Book excerpt: Storage Dam consists of an earth embankment approximately 500 feet long with a maximum height of 36 feet from the downstream toe and a crest width of approximately 15 feet. The appurtenant structures of the dam consist of a combined primary and emergency spillway located on the left abutment (looking downstream) and outlet works near the right abutment. Based on the evaluation of the existing conditions, the condition of Storage Dam is considered to be good. Numerous seeps were observed along the left abutment, but these seeps are not considered to be a threat to the integrity of the embankment. However, a slump and an associated wet area were observed along the toe of the dam near the left abutment. This condition should be investigated and necessary repairs performed. The flood discharge capacity of the Storage Dam spillway was evaluated according to the recommended procedure and it was found to be 40 percent of the probable maximum flood (PMF) without overtopping the embankment. Therefore, according to the recommended criteria, the flood discharge capacity of the dam is classified to be inadequate.

Book excerpt: Based on the evaluation of the existing conditions on March 14 and April 16, 1979, the condition of Little Blue Run Dam is considered to be good. No conditions were noted at this time that would significantly affect the structural performance of the dam. The flood discharge facilities for the dam were designed to pass the probable maximum flood (PMF) without overtopping the embankment. Therefore, according to the recommended criteria, the flood discharge facilities of the dam are adequate. Presently, the pool elevation is approximately 170 feet below the dam crest level. The pool elevation is expected to reach spillway crest level in about the year 2010. The following recommendations should be implemented on a continuing basis: (1) The cracking observed on the crest should continue to be evaluated by the design engineers. Also, attention should be given to the slump and seepage observed on the left abutment. Erosion ditches on the right abutment should be filled; and (2) A formal warning system should be developed to alert the downstream residents in the event of emergencies.

Book excerpt: Bradford City No. 5 Dam and Reservoir is a water supply dam owned and operated by the Bradford City Water Authority. The dam is also known by the names Tuna Creek and Hazelton Mills Dam. The total length of the dam is approximately 1200 feet of which the embankment is 1100 feet and the crest width of the ogee section spillway is 93 feet. The maximum height of the dam is 68 feet and the maximum reservoir storage is 3390 acre-feet. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will pass the Probable Maximum Flood(PMF) without overtopping the dam. The spillway is therefore considered 'adequate'. The dam was found to be in good overall condition at the time of the inspcetion. However, the owner should perform several items of work without delay.

Norton Dam consists of an earthfill embankment approximately 37 feet high and 675 feet long. A 75-foot wide, concrete ogee spillway is located at the right abutment, and a 150-foot wide emergency spillway is located at the left abutment. Seepage control is provided by a steel core wall comprised of 72-inch by 96-inch by 1/4-inch steel sheets, which are riveted and welded in place. The downstream embankment rises at a slope of 2 horizontal to 1 vertical (2H:1V) to the crest at elevation 1422.4 feet. The upstream slopes rise at 2.5H:1V to elevation 1412 feet and then 2H:1V to the 12-foot wide crest. The upstream face is lined with well-placed sandstone riprap, typically 1- to 2-foot square. The ogee type spillway has a crest elevation of 1415 feet, or 7 feet lower than the top of dam. Approximately 225 feet to the left of the ogee spillway is a concrete screen chamber built into the embankment. The intake consists of two pipes, a 16-inch cast-iron pipe with an invert elevation of 1400 feet and a 36-inch cast-iron pipe with an invert at elevation 1386.0 feet.

Book excerpt: Bear Creek Lake Dam is a rock filled timber crib structure approximately 325 feet long and 17 feet high at its maximum section. An ungated drop spillway with an 158-foot long sharp crested weir is located approximately 60 feet from the right abutment (looking downstream). The 80 acre lake is used for recreation by the residents of Bear Creek Village. It is unknown when the dam was constructed. In the state inspection report of April, 1915 it is stated, 'This dam was constructed a great many years ago, but it has been repaired from time to time, so that more than likely none of the original structure is now in existence.' Examination of the results of the hydrologic and hydraulic analysis indicate that the drop spillway is capable of passing 11 percent of the Probable Maximum Flood (PMF) without overtopping of the embankment. Failure of the dam would significantly increase the hazard to loss of life downstream of the dam. Therefore, the capacity of the drop spillway is classified as 'seriously inadequate.' The dam is considered to be 'unsafe (non-emergency).'

