Book excerpt: In the devastation that follows a major disaster, there is a need for multiple sectors to unite and devote new resources to support the rebuilding of infrastructure, the provision of health and social services, the restoration of care delivery systems, and other critical recovery needs. In some cases, billions of dollars from public, private and charitable sources are invested to help communities recover. National rhetoric often characterizes these efforts as a "return to normal." But for many American communities, pre-disaster conditions are far from optimal. Large segments of the U.S. population suffer from preventable health problems, experience inequitable access to services, and rely on overburdened health systems. A return to pre-event conditions in such cases may be short-sighted given the high costs - both economic and social - of poor health. Instead, it is important to understand that the disaster recovery process offers a series of unique and valuable opportunities to improve on the status quo. Capitalizing on these opportunities can advance the long-term health, resilience, and sustainability of communities - thereby better preparing them for future challenges. The National Disaster Recovery Framework (NDRF) is a guide to promote effective recovery, particularly for those incidents that are large-scale or catastrophic. The NDRF provides guidance that enables effective recovery support to disaster-impacted States, Tribes and local jurisdictions. It provides a flexible structure that enables disaster recovery managers to operate in a unified and collaborative manner. It also focuses on how best to restore, redevelop and revitalize the health, social, economic, natural and environmental fabric of the community and build a more resilient Nation. The NDRF defines: Core recovery principles; Roles and responsibilities of recovery coordinators and other stakeholders; A coordinating structure that facilitates communication and collaboration among all stakeholders; Guidance for pre- and post-disaster recovery planning; The overall process by which communities can capitalize on opportunities to rebuild stronger, smarter and safer. These elements improve recovery support and expedite recovery of disaster-impacted individuals, families, businesses and communities. While the NDRF speaks to all who are impacted or otherwise involved in disaster recovery, it concentrates on support to individuals and communities. The NDRF introduces four new concepts and terms: Federal Disaster Recovery Coordinator (FDRC); State or Tribal Disaster Recovery Coordinators (SDRC or TDRC); Local Disaster Recovery Managers (LDRM); Recovery Support Functions (RSFs). The FDRC, SDRC, TDRC and LDRM provide focal points for incorporating recovery considerations into the decisionmaking process and monitoring the need for adjustments in assistance where necessary and feasible throughout the recovery process. The RSFs are six groupings of core recovery capabilities that provide a structure to facilitate problem solving, improve access to resources, and foster coordination among State and Federal agencies, nongovernmental partners and stakeholders. The concepts of the FDRCs, SDRCs, TDRCs and RSFs are scalable to the nature and size of the disaster. The NDRF aligns with the National Response Framework (NRF). The NRF primarily addresses actions during disaster response. Like the NRF, the NDRF seeks to establish an operational structure and to develop a common planning framework. Fundamentally, the NDRF is a construct to optimally engage existing Federal resources and authorities, and to incorporate the full capabilities of all sectors in support of community recovery. The effective implementation of the NDRF, whether or not in the context of a Robert T. Stafford Disaster Relief and Emergency Assistance Act (Stafford Act) declaration, requires strong coordination across all levels of government, NGOs and the private sector. It also requires an effective, accessible public information effort so that all stakeholders understand the scope and the realities of recovery. The NDRF provides guidance to assure that recovery activities respect the civil rights and civil liberties of all populations and do not result in discrimination on account of race, color, national origin, religion, sex, age or disability. Understanding legal obligations and sharing best practices when planning and implementing recovery strategies to avoid excluding groups on these bases is critical. The NDRF is a guide to promote effective recovery. It is a concept of operations and not intended to impose new, additional or unfunded net resource requirements on Federal agencies.

The United States has experienced and continues to face the threat of disasters, and, like all entities, the academic biomedical research community can be affected. Recent disasters, from hurricanes to cyber-attacks, and their consequences have shown that the investments of the federal government and of the many other entities that sponsor academic research are not uniformly secure. First and foremost, events that damage biomedical laboratories and the institutions that house them can have impacts on the safety and well-being of humans and research animals. Furthermore, disasters can affect career trajectories, scientific progress, and financial stability at the individual and institutional levels. Strengthening the Disaster Resilience of the Academic Biomedical Research Community offers recommendations and guidance to enhance the disaster resilience of the academic biomedical research community, with a special focus on the potential actions researchers, academic research institutions, and research sponsors can take to mitigate the impact of future disasters.

Book excerpt: When communities face complex public health emergencies, state local, tribal, and territorial public health agencies must make difficult decisions regarding how to effectively respond. The public health emergency preparedness and response (PHEPR) system, with its multifaceted mission to prevent, protect against, quickly respond to, and recover from public health emergencies, is inherently complex and encompasses policies, organizations, and programs. Since the events of September 11, 2001, the United States has invested billions of dollars and immeasurable amounts of human capital to develop and enhance public health emergency preparedness and infrastructure to respond to a wide range of public health threats, including infectious diseases, natural disasters, and chemical, biological, radiological, and nuclear events. Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.

As part of the Post Katrina Emergency Management Reform Act of 2006, Congress required FEMA to create a national disaster recovery strategy. In response, FEMA developed the NDRF- a comprehensive framework intended to facilitate coordination of recovery assistance across all levels of government. GAO was asked to examine FEMA's implementation of the NDRF. This review assessed (1) the roles and responsibilities of FEMA and state emergency management offices in implementing the NDRF and (2) the extent to which FEMA has worked with selected states to implement the NDRF. GAO selected for review five states- Alaska, Arizona, Colorado, New York, and Oklahoma-based on factors including the range and type of disasters. GAO reviewed state recovery plans and other documents and interviewed state officials. GAO also reviewed statutes, FEMA plans and policies, and interviewed FEMA officials, including 5 of its 10 regional offices.

As part of its ongoing mission to foster dialogue among stakeholders and to confront the challenges inherent in ensuring the nation's health security, the Institute of Medicine (IOM) Forum on Medical and Public Health Preparedness for Catastrophic Events sponsored a town hall session at the 2012 Public Health Preparedness Summit. This event took place February 21-24 in Anaheim, California. In this session sponsored by the IOM, the focus of discussion was sustaining health care delivery beyond the initial response to a disaster and facilitating the full long-term recovery of the local health care delivery systems. Many elements required for recovery are also fundamental to the day-to-day operations of these systems. Investing in improved health care delivery systems, both financially and through collaborative capacity building, can enhance economic development and growth before a disaster, and also prove instrumental in sustaining services and recovering after a disaster. Post-Incident Recovery Considerations of the Health Care Service Delivery Infrastructure serves as a summary of the session and explains the value of regional capacity building; the importance of interagency, intergovernmental, and public-private collaboration; and the significant role that health care coalitions can play in ensuring resilient communities and national health security.

To assist government planning in this area, the Congress, in the E-government Act of 2002, directed the Federal Emergency Management Agency (FEMA) to request the NRC to conduct a study on the application of IT to disaster management. This report characterizes disaster management providing a framework for considering the range and nature of information and communication needs; presents a vision of the potential for IT to improve disaster management; provides an analysis of structural, organizational, and other non-technical barriers to the acquisition, adoption, and effective use of IT in disaster; and offers an outline of a research program aimed at strengthening IT-enabled capabilities for disaster management.
