The increase is due to lack of proper sewage treatment, and primarily to excess fertilizer application. Also, eutrophication in several coastal areas is triggered by high nitrate concentrations. The main purpose of this book is to integrate scientific knowledge related to exposure assessment, health consequences and control of nitrate contamination in water. The motivation is related to the magnitude, the possible adverse health effects, and the high cost of control ling nitrate contamination. Future research tasks are defined by an interaction among hydro logists, toxicologists and environmental engineers in an integrated framework for nitrate risk management. The target readership of this book is a mix of university colleagues, practitioners from both the private and public sectors and advanced graduate students working with the hydrological, health science or environmental engineering aspects of nitrate contamination. The main conclusions include: 1. For risk assessment purposes, knowledge and sufficiently accurate models are available to predict nitrate load and its fate in water under changes in land use. 2. Once agricultural exposure controls are implemented, the response times in ground water may be so long as to make controls unrealistic. 3. It is still unknown whether agricultural best management practice is a compromise between nitrate risk reduction and agricultural revenue. 4. The current drinking water guidelines of 10 mg/L NOrN need not be changed.

Despite important improvements in the last decades, access to safe drinking water is not universal. The World Health Organization estimates that almost 10% of the population in the world do not have access to improved drinking water sources. Among other diseases, waterborne infections cause diarrhea, which kills nearly one million people every year, mostly children under 5 years of age. On the other hand, chemical pollution is a concern in high-income countries and an increasing problem in low- and middle-income countries. Exposure to chemicals in drinking water may lead to a range of chronic non-communicable diseases (e.g., cancer, cardiovascular disease), adverse reproductive outcomes, and effects on children’s health (e.g., neurodevelopment), among other health effects. Although drinking water quality is regulated and monitored in many countries, increasing knowledge leads to the need for reviewing standards and guidelines on a nearly permanent basis, both for regulated and newly identified contaminants. Drinking water standards are mostly based on animal toxicity data, and more robust epidemiologic studies with accurate exposure assessment are needed. The current risk assessment paradigm dealing mostly with one-by-one chemicals dismisses the potential synergisms or interactions from exposures to mixtures of contaminants, particularly at the low-exposure range. Thus, evidence is needed on exposure and health effects of mixtures of contaminants in drinking water. Finally, water stress and water quality problems are expected to increase in the coming years due to climate change and increasing water demand by population growth, and new evidence is needed to design appropriate adaptation policies. This Special Issue of International Journal of Environmental Research and Public Health (IJERPH) focuses on the current state of knowledge on the links between drinking water quality and human health.

Book excerpt: The Safe Drinking Water Act directs the U.S. Environmental Protection Agency (EPA) to establish national drinking-water standards for chemical and biological contaminants in public water supplies. The standards are to be set at concentrations at which no adverse effects on human health occur or are expected to occur from lifetime consumption, allowing a margin of safety; enforceable standards are standards that are feasible to achieve with the use of the best technology available. The standards are to be reviewed periodically to ensure continued protection of public health. Consistent with the requirement for periodic review, EPA asked the National Research Council to evaluate the current drinking-water maximum-contaminant-level goals (MCLGs) and maximum contaminant levels (MCLs) for nitrate and nitrite in public water supplies. The Subcommittee on Nitrate and Nitrite in Drinking Water, convened under National Research Council procedures, reviewed information on the occurrence and toxicity of nitrate and nitrite. The subcommittee evaluated this information in the context of the drinking-water standards for those substances and drew conclusions about the adequacy of the current standards to protect human health.

Methemoglobinemia, or blue-baby syndrome, is a potentially fatal condition that inhibits the ability of red blood cells to bind and transport oxygen. Nitrates/nitrites have also been linked to such conditions as cancer, birth defects, and behavioral and developmental abnormalities. Nitrates are frequently found in wells in rural farming areas because synthetic fertilizers (containing nitrates) leach from the soil into the groundwater. The Lower Umatilla Basin (LUB) in Morrow and Umatilla counties of Oregon represents an intensively farmed and irrigated area in which relatively high amounts of nitrates are present in the groundwater and domestic well water. This study investigated population demographics for the rural Lower Umatilla Basin, comparing these data to identified well-water nitrate levels for the purpose of estimating nitrate exposures and potential risk of adverse health effects in the survey area. Results of the investigation revealed that 25 percent of the domestic-use wells in the survey area had nitrate levels that were in excess of the 10 ppm nN MCL for drinking water, as established by the U.S. Environmental Protection Agency. From access to these wells, 23 percent of the surveyed population was exposed to nitrate concentrations in excess of the MCL standard. However, resident infants were neither exposed to well-water nitrates in excess of the standard, nor were they exposed to illness that could have increased the risk of methemoglobinemia. The LUB survey population was generally older than the populations from cities in the LUB or the combined populations of rural areas of Morrow and Umatilla counties. The population included few women of childbearing age, and it was not subject to an appreciable increase in the proportion of younger to older families. These factors reduced the likelihood of a significant increase in the infant population, which also minimized the risk of methemoglobinemia to this population. Even though the risk of methemoglobinemia to infants was low in the LUB area, it is recommended that exposures to well-water nitrates be prevented, if possible even for adults, to reduce the potential for chronic, adverse health effects from excess nitrate ingestion. Continued monitoring of private wells by state agencies is recommended, with attention directed at domesticuse wells with nitrate levels in excess of 10 ppm nN. This information should be shared with local health departments for follow-up, investigation, and educational efforts as needed. Future studies by the Oregon DEQ, or other agencies which seek to document the sources of well-water nitrate contamination in the LUB, should include an investigation of the influence of local sources of nitrate contamination.

Book excerpt: Nitrate, a naturally occurring ion in the environment is one of the most common causes of contaminated groundwater. The primary concern with ingestion of nitrate contaminated groundwater is a condition called infant methemoglobinemia and recent studies have found associations between exposure to nitrate contaminated groundwater, cancer, and reproductive and developmental outcomes. Because testing for nitrate is the responsibility of the private well owner, efforts to assess knowledge, awareness, and practices related to nitrate well water testing for the purposes of developing or improving educational efforts was investigated by use of a mailed survey among randomly selected private well owners in La Crosse County, Wisconsin. A total of 165 (out of 500) surveys were returned and analyzed. Awareness of nitrate well water testing (yes or no) and tested for nitrate (yes or no) was cross tabulated and found to be significant (Asymp. Sig. 2-sided 0.000, Pearson chi-square value 37.73). Among those that were aware of nitrate well water testing, 69% had tested their well water for nitrate and 31% had not tested their well water for nitrate suggesting the need for educational efforts, particularly for high risks groups. In addition, efforts aligned with encouraging regular testing practices should be a priority.
