The task imposed, although apparently not difficult, was therefore of moment, involving, as it did, the careful collection of details, and verification of data. It thus occurs that this little book, commenced over a year ago, and written under varying circumstances in many places, is only now completed. The authors have endeavored to give the simple facts in a plain way, and trust they may prove of as much interest to brother officers and others as their setting forth has been to themselves. Book excerpt: From the Preface and Introduction - The Medical Department: Medical Service in the European Theater of Operations is the second of three volumes recounting the overseas activities of the U.S. Army Medical Department in World War II. Charles M.Wiltse's volume on the Mediterranean and minor theaters was published in 1963, and a completed manuscript dealing with the war against Japan now exists in the Center of Military History. These volumes deal primarily with the operational and organizational history of Army medicine in the theaters, as distinguished from the clinical volumes published by the Office of the Surgeon General. In each case the combat narrative has been drawn from relevant volumes in the United States Army in World War II series, as well as from the large body of subsequent scholarship. Our aim has been to show how the military medical system organized itself in a combat theater; how medical planning was integrated with logistical and tactical planning; how medical troops were organized, trained and deployed; how hospitals were built and supplies assembled and moved forward; and how casualties were treated and evacuated from the field of battle. Although readily admitting the importance of combat service support forces, military students and historians alike tend to concentrate on combat and combat support units when studying operations, giving only passing attention to the vital work of the logisticians, signalmen, transport troops, and the rest. This is regrettable, for the operations of combat service support units-especially in a global conflict like World War II with its vast distances and varied terrains-have much to teach us about modern warfare, lessons that remain of surpassing importance to our profession. The Medical Department: Medical Service in the European Theater of Operations supports the proposition that the experience of medical personnel in war directly stimulates advances in medical science. More importantly, it demonstrates that the organization of health care in the combat zones, including evacuation of the wounded, control of disease among troops and civilian populations, and care of prisoners of war, contributed directly to the Allied victory. The exploits of the doctors, corpsmen, and medical support units provide a model for the planning and organization of medical support in today's Army.

The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

The present volume, together with the greater part of the third volume of the General History, deals with the Medical Services of the British Expeditionary Force in France and Belgium. It has been compiled almost entirely from the war diaries of medical units, from the Office files and diaries of the Administrative Medical Services, and from the official despatches; but as some of the war diaries are missing altogether and many fail to record facts which are Of import ance in compiling a history, several gaps have had to be filled from personal diaries and information Obtained from various other sources. The war diaries are, in fact, of very unequal value. The most useful have been those which give Sketch maps, details of the position and work of the medical units and changes in organization and policy, but these important details have been omitted in a considerable number of instances. The campaign on the Western Front was characterized, so far as the Medical Services were concerned, by the unparalleled number of wounded who came under their charge. Bruce, who served with a field ambulance of the Slst (highland) Division. Several of the photographs in the text of the preceding volume were also obtained from this Officer. The other photographs are from official sources, several of them, notably those in the chapter on the Medical Services in Italy, having been supplied by lieut.-colone1 F. S. Brereton, who visited areas of operation with a view to Obtaining material for the Imperial War Museum and the Medical History of the War. The compiler is again deeply indebted to brig-general J. E. Edmonds, the Director of the Military Branch of the Historical Section of the Committee of Imperial Defence, for his valuable assistance in reading through the manuscript. The limitations of time and space, which had been imposed for the completion of the Medical History of the War, made it difficult to introduce incidents that would render this and other volumes more attractive to the general reader. All that has been attempted therefore has been to provide a basis on which those who are desirous of studying more closely the medical situation during the various battles and phases of the war may be able to reconstruct with the aid of the maps the chief features of the work of the medical services.
