While neonatal survival following preterm birth continues to improve, this has not been matched by a decline in neurological outcome. There is still a high prevalence of motor problems, executive dysfunction, and cognitive impairment in infants born preterm. Improved neuroimaging has helped to describe different types of neonatal brain injuries in this population and has given a better understanding of underlying pathogenesis. However, therapies are still lacking and there is a great need to find novel strategies to improve injury and functional outcome.

Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. The birth of a preterm infant can also bring considerable emotional and economic costs to families and have implications for public-sector services, such as health insurance, educational, and other social support systems. Preterm Birth assesses the problem with respect to both its causes and outcomes. This book addresses the need for research involving clinical, basic, behavioral, and social science disciplines. Such aims have been advanced by previous European Congresses and it was our privilege in Dublin to host the IXth Congress held at the Royal Dublin Society from September 3rd to 5th 1984. Over nine hundred delegates rep resenting thirty-nine countries attended the scientific and social programme. The theme of the congress was the mature baby. The organizers felt that while major advances had been made in the management of prematurity there were still far too many mature fetuses dying and too many deaths in the nor mally formed appropriate weight for gestational age neonate. Hence antenatal fetal assessment, the management of labour including electronic monitoring in normal women and asphyxial brain damage were major topics. Pregnancy hy pertension, caesarean section, breech presentation, diabetes and unexplained mature stillbirth were workshop topics.
