For the leg we have a choice of many methods. Each surgeon can suit his fancy. Usually the method is determined by the kind of covering it will give the bones of the stump, or by the facility or celerity with which it can be executed, or by the evil consequences it enables us to avoid, or because of the like lihood of its yielding quick union. A purely circular incision, with tegumentary ap, used to be greatly in vogue, and is yet practiced by some. It is tedious to turn up the ap, like I do the cuff of my sleeve, and this cannot be done where the leg is muscular and conical. Such a ap comes together like the mouth of a purse, and this is pre ventive of union by the first intention. I never practice or recom mend such a method. I also am disposed to reject antero-posterior aps, whether tegumentary or in part muscular, for the edge of the cut tibia is apt to induce ulceration of the anterior ap, or the weight of the posterior ap prevents union. There is greater danger of necrosis of the bone and of a conical stump after this method. I rather prefer the lateral skin aps of equal size, or the hood-flap of Dr. Stephen Smith of New York. This latter, however, is not as easily turned up as are the lateral aps, and moreover, when the sutures are adjusted, it is found that there is a super uity of tissue, above and over the tibia, which rather opposes primary union, and does not readily permit of the covering adapting itself to the bones. Three or four sutures are usually sufficient for each side. A little care is required in passing the one through the crown of the cervix and approximating the edges of the 08. If the sutures for both sides be passed before securing any of them, much difficulty will be avoided. An undue ow of blood may usually be checked bv tightening the tourniquet, when one is used. In all abdominal operations, however, the intra-peritoneal method of treating the pedicle has not been the most successful one, especially in the opera tions of myomotomy and the Porro operation. In this, however, the fault does not lie with the closure of the peritoneum, but in the peculiar condition which prevents the return of the pedicle to the abdominal cavity. In all cases, where it is possible so to do, the peritoneum should be united by a continuous cat-gut suture, and then the abdominal walls united by their own sutures. This is particularly insisted upon by Spencer Wells for another reason - he says that by so doing the presence of an abdominal hern'ia at the point of incision is prevented later on. Operation - The operation is to be performed as follows: The room should be of a temperature of about 80 degrees Fahrenheit everything needed for the operation should be in the room.
