Book excerpt: Morris Lake Dam, classified under high hazard potential category, is in fair overall condition. The structural stability of the dam is uncertain. The concrete gunite facing on the down stream face of the dam has numerous large cracks and is separating from the dam in areas. There is seepage through one of the cracks. Spalling and deteriorating concrete exist at a number of locations on the upstream and downstream face of the dam, and are particularly severe at the east upstream abutment and on the spillway wingwall near the west abutment. The condition of the original masonry dam within the present dam is unknown. There is now emergency low level outlet for the dam. There is no engineering data available concerning the design or construction of the original dam or subsequent modifications. The spillway capacity, as determined by the Corps of Engineers screening criteria is inadequate. The dam can adequately pass only 50% of the probable maximum flood. The following are recommended to be done soon: Perform additional investigation to determine seepage conditions through and under dam, the engineering properties of the dam and foundation, ad whether or not conventional safety margins exist under more severe stress conditions than those observed during our inspection, and, what modifications may be required to achieve such safety margins. provide emergency low level outlet for the dam. Repair areas of spalled and deteriorated concrete on the dam appurtenances.

Book excerpt: Based on available records, past operational performance, visual inspection and Phase I engineering analysis, Lake Grinnel Dam is assessed as being in fair overall condition. Based on investigations of the downstream flood plain made in connection with this report, it is recommended that the hazard potential classification be downgraded from high to significant hazard. Hydraulic and hydrologic analyses indicate that the spillways are inadequate. Discharge capacity of the spillways is not sufficient to pass the designated spillway design flood (100-year storm) without an overtopping of the dam. The spillways are capable of passing approximately 1 percent of the SDF. Therefore, the owner should engage a professional engineer experienced in the design and construction of dams in the near future to perform more accurate hydraulic and hydrologic analyses relating to spillway capacity. Based on the findings of the analyses, the need for and type of remedial measures should be determined and then implemented. The owner should, in the near future, develop an emergency action plan together with an effective warning system outlining actions to be taken by the operator to minimize downstream effects of an emergency at the dam.

Book excerpt: Lake Panorama Dam is an earth embankment with a spillway consisting of three 12-inch concrete culvert pipes that transversely penetrate the dam. In addition to the dam, lake Panorama Dike located to the south or left of the dam, impounds Lake Panorama. An unpaved road is located on the crests of both the dam and dike which are separated by a distance of approximately 200 feet. Based on visual inspection, past operational performance, and Phase I engineering analyses, Lake Panorama Dam and Lake panorama Dike are assessed as being in fair overall condition. The spillway of the dam is capable of passing the designated spillway design flood (100-year storm) without an overtopping of the dam or dike and, therefore, is assessed as being adequate. Variations in the slope of the downstream face of the dam together with areas of seepage were observed, indicating the possibility of embankment distress. In addition, seepage at the toe of the dike was observed.

Lake Wanda Dam, initially listed as a high hazard potential structure but reduced to a low hazard potential structure as a result of this inspection, is judged to be in poor overall condition. The dam's three spillways are considered inadequate because a flow equivalent to five percent of the One Hundred Year Flood would cause the dam to be overtopped. The low hazard potential classification means that in the event of failure of the dam, no loss of life and only minimal economic loss is expected. As the hazard potential classification could change with future development, it is recommended that the classification be reviewed periodically.

The seriously inadequate spillways are assessed as an UNSAFE, non-emergency condition, until more detailed studies prove otherwise or corrective measures are completed. The classification of UNSAFE applied to a dam because of a seriously inadequate spillway is not meant to indicate the same degree of emergency as would be associated with an UNSAFE classification applied for a structural deficiency. It does mean, however, that based on an initial screening and preliminary computations, there appears to be a serious deficiency in spillway capacity so that if a severe storm were to occur, overtopping and failure of the dam would take place, significantly increasing the hazard of loss of life downstream from the dam.
