Book excerpt: The assessment of the Hamilton Lake Dam is based upon visual observations made at the time of inspection (June 28, 1979), review of available records and data, review of available data, hydrology and hydrologic and past operational performance. The inspection and review of data of Hamilton Lake Dam did not reveal any problems which require emergency action. The dam appears to be stable, well maintained, safely operated and in good condition. Hamilton Lake Dam is a high hazard-intermediate size dam. The spillway design flood is the PMF (Probable Maximum Flood). The spillway and reservoir are capable of controlling the PMF. Based upon the criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted immediately: (1) Continue annual safety inspections and operation and maintenance inspections; and (2) A warning system should be instituted to warn downstream residents of large spillway discharges or failure of the dam.

The spillway and reservoir are capable of controlling the spillway design flood (PMF). Based on criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted as soon as possible. (1) A regularly scheduled maintenance program should be developed and implemented. (2) A warning system should be developed to warn downstream residents of large spillway discharges or imminent failure of the dam. (3) A safety inspection program should be implemented with inspections at regular intervals by qualified personnel. (4) The drainline valve should be operated on a regular basis.

The dam is a zoned earth embankment with a center core consisting of silty clay material and the upstream and downstream portions consisting of weathered shale. The upstream and downstream slopes were constructed at 2.5H:1V with berms at elevation 582.0. The crest of the dam is 12 feet wide and forms an access road at elevation 605.5. The outlet works consist of a concrete control tower connected to two 36 in. steel pipes encased in concrete. The outlet works has a positive upstream shut off in the form of a bulkhead. The flow through these pipes is regulated by a 36 in. sluice gate, an 18 in. cone valve or an 8 in. by pass valve. At the toe of the dam is a valve house to control flow through these pipes. The earthen emergency spillway is located on the left abutment and contains a concrete sill 750 feet long. The spillway discharge channel is 1300 feet long and discharges to a natural stream. The embankment is 53 feet high and 1360 feet long. The axis of the dam has a 137 deg. bend in the upstream direction 860 feet from the left abutment. The emergency spillway is located on the right (north) abutment. The spillway is 170 feet wide and is open cut in shale and weathered shale. The principal spillway consists of 48 inch concrete pipe with a reinforced concrete riser inlet. Extending upstream of the riser is a 36 inch concrete pipe with a reinforced concrete reservoir drain inlet at the upstream toe of the embankment. At the downstream end of the 48 inch diameter pipe is a concrete impact stilling basin. The embankment and appurtenant structures appear to be in good condition and well maintained. However, the presence of the seepage zones and wet areas are of concern. The long term effect of these areas on the stability of the embankment is unknown. The existing spillways and reservoir are capable of controlling the PMF (probable maximum flood). Based upon criteria established by the Corps of Engineers, the spillway is termed adequate.

A 4-inch drain tile located below the downstream portion of the embankement. The spillway is located on the right abutment and consists of a concrete lined chute. The spillway weir consists of a concrete ogee section. The drainline consists of a 30-inch cast iron pipe with a control tower located in the upstream portion of the embankment. Flow in the drainline is regulated at the top of the dam through a stem in the control tower to the valve on the 30-inch line. The drainline discharges into the emergency spillway exit channel. The crest width of the dam is 17 feet. The upstream slope is 3H:1V and the downstream slope is 2.5H:1V. Both the upstream and downstream slopes are grass covered. Some riprap exists on the upstream slope. The principal spillway consists of a reinforced concrete riser and a 30 inch reinforced concrete pipe which passes under the dam. The top of the riser unit is provided with an anti-vortex device. A gate valve is provided on an inlet pipe just inside the riser unit. Based on our hydraulic and hydrologic analysis it was determined that the spillway and reservoir are capable of controlling approximately 52% of the PMF. Based on criteria established by the Corps of Engineers, the spillway is termed inadequate. No major deficiencies were observed during the inspection. Seepage was noted at the left abutment contact and did not appear to be excessive. Erosion was observed at the outlet structure for the spillway and drainline.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the facility is considered to be in good condition. The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.
