This report (1) examines the services provided, population served, and administrative challenges faced by MAI grantees; and (2) describes the best practices identified by grantees and other stakeholders for community outreach and capacity building. Book excerpt: MAI was established in 1998 in response to growing concern about the impact of HIV/AIDS on racial and ethnic minorities in the United States. According to the most recent CDC data, there were an estimated 23,734 AIDS diagnoses in 2011 among persons of minority races/ethnicities, accounting for 74 percent of total AIDS diagnoses in the United States and dependent areas; these data reflect an overall trend present since 1994. National HIV/AIDS data indicate that since 1994 minorities have become a significant majority of persons with HIV/AIDS. In 2011, Blacks/African-Americans accounted for 13 percent of the U.S. population, but accounted for 49 percent of AIDS diagnoses; Hispanics/Latinos accounted for 17 percent of the population, but accounted for 21 percent of AIDS diagnoses; and Caucasians/Whites accounted for 74 percent of the population, but accounted for 26 percent of AIDS diagnoses.

GAO conducted a review of services reported in fiscal year 2011 MAI grantee annual reports from a generalizable sample of 100 grantees, and interviewed agency officials and other stakeholders. GAO also reviewed grant administrative requirements, and data on MAI grant amounts and populations served. "

The Supplemental Technical Assistance, Testing and United Services (STATUS) Project is a key part of NASTAD's response to the Minority AIDS Initiative (formerly known as the Congressional Black Caucus Initiative) to better serve communities of color disproportionately impacted by HIV/AIDS. The STATUS Project focuses on three primary areas: 1.) technical assistance and capacity building assistance (TA/CBA) provided by health departments to minority CBOs; 2.) targeted testing efforts (including the use of rapid tests); and 3.) referral efforts linking HIV counseling and testing with medial and other support services. Through this research project, funded by CDC in August 1999, NASTAD had the opportunity to identify model health department programs and initiatives and to position health departments to lead future national initiatives in these areas. This report examines only the first area of focus--technical assistance and capacity building; a separate report will examine issues relating to targeted testing and referrals." - p. 3

Under the CARE Act, funding for UA is provided through three categories of grants: (1) formula grants that are awarded based on the case counts of people with HIV/AIDS in an UA; (2) supplemental grants that are awarded on a competitive basis based on an UA's demonstration of need; and (3) Minority AIDS Initiative grants, which are awarded to UA to address disparities in access, treatment, care, and health outcomes. This report developed an estimate of CARE Act funding with the stop-loss provision in, and also developed an estimate of such funding without that provision. Ill.
