Pulpectomy is a conservative treatment option to salvage infected primary teeth. However, primary teeth with their bizarre root canals that undergo continued secondary dentin deposition and physiological root resorption combined with pathological changes present a significant endodontic challenge especially during obturation. The obturation technique used should ensure a hermetic seal and at the same time prevent extrusion of the obturation material which would be detrimental to the successor teeth. This book discusses various obturation techniques for primary teeth root canals and summarize the evidence . It was found that various factors like obturation material used, location of teeth in the oral cavity, canal anatomy, handling characteristics, cost and treatment time play an important role in the clinician’s choice of obturation technique. Therefore, there is no single technique that can be recommended as ‘ideal’ for obturation.

Book excerpt: The primary goal of a root canal treatment in deciduous teeth is to eliminate infection and to retain the tooth in a functional state until it gets normally exfoliated. The ideal root canal filling material for primary teeth should resorb at a similar rate as the roots of primary tooth, be harmless to the periapical tissue and to the permanent tooth germ, resorb readily if pushed beyond the apex, be antiseptic, radio-opaque, should not shrink, easy to fill and can be removed easily if required at any stage, should adhere to the walls and should not discolor the tooth. In order to achieve desirable properties of an ideal root canal filling material various investigations have been conducted the world over. Presently commonly used obturating materials for primary teeth are zinc oxide eugenol, calcium hydroxide and iodoform based pastes.

They are also considered important for phonetics, appearance, as well as psychological well-being.1 Dental caries is one of the most prevalent diseases of childhood that cause destruction of the dental tissues approximately at an incidence of 20-76%.2 It compromises the structural integrity of teeth leading to the ingress of micro-organisms into the endodontic spaces. The microbial species identified from the root canals of such teeth included pure aerobic, anaerobic, and mixed gram-positive as well as gram-negative cocci, bacilli, and fungi.3 The pulps of such jeopardized teeth get damaged irreversibly, and such teeth become unable to withstand normal function for a long duration in the oral cavity. Preservation of such teeth in the dental arch is achieved through endodontic therapy, which is primarily aimed at the elimination of causative agents responsible for endodontic infections. The treatment modalities include mummification pulpotomy, partial pulpotomy, and pulpectomy. For pulpless teeth or teeth with necrotic pulp, pulpectomy is the treatment of choice. The microorganisms and their by-products responsible for the initiation and progression of endodontic diseases are eliminated through the cleaning and shaping of the endodontic spaces.4
