No curative treatment exists for hormone resistant prostate cancer, but immunotherapy has the potential to prevent disease recurrences and to destroy metastases. We investigated the efficacy of immunotherapy in the RM-1 model of prostate cancer. RM-1 cells are aggressive, non-immunogenic mouse prostate cancer cells that express very little MHC I, similar to many human prostate cancers. Our goal was to develop a whole cell tumor vaccine that would generate a protective T cell-dependent antitumor response.

Book excerpt: This idea development award developed a micro-simulation model of prostate cancer screening and treatment. As described in our initial proposal, we developed an detailed model of the natural history, screening, and treatment of prostate cancer organized around a Markov model that incorporated true stage and grade of tumor, misclassification of tumors that generates discrepancies between clinical and true stage, and other innovative features that had been neglected in prior models. We also extended the model to account for persistent heterogeneity that would not be adequately modeled by usual Markov models. We also did a detailed structured literature review to estimate key parameters of our model and developed new techniques to estimate the parameters of the natural history model by fitting it to aggregate and individual-level data from the SEER program. Our baseline analysis suggests that screening and treatment will increase life expectancy but decrease quality-adjusted life expectancy. Potential gains are largest in men ages SO-70 and when undiagnosed cancer creates substantial anxiety. We published a paper based on some initial analyses of treatment patterns, have a main cost-effectiveness manuscript in progress, and received ROl funding from the National Cancer Institute to further extend this work.

Book excerpt: The overall goal of our proposal is to develop a new transgenic model of prostate cancer by targeting transformation to prostatic basal cells using the prostate stem cell antigen promoter (PSCA). A 9kb region upstream of the PSCA coding sequence was isolated and demonstrated to drive prostat-specific expression in vitro. This region was subsequently tested in vivo using a GFP reporter construct. Analysis of the PSCA promoter-GFP transgenic mice have demonstrated relatively prostate-specific expression of GFP. GFP expression in these mice is regulated developmentally, with prominent expression localized to the ductal tips of the prostate. Promoter activity appears to be androgen responsive and androgen independent. The PSCA promoter can drive target gene expression to the prostate of mice. Expression is restricted to a subset of prostatic cells, potentially the progenitor cells of the prostate. We are currently using this promoter to create new transgenic models of prostate cancer.

Book excerpt: The objective of the development of Internet-accessible prediction models is to enhance the diagnosis accuracy, treatment efficacy and prognosis for patients with carcinoma of prostate cancer (CaP). On the basis of the established tools (Oracle database, Internet-accessible data collection applications, and program packages for data retrieval and standardization) in the first year of the support, the roles of a set of variables (race/ethnicity, diagnostic age, labs, delayed treatment interval, treatment types, early and late hormonal therapy, etc.) on the outcome of CaP patients were analyzed. The results show that delayed surgery over 3 months post diagnosis would impact the outcome of patients with high risk disease (submitted to J Urol). Early hormonal therapy can delay clinical metastasis only in men with high risk disease (J Urol, in press). Race, percentage of cancer positive cores over total biopsy cores, surgical margin status and seminal vesicle invasion are independent prognostic variables for men with intermediate risk disease (submitted to J Urol). Race plays an important role in tumor volume and spatial distribution based on 3 dimensional reconstructed models with radical prostatectomy specimens (submitted to J Urol).