Book excerpt: Little Shenango Dam (PA 490) is a homogeneous earth floodwater retarding dam designed by the U.S. Department of Agriculture, Soil Conservation Service. The dam has a crest length of 430 feet, a maximum height of 45 feet, a storage volume of 472 acre-feet at spillway crest level, and a storage volume of 19 acre-feet at normal pool level. The dam was found to be in generally poor condition at the time of inspection. The inspection did disclose potential problems with seepage and piping in the dam embankment and abutments. A detailed engineering investigation of seepage, piping, erosion, and structural stability of the dam should be initiated as expeditiously as possible. Hydraulic/hydrologic evaluations revealed that the spillway will pass the Probable Maximum Flood (PMF) without overtopping the dam. The spillway is therefore considered 'adequate'.

The spillway is a 12.7 foot long broad crested weir with a center pier that is 2.1 feet wide. The outlet works for the dam is an 8-in. cast iron pipe through the dam. Details of the inlet and the operational status of the outlet works could not be determined. Perch Pond Dam is judged to be in good structural condition, based on the visual inspection. Since the spillway cannot pass the 100-year flood without overtopping the dam, the spillway is rated as inadequate.

Erosion of the upstream slope has reduced the embankment crest to a width of 4 feet. Minor seepage was observed on the downstream slope of the dam, and a wet area was observed immediately beyond the toe of the dam. Brush and trees litter the downstream slope of the dam. No determination could be made as to the actual condition of the drainline culvert through the embankment. No determination could be made as to the condition or type of upstream closure on the culvert or whether the control is operable. The spillway located at the right abutment of the dam is in a deteriorating condition. Erosion exists along the right outside edge of the spillway wingwall. If the erosion is left unchecked, the potential for failure of the spillway and dam exists. The Chambers Dam is a high hazard-small size dam. The recommended Spillway Design Flood (SDF) for a dam of this size and classification is in the range of 1/2 PMF to PMF. Based on the downstream potential for loss of life and property damage, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling less than 1% of the PMF without overtopping the embankment low spot.

The size classification of the facility is intermediate and the hazard classification is considered to be high. The emergency spillway is capable of discharging the peak inflow resulting from a storm of PMF intensity and is, therefore, considered adequate. It is recommended that the owner: (a) Repair the damaged pond drain gate control and replace the missing manhole cover atop the service spillway riser. Consideration should be given to redesigning the gate control mechanism to protect it from future vandalism. (b) Consult with the U.S.D.A., Soil Conservation Service, to assess the erosion on the upstream slope in the area of the service spillway riser and implement any remedial measures deemed necessary. (c) Curtail the unauthorized vehicular travel across the embankment and repair and reseed any damaged (bare) areas. If control of unauthorized travel is not practical, some provision should be made to protect the embankment from rutting and erosion by placing a layer of gravel or stone across the crest. (d) Develop a formal operation and maintenance manual to ensure the continued proper care of the facility. In addition, a formal warning system should be implemented which provides detailed procedures to protect the lives and property of downstream residents. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy percipitation.

The recommended Spillway Design Flood (SDF) for the size and hazard classification of the dam is the Probable Maximum Flood (PMF). The existing spillway will pass about 84 percent of the PMF before overtopping of the dam occurs. The spillway capacity is rated as inadequate. Several deficiencies were observed at the dam. The most serious is the probable lack of an adequate impervious barrier through the dam. Although some maintenance has been performed, the existing maintenance program could be upgraded. The following remedial measures, listed in approximate order of priority, are recommended to be undertaken by the Owner without delay: (1) Clear brush and debris on and adjacent to the dam. (2) Engage the services of a professional engineer experienced in the design and construction of dams. Utilize his advice in removing the trees on and near the embankment. If any seepage becomes evident after the brush, trees, and debris are removed, the condition should be assessed by the engineer. (3) Fill the slots by the spillway bridge with concrete or other suitable impervious material. (4) Fill the burrowing animal hole adjacent to the spillway with suitable impervious material.
