Protocole : Il s'agit d'une étude prospective de type avant/après. La connaissance des patients sur leur traitement anticoagulant oral est évaluée juste avant la consultation pharmaceutique, juste après cette consultation et 1 mois après cette consultation. Malades : Les patients inclus sont des patients hospitalisés dans le service de médecine du Centre Hospitalier de Wattrelos et qui sont sous traitement anticoagulant oral (de type Anti-Vitamine K ou Anticoagulant Oral Direct). 77 patients n'ont pas été inclus, 54 patients ont été inclus. Sur les 54 patients inclus (ayant bénéficié de la consultation), 43 patients ont bénéficié du suivi téléphonique. Intervention : L'inclusion des patients s'est déroulée sur une période de 6 mois. Leurs connaissances ont été évaluées à l'aide de questionnaires (questionnaire identique pour la consultation dans le service et le suivi téléphonique). Critère de jugement principal : le critère de jugement principal était l'augmentation significative du score de connaissance grâce à la consultation (avant consultation versus juste après consultation, ainsi qu'avant consultation versus 1 mois après consultation). Le pharmacien d'officine peut réaliser des entretiens pharmaceutiques pour les antivitamines K (AVK), l'asthme et désormais pour les anticoagulants oraux directs (AOD). La mise en place des entretiens pharmaceutiques au sein d'un hôpital pour tous les patients recevant un traitement anticoagulant a permis de rencontrer les patients, de comprendre leurs attentes et de créer des documents leur étant destinés. Un lien ville-hôpital a été amorcé afin d'inclure le pharmacien d'officine dans le processus de soin centré sur le patient. Une étude sur les connaissances des patients traités par AOD a été réalisée en parallèle afin d'évaluer l'état des connaissances des patients sur ces médicaments. Trente patients sur une durée de six mois ont été interrogés afin de tester nos outils et d'évaluer leurs connaissances avant l'entretien pharmaceutique. Notre étude a mis en lumière le manque de connaissances des patients au sujet de leur traitement, en particulier sur le risque hémorragique des médicaments anticoagulants. Il est nécessaire d'informer les patients des risques liés à leurs médicaments tout en encourageant l'observance médicamenteuse. Cette gymnastique n'est pas chose facile, c'est pourquoi les entretiens doivent être structurés et documentés. Les patients sont très souvent en quête d'informations au sujet de leur santé. Ils recherchent une prise en charge individualisée pouvant être mise en place par une collaboration patient-soignants.

L'arrivée des anticogulants oraux directs dans l'arsenal thérapeutique n'a pas écarté le risque hémorragique lié à la nature du traitement et l'utilisation des AVK devrait perdurer encore de nombreuses années. L'entretien pharmaceutique est une nouvelle mission de santé publique confiée au pharmacien, dont l'objectif est de réduire la iatrogénie des traitements anticoagulants oraux à travers l'éducation du patient et le développement de la coopération interprofessionnelle. Cette thèse a pour but d'apporter des informations et des outils pratiques (aide-mémoire, fiche bilan) à destination des pharmaciens officinaux, afin de les aider à appréhender et mettre en place ces entretiens pharmaceutiques. Elle contient pour cela des rappels sur la pharmacologie et l'utilisation des traitements anticoagulants oraux (antivitamines K et anticoagulants oraux directs). Les notions de base de la communication en santé; indispensables à connaître pour envisager une bonne communication avec les patients et les autres professionnels de santé sont également présentées. A titre informatif, nous effectuerons un tour d'horizon des différents types de suivi des traitements AVK ayant été expérimentés depuis les 15 dernières années en France ainsi que des différentes missions du pharmacien dans les pays européens et nord-américains. En prenant exemple sur la mise en place des entretiens AVK, des outils adaptés aux entretiens pharmaceutiques des patients sous anticoagulants oraux directs AOD ont été créés et évalués par un panel de pharmaciens en officine pendant 3 mois de fin 2013 à début 2014. Une fiche de suivi patient comprenant le questionnaire d'entretien et un guide d'accompagnement permettant d'aider le pharmacien lors de l'entretien ont été élaborés ainsi que des outils à remettre aux patients : carte d'anticoagulation et livret d'information. La satisfaction, l'utilité, la facilité d'utilisation, l'aide apportée par ces outils, les difficultés rencontrées ainsi que les points à ajouter sur ces outils ont été évalués à l'aide d'une grille d'évaluation au décours de l'entretien. Sur 67 pharmaciens contactés, seulement 15 grilles d'évaluation ont été récoltées, néanmoins les résultats sont intéressants. Après analyses, les outils ont été jugés utiles et satisfaisants, les remarques recueillies ont permis de les faire évoluer. Cependant, les entretiens pharmaceutiques paraissent longs pour les pharmaciens (entre 20 et 40 minutes), ils sont pourtant une opportunité intéressante qui permet de diversifier ses compétences et d'avoir un contact privilégié avec le patient afin de valoriser le métier de pharmacien.

Nous avons ensuite conçu des outils pédagogiques (EVA, baromètre, cartes situations, etc.) adaptés au format de ces entretiens. Il nous semblait important de proposer ce dispositif à un maximum de patients issus de services de soins variés. Nous avons alors décidé d'impliquer les internes en Pharmacie ainsi que les étudiants en Pharmacie en 5ème année hospitalo-universitaire. Pour cela, ils ont suivi au préalable une formation théorique et pratique. Enfin, l'instauration d'un lien ville-hôpital nous a paru essentiel afin d'optimiser la prise en charge du patient. Au cours des années 2014-2015, 28 étudiants ont été formés à la réalisation d'entretiens pharmaceutiques par le biais de rappels théoriques et de mises en situations pratiques (jeux de rôle). Entre mars 2014 et juillet 2015, 142 entretiens ont été menés au CHU de Nancy. L'évaluation des connaissances du patient a été réalisée à l'aide d'un quizz. Pour les patients sous AVK ayant bénéficié de 2 entretiens, la différence de pourcentages de réponses correctes au quizz entre les séances est statistiquement significative : 54,0% pour la séance 1 versus 73,7% pour la séance 2 (p
