Book excerpt: The management of Alzheimer's Disease and the related dementias is one of the major challenges to health care professionals and American society-at-large for the coming decade and the coming millennium. The rapid growth of the over-eighty-five population, the group which, as recent studies have confirmed and as many of us clinicians have long suspected, has an even higher prevalence than previously quoted of dementing disorders, is the major cause of this. We are thus challenged by, as Bernard Issacs used to call it, "the survival of the unfittest," as well as the oPtimistic approach of "bringing life to years," as John F. Kennedy said. The fact is that we, as a society, tend to confuse "treatment" and "cure" (and "prevention"). As the proceedings of the conference which this book represents emphasize, there is considerable work going on about the potential prevention of, or at least the reduction of, symptomatology in these illnesses by interventions genetically, chemIcally, and so forth. However, the more we find out, the more complicated it becomes, and the more heterogeneous Alzheimer's and the related disorders appear to be, not only in their manifestations (as clinicians have long recognized) but also in the individual initiating and underlying processes. For these reasons, absolute preventive techniques or the likelihood of an intervention which will reverse the process in a high proportion of patients, do not appear to be just around the corner.

Sociology offers a knowledge base, a number of useful analytic approaches and tools, and unique theoretical perspectives that can facilitate understanding of these demographic, economic, and social changes and, to the extent possible, their causes, consequences and implications. New Directions in the Sociology of Aging evaluates the recent contributions of social demography, social epidemiology and sociology to the study of aging and identifies promising new research directions in these sub-fields. Included in this study are nine papers prepared by experts in sociology, demography, social genomics, public health, and other fields, that highlight the broad array of tools and perspectives that can provide the basis for further advancing the understanding of aging processes in ways that can inform policy. As fall incidence rates increase alongside our globally aging population, fall-related mortality, hospitalizations, and costs are reaching never seen before heights. Because falls occur in clinical and community settings, additional efforts are needed to understand the intrinsic and extrinsic factors that cause falls among older adults; effective strategies to reduce fall-related risk; and the role of various professionals in interventions and efforts to prevent falls (e.g., nurses, physicians, physical therapists, occupational therapists, health educators, social workers, economists, policy makers). As such, this Research Topic sought articles that described interventions at the clinical, community, and/or policy level to prevent falls and related risk factors. Preference was given to articles related to multi-factorial, evidence-based interventions in clinical (e.g., hospitals, long-term care facilities, skilled nursing facilities, residential facilities) and community (e.g., senior centers, recreation facilities, faith-based organizations) settings. However, articles related to public health indicators and social determinants related to falls were also included based on their direct implications for evidence-based interventions and best practices.

Book excerpt: As the United States devotes extensive resources to health care, evaluating how successfully the U.S. system delivers high-quality, high-value care in an equitable manner is essential. At the request of Congress, the Agency for Healthcare Research and Quality (AHRQ) annually produces the National Healthcare Quality Report (NHQR) and the National Healthcare Disparities Report (NHDR). The reports have revealed areas in which health care performance has improved over time, but they also have identified major shortcomings. After five years of producing the NHQR and NHDR, AHRQ asked the IOM for guidance on how to improve the next generation of reports. The IOM concludes that the NHQR and NHDR can be improved in ways that would make them more influential in promoting change in the health care system. In addition to being sources of data on past trends, the national healthcare reports can provide more detailed insights into current performance, establish the value of closing gaps in quality and equity, and project the time required to bridge those gaps at the current pace of improvement.
