-intervention in MIDCAB compared to CCABG. There is insufficient evidence to determine whether MIDCAB procedures were less costly than CCABG procedures, although shorter operating times and shorter hospital stays are likely to make MIDCAB less costly than CCABG.
