Book excerpt: This study explores the appropriateness of health-related Millennium Development Goals (MDGs) for the countries of the Europe and Central Asia region, and the implications for policy choices at the regional, subregional and country levels. Findings include that, for the region as a whole, proportionately more gains in life expectancy would accrue from the control of non-communicable diseases (NCDs) than from achieving the targets in the classic MDGs. Other issues highlighted include the importance of reducing morbidity and premature mortality from NCDs and external causes across the entire region; and the need to focus on the classic MDG indicators (infant mortality rates, under-five mortality rates and maternal mortality ratios) in the countries of Central Asia and the Caucasus.

Book excerpt: This report reviews the status of the 28 countries of Europe and Central Asia (ECA) with respect to the environmental Millennium Development Goal (MDG). The aim of this goal is to 'ensure environmental sustainability,' which is elaborated by a set of three targets and eight indicators. The indicators for the environment MDG are important not only as measures of environmental sustainability, but also as contributors to the health and poverty goals. In ECA, these linkages are clearly brought out for the water supply and sanitation indicators, but also apply to the carbon reduction, forestry and biodiversity indicators. The three targets and its indicator are: (i) halve, by 2015, the proportion of people without sustainable access to safe drinking water and basic sanitation: (a) proportion of population with sustainable access to an improved water source; and (b) proportion of population with access to basic sanitation; (ii) integrate the principles of sustainable development into country policies and programs and reverse the losses of environmental resources: (a) energy use (kg oil equivalent) per $1 GDP(Purchasing Power Parity(PPP)); (b) carbon dioxide emissions (per capita); (c) proportion of land area covered by forest; (d) ratio of area protected to maintain biological diversity to surface area; and (e) proportion of population using solid fuels; (iii) by 2020 to have achieved a significant improvement in the lives of at least 100 million slum dwellers: (a) proportion of households with access to secure tenure.

This report is about how to improve health system outcomes in countries in the ECA region. This report, which explores the development challenge facing health sectors in ECA, identifies three key agendas for achieving more rapid convergence with the world's best-performing health systems: the first is the health agenda, in which the main imperative is to strengthen public health and primary-care interventions to help achieve the cardiovascular revolution that has taken place in the West in recent decades. The second is the financing agenda, in which growing demand for medical care must be satisfied without imposing an undue burden on households, by achieving better financial protection, or on government budgets, by ensuring a more efficient use of resources. The third agenda relates to broader institutional arrangements. Here, a few key reform ingredients are identified, each of which is common to most advanced health systems but lacking in many ECA countries.

The framework focuses on the service delivery aspects of achieving the MDGs and emphasizes: a) monitoring outcomes to identify groups which are not achieving MDGs, b) identifying interventions which are effective in raising MDG performance, c) targeting interventions to groups which are not meeting MDGs, and d) strengthening incentives to service providers and users to raise MDG outcomes for these groups. The framework is illustrated with case studies on health and education in the Kyrgyz Republic. As the cases studies show, application of the approach requires good information on the status o f MDG attainment, and the constraints to fuller attainment - including, crucially, qualitative dimensions of MDG attainment, and resource constraints. The Kyrgyz Republic was chosen to illustrate the framework because prior analytical and project work provide a relatively rich base of knowledge and experience and because there is significant government ownership of the key principles involved in the approach. The case studies do not aim to present new analysis of the technical issues in the health and education sectors in the Kyrgyz Republic. Rather, they aim to bring together relevant existing sector knowledge and lessons of experience in order to derive the options for moving ahead more aggressively on MDG attainment in the health and education sectors.

Subtitled "Confronting the Challenges of Gender Equality and Fragile States," this year's report highlights two key thematic areas-gender equality and empowerment of women (the third MDG) and the special problems of fragile states, where extreme poverty is increasingly concentrated. The report, which is jointly issued by the World Bank and the International Monetary Fund, argues that gender equality and the empowerment of women are central to the development agenda. This is because gender equality makes good economic sense and because it helps advance the other development goals-including education, nutrition, and reducing child mortality.
