Book excerpt: The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.

The prevalence ofpharmacotherapy treatment in newborns withdrawing from opioids remains high in theUnited States, and each year millions of funds are spent caring for infants impacted byNeonatal Opiate Withdrawal Syndrome (NOWS). The nurse practitioner (NP) identifiedand implemented a system change for the care and treatment of infants with NOWS alsoknown as Neonatal Abstinence Syndrome (NAS). He also addresses complications withthe current Finnegan Neonatal Abstinence Syndrome assessment tools and treatmentprotocol in a Midwest health care facility. The NP implemented a new system changecalled Eat, Sleep, and Console that included the creation of policies, protocols, andalgorithms; education of clinicians and families; and the formation of communitycollaborations to improve and support families of infants impacted by NOWS. Educationfor staff to break down biases and judgements was also provided and resulted in deliveryof effective, high quality, and culturally congruent care in an improved environment. Thenursing theory utilized to guide this system change was Dr. Leininger's Theory ofCultural Care Diversity and Universality, also known as the Cultural Care Theory. Book excerpt: This paper summarizes the published Cochrane Review, “Non-pharmacological Care for Opioid Withdrawal in Newborns,” by A. Pahl, L. Young, M. E. Buus-Frank, L. Marcellus, and R. Soll (https://doi.org/10.1002/14651858.CD013217.pub2), and discusses it through a treatment policy and practice lens. The study population in the review included infants born at term (37 weeks’ gestation or greater) and late preterm (34 weeks’ gestation to 37 weeks’ gestation) who had a known or suspected prenatal exposure to opioids or who were exhibiting symptoms consistent with opioid withdrawal in the first 7 days of life. Nonpharmacological interventions included a wide range of care and were broadly categorized as: modification of environmental stimulation, feeding practices, and support of the mother-infant dyad. The review found that randomized controlled trials, identified in searches conducted in October 2019, did not provide enough evidence regarding clinically meaningful outcomes. However, recently published trial data suggest that “Eat, Sleep, Console,” an individualized, trauma-informed, family-centered nonpharmacological approach to care, can reduce the number of days an infant experiencing symptoms consistent with opioid withdrawal requires in-hospital care.

Book excerpt: Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.
