Malgré l'amélioration de leurs propriétés biomécaniques et esthétiques, environ 30 à 40% des restaurations en matériaux composites sont remplacées dans les 5 ans qui suivent leur mise en place. Ceci est principalement dû à l'altération de la couleur du matériau composite. Il est admis que la finition et le polissage de ces matériaux améliore l'esthétique, l'état de surface, préserve l'intégrité marginale, réduit les caries secondaires et participe à la bonne santé parodontale, ce qui va de pair avec une pérennité des restaurations. Il n'y a actuellement aucun consensus concernant les techniques de finition et de polissage. De plus, la multitude de dispositif de finition et de polissage ne permet pas au chirurgien-dentiste de s'y retrouver. Nous avons fait un inventaire des différentes procédures de finition et de polissage des matériaux composites utilisés en techniques directe, au travers d'une revue systématique de la littérature d'études publiées de 2010 à mars 2016. Ce travail servant à définir les techniques les plus adaptées permettant d'assurer la pérennité des restaurations en matériaux composites, par l'étude de la stabilité colorimétrique et de l'état de surface.

Ils sont utilisés dans de nombreux domaines et plus particulièrement pour les restaurations dentaires. Plusieurs facteurs tels que l'étanchéité, le mimétisme et l'esthétique sont indispensables au maintien à long terme, au sein de la cavité buccale, des restaurations réalisées en technique directe. Les techniques de finition et de polissage assurent l'intégration esthétique et fonctionnelle de ces matériaux et favorisent leur succès à long terme. En 2007, une revue de la littérature faisait l'état des connaissances scientifiques sur les méthodes de finition et de polissage disponibles sur le marché pour les ciments verres ionomères. Les résultats obtenus démontraient que l'instrumentation idéale pour leur finition et leur polissage n'était pas encore disponible. Cependant, les séquences de disques Sof-Lex® semblaient être les plus appropriées pour la finition et le polissage de ces matériaux. Les recherches cliniques et industrielles permettent une constante évolution des techniques et des matériaux. L'objectif de cette thèse d'exercice est de recenser au travers de la littérature scientifique, les instruments, les méthodes de finition et de polissage les plus appropriées pour les ciments verres ionomères.

Ces dernières possèdent des propriétés mécaniques et esthétiques de plus en plus performantes, grâce, notamment, à l'évolution de la chimie de la phase résineuse mais également, grâce à l'augmentation de la teneur en charges. Les composites directs majoritairement utilisés aujourd'hui sont des composites microhybrides ou microhybrides nanochargés. Actuellement, quand on envisage une restauration en composite, le débat porte majoritairement sur le choix des matériaux et sur la technique de stratification. La finition et le polissage des composites, souvent réalisés très rapidement en fin de séance, restent les étapes les plus négligées. Si les protocoles de stratification des composites sont aujourd'hui bien codifiés, aucun consensus n'existe, à l'heure actuelle, pour le polissage des composites. Par ailleurs, il est nécessaire de corréler le système de polissage à la nature du composite. En 2017, une thèse de revue de la littérature sur le polissage des composites a été réalisée à l'université de Bordeaux. La thèse conclut que le système majoritairement étudié et utilisé dans la pratique est le système de disques diamantés Sof-Lex®. D'après ces études, le Sof-Lex® permet d'obtenir la surface la moins rugueuse. En revanche, aucune étude ne fait mention du respect de l'intégrité des tissus dentaires bordant la restauration. Le polissoir idéal doit certes permettre de lisser et polir la surface en composite, mais il doit être suffisamment sélectif pour préserver au maximum l'émail adjacent. L'objectif de ce travail de thèse est de comparer deux séquences de polissage : le Sof-Lex® et un nouveau système commercialisé par Komet conçu pour optimiser le polissage du composite tout en préservant les tissus adjacents : le Kompoline®. Une étude ex-vivo a été réalisée sur dents avulsées. L'intégrité des tissus dentaires bordant la restauration a été observée au microscope numérique. Ce travail de recherche pourrait aboutir à une prise de conscience du délabrement dentaire généré par les séquences de polissage actuellement plébiscitées.
