La multiplication des nouvelles missions nécessite une sécurisation de l'activité. Chaque étape doit être codifiée pour permettre à tous d'avoir la même action. La qualité est un synonyme de sécurité non seulement pour les patients, mais aussi pour le pharmacien. La non-qualité expose les patients à des risques significatifs pour la santé, peut aussi entraîner une rupture de confiance entre le patient et les professionnels de santé et est source d'une perte financière pour l'entreprise. Une amélioration de l'efficience de la prise en charge des patients est donc nécessaire pour limiter les conséquences sanitaires et financières de la non-qualité. Depuis de nombreuses années, l'Ordre National des Pharmaciens incite fortement les pharmaciens à mettre en place une démarche qualité. Pour cela, ils ont élaboré un certain nombre d'outils afin d'aider les pharmaciens dans leur démarche. Dans ce travail, nous envisagerons le démarrage de la mise en place de la qualité dans une officine, lieu de notre stage de sixième année. Pour commencer, nous étudierons la législation et les outils disponibles pour démarrer une démarche qualité, ainsi que le management de l'équipe officinale, et nous nous intéresserons à la façon d'établir l'état des lieux de l'officine. Ensuite, nous mettrons en pratique la méthodologie choisie, c'est-à-dire la mise en place de questionnaires de satisfaction des patients et de l'évaluation de l'état des lieux de l'officine. Pour finir, nous analyserons les résultats obtenus et envisagerons des solutions aux problèmes soulevés, notamment par la réalisation de procédures.

Parmis elles, la dispensation sous protocole a pour objectif de fluidifier certains parcours de soins. Ce travail a pour but d'élaborer une formation la plus efficace et la plus proche possible des attentes des professionnels pour leur permettre de mener à bien ce nouveau dispositif.Pour cela, une première partie présente la dispensation sous protocole mais également les techniques qui vont être utilisées au cours de l'étude (la simulation). Un travail expérimental reposant sur deux parties fait suite. La première a pour but d'évaluer le positionnement des pharmaciens d'officine face à cette nouvelle mission. La seconde permet de tester une approche pédagogique encore peu développée en pharmacie : l'apprentissage par simulation. Après analyse des résultats, il en ressort un grand interêt des pharmaciens d'officine pour cette nouvelle mission et une envie de formation. La technique de simulation ne semble pas etre plus efficace qu'un apprentissage conventionnel mais plaît beaucoup plus aux étudiants qui semblent plus impliqués au sein de la formation.

Book excerpt: La loi HPST a redéfini il y a 10 ans les missions du pharmacien d'officine dans le parcours de soin. Certains pharmaciens se sont ainsi investis dans la réalisation de nouvelles missions d'éducation avec la mise en oeuvre d'entretien pharmaceutiques, de bilans de médication partagés. Cependant, l'implantation de ces nouvelles pratiques semble encore difficile dans beaucoup de pharmacies. De ce fait, nous avons souhaité comprendre comment les pharmaciens vivaient leurs missions d'éducation du patient à l'officine, ainsi que leurs freins et leviers à la mise en oeuvre de celles-ci. Pour répondre à cette problématique, une recherche qualitative inductive a été menée à partir d'entretiens semi-directifs auprès de 12 pharmaciens du département des Alpes-Maritimes durant l'été 2019. Les résultats de cette étude ont permis de décrire des pratiques éducatives très variables selon les pharmaciens. Si certains pharmaciens ne souhaitaient pas développer ces pratiques du fait de leur objectif de vente prépondérant, d'autres souhaitaient réellement s'investir dans ce nouveau champ, mais se sont retrouvés confrontés à différentes difficultés : confusion entre ETP et entretiens pharmaceutiques et bilans de médication partagés, manque de compétences et d'accompagnement, manque de reconnaissance des médecins généralistes, contexte économique de la pharmacie, etc. De nombreuses tensions d'impuissance d'action ont émergé lors des entretiens. Ce premier travail de recherche pointe l'importance d'un dispositif d'accompagnement de pharmaciens d'officine dans leurs missions d'éducation des patients qui pourrait être évalué dans une recherche-action ultérieure.
