Book excerpt: But de l'étude : Le but de l'étude est de savoir si les consultations pharmaceutiques mises en place permettent une transmission efficace et durable du bon usage des traitements anticoagulants oraux auprès des patients. Protocole : Il s'agit d'une étude prospective de type avant/après. La connaissance des patients sur leur traitement anticoagulant oral est évaluée juste avant la consultation pharmaceutique, juste après cette consultation et 1 mois après cette consultation. Malades : Les patients inclus sont des patients hospitalisés dans le service de médecine du Centre Hospitalier de Wattrelos et qui sont sous traitement anticoagulant oral (de type Anti-Vitamine K ou Anticoagulant Oral Direct). 77 patients n'ont pas été inclus, 54 patients ont été inclus. Sur les 54 patients inclus (ayant bénéficié de la consultation), 43 patients ont bénéficié du suivi téléphonique. Intervention : L'inclusion des patients s'est déroulée sur une période de 6 mois. Leurs connaissances ont été évaluées à l'aide de questionnaires (questionnaire identique pour la consultation dans le service et le suivi téléphonique). Critère de jugement principal : le critère de jugement principal était l'augmentation significative du score de connaissance grâce à la consultation (avant consultation versus juste après consultation, ainsi qu'avant consultation versus 1 mois après consultation). Résultats : Le score moyen des 54 patients était de 10,7/20 avant consultation contre 18,4/20 juste après consultation (augmentation significative, p-value

Cependant, leur principale limite est leur mode d’administration. Ces anticoagulants ne peuvent être administrés ne peuvent être administrés que par voie parentérale en raison de leur masse moléculaire élevée et de leur forte charge anionique. Sans contexte, la voie orale constitue la voie d’administration privilégiée permettant une sécurité dans l’administration et un idéal de confort et de simplicité pour les patients. Ainsi, la thérapie anticoagulante orale reste à ce jour un véritable défi. Pendant plus de 60 ans, les antivitamines K (AVK) représentaient le seul traitement anticoagulant oral. Cependant, les AVK présentent de nombreux inconvénients, notamment une variabilité interindividuelle importante, des effets tératogènes, de nombreuses interactions médicamenteuses ou encore la nécessité d’une surveillance biologique étroite lors de la mise en place du traitement. Récemment, le développement de nouveaux anticoagulants oraux (NACOs) est devenu un enjeu de santé publique. La mise sur le marché de trois NACOs dans la prévention et le traitement des désordres thromboemboliques a permis de réduire la prescription des AVK. Néanmoins, le recul sur le profil bénéfice/risque est encore trop court pour pouvoir les utiliser en remplacement des héparines et du fondaparinux. Dès lors, face à l’utilisation fréquente et plus répandue de ces anticoagulants, le développement d’une forme orale de ces polysaccharides est resté d’actualité. L’objectif de cette revue bibliographique est de résumer les avancés de la thérapies anticoagulante, de la mise sur le marché de nouvelles molécules anticoagulantes à la mise au point de formes galéniques innovantes des anticoagulants de référence, les héparines et le fondaparinux.
