Book excerpt: Abstract: Alzheimer's disease (AD) is the most common form of dementia, and the need for the identification of biomarkers for predicting future development of AD is imperative for the effective application of preventive treatments. Magnetic resonance imaging (MRI) is a useful means for measuring atrophy in the brain in vivo, and for identifying possible imaging biomarkers to predict who will develop AD. Regions that have received attention as possible predictors of AD include the hippocampus, entorhinal cortex, and more recently, regions of the parietal lobe. To gain further insight into these regions, the hippocampus and inferior parietal lobule (IPL) were subdivided into discrete subregions and compared in normal aging, mild cognitive impairment (MCI) and AD subjects of the Alzheimer's Disease Neuroimaging Initiative. MRI images were processed through FreeSurfer to gain morphometric measures of each region, and subregions were manually defined. The IPL was subdivided into the gyrus, banks, and fundus. These regions were differentially affected among groups, suggesting that reductions occur in the left IPL banks prior to the gyrus and fundus, and that IPL measures, when combined with neuropsychological (NP) data, were predictive of group membership and future decline. The hippocampus was subdivided into the head, body, and tail. Group differences in volume primarily occurred in the head and body, which demonstrated the strongest correlations with NP performance, and together predicted group membership. Finally, the effect of correcting for intracranial volume (ICV), a common method for accounting for head size variation in MRI, was investigated. Volumetric measurements of the hippocampus, entorhinal cortex, and ICV were obtained via FreeSurfer, and raw and ICV corrected data (expressed as % ICV) were evaluated. Results suggest that when performing large group analyses, correction for head size may be necessary, however when evaluating relationships between volumetric and NP data, correlations are stronger in raw data. Moreover, the raw data may be more accurate in the diagnosis of AD. In summary, overall results suggest that IPL and hippocampal subregions may serve as useful biomarkers that predict the development of AD, and the utilization of ICV correction may be dependent upon the type of analyses being performed.

For example, longitudinal neuroimaging studies depicting the trajectories of patterns of structural and functional brain networks of mild cognitive impairment may provide potential imaging markers for the onset of dementia. Population-based studies have addressed the potential interaction between mood and cognitive impairment in old age. However, there are few studies to explore the potential neural mechanism of the relationship between depression and cognitive impairment in old age. In all of this process the contribution of multiple biological events cannot be neglected, particularly the underlying influence of chronic diseases and concomitant polymedication as well as the geriatric conditions, like frailty, frequently present in this elderly population, which also compromise the cognitive function and mood determining depression and conducing to worse outcomes with more morbidity and mortality.
