Book excerpt: In response to concern about increases in the federal debt and deficit, prominent policymakers and commissions have proposed reducing federal spending on entitlement programs, including Medicare. One of the most frequently discussed proposals would transform Medicare from a defined benefit program, in which beneficiaries are guaranteed coverage for a fixed set of benefits, to a defined contribution or "premium support" program, in which beneficiaries are guaranteed a fixed federal payment to help cover their health care expenses. The idea of transforming Medicare into a system of premium supports, establishing fixed contributions per Medicare beneficiary, has been raised intermittently since the early 1980s, and more recently, a number of proposals have been advanced in the context of reducing the federal deficit and debt. Premium support proposals differ in a number of ways, including how federal payments on behalf of beneficiaries would be determined; whether Medicare fee-for-service would remain an option; what protections would be provided for low-income beneficiaries; and the extent to which the proposal would rely on a federal cap on Medicare spending to help constrain the growth in program spending. These differences have important implications for beneficiaries, Medicare program spending, health care providers, and health plans. This document provides a side-by-side comparison of the most recent proposals for transforming Medicare into a premium support program that have been put forth by policymakers and commissions as part of broad-based deficit- and debt-reduction packages.

The primary goals of a premium support system are to reduce the growth in Medicare spending, and rely more on a competitive marketplace. While the parameters of various premium support proposals differ, the general idea is for the federal government to make a predetermined contribution on behalf of each person on Medicare that would be applied toward the premium for a health insurance plan. This approach contrasts with the current Medicare program under which Medicare beneficiaries are entitled to a defined set of benefits, with the federal government contributing to the cost of these services, provided by either the traditional fee-for-service (FFS) program or Medicare Advantage plans.

The benefit package is uniform and premiums do not rise with age. As Marilyn Moon shows in this important work, however, the characteristics of older women make them extremely vulnerable to gaps in Medicare coverage and other features of the program. Because older women are more likely to suffer from multiple chronic illnesses than men, Medicare's failure to cover most outpatient prescription drugs affects them the most. Because women live longer on average than men and have lower incomes from pensions and other sources, they are less able to meet the out-of-pocket costs associated with Medicare or supplemental insurance. Moon evaluates Medicare reform proposals with an eye to whether they would alleviate or worsen the existing position of women in Medicare. She argues that making Medicare benefits more comprehensive and simplifying the program's rules would improve the health of older women and reduce their financial burdens.

Book excerpt: This brief examines key Medicare provisions included in "The Path to Prosperity: Restoring America's Promise," a long-term budget proposal released by House Budget Chairman Paul Ryan on April 5, 2011, which outlines a strategy for reducing federal spending and reducing the national debt over time. The Medicare provisions are among the many significant changes to programs affecting the elderly and disabled in the "Path to Prosperity" proposal. The central Medicare proposal would transform the program from one that helps pay for a defined set of benefits to one that provides "premium support" payments to private health insurers on behalf of Medicare enrollees, beginning in 2022. Under the plan, the government would contribute a pre-determined amount toward the cost of private health insurance, with beneficiaries responsible for costs above that amount. The annual increase in the government contribution would be limited to the consumer price index, a measure of general inflation. Under the proposal, a typical 65-year-old retiring in 2022 would be expected to devote nearly half their monthly Social Security checks toward health care costs, more than double what they would spend under current Medicare law, according to the analysis. The brief also describes other Medicare provisions included in the proposal, including gradually raising Medicare's age of eligibility from 65 to 67, and repealing provisions of the 2010 law that would have closed the Medicare drug benefit's coverage gap, or "doughnut hole," and created an Independent Payment Advisory Board (IPAB). The brief is a product of the Foundation's new Project on Medicare's Future, which focuses on producing timely analysis of leading Medicare deficit reduction options and other reforms affecting people on Medicare.
