Available in PDF, EPUB and Kindle. Book excerpt: Prior to and during the Second World War, the Japanese Army established programs of biological warfare throughout China and elsewhere. In these “factories of death,” including the now-infamous Unit 731, Japanese doctors and scientists conducted large numbers of vivisections and experiments on human beings, mostly Chinese nationals. However, as a result of complex historical factors including an American cover-up of the atrocities, Japanese denials, and inadequate responses from successive Chinese governments, justice has never been fully served. This volume brings together the contributions of a group of scholars from different countries and various academic disciplines. It examines Japan’s wartime medical atrocities and their postwar aftermath from a comparative perspective and inquires into perennial issues of historical memory, science, politics, society and ethics elicited by these rebarbative events. The volume’s central ethical claim is that the failure to bring justice to bear on the systematic abuse of medical research by Japanese military medical personnel more than six decades ago has had a profoundly retarding influence on the development and practice of medical and social ethics in all of East Asia. The book also includes an extensive annotated bibliography selected from relevant publications in Japanese, Chinese and English.

Book excerpt: This public inquiry report into serious failings in healthcare that took place at the Mid Staffordshire NHS Foundation Trust builds on the first independent report published in February 2010 (ISBN 9780102964394). It further examines the suffering of patients caused by failures by the Trust: there was a failure to listen to its patients and staff or ensure correction of deficiencies. There was also a failure to tackle the insidious negative culture involving poor standards and a disengagement from managerial and leadership responsibilities. These failures are in part a consequence of allowing a focus on reaching national access targets, achieving financial balance and seeking foundation trust status at the cost of delivering acceptable care standards. Further, the checks and balances that operate within the NHS system should have prevented the serious systemic failure that developed at Mid Staffs. The system failed in its primary duty to protect patients and maintain confidence in the healthcare system. This report identifies numerous warning signs that could and should have alerted the system to problems developing at the Trust. It also sets out 290 recommendations grouped around: (i) putting the patient first; (ii) developing a set of fundamental standards, easily understood and accepted by patients; (iii) providing professionally endorsed and evidence-based means of compliance of standards that are understood and adopted by staff; (iv) ensuring openness, transparency and candour throughout system; (v) policing of these standards by the healthcare regulator; (vi) making all those who provide care for patients , properly accountable; (vii) enhancing recruitment, education, training and support of all key contributors to the provision of healthcare; (viii) developing and sharing ever improving means of measuring and understanding the performance of individual professionals, teams, units and provider organisations for the patients, the public, and other stakeholders.

Although research on health literacy has grown tremendously in the past decade, there is no widely agreed-upon framework for health literacy as a determinant of health outcomes. Most instruments focus on assessing an individual's health literacy, yet the scope of health literacy reaches far beyond an individual's skills and abilities. Health literacy occurs in the context of the health care system, and therefore measures of health literacy must also assess the demands and complexities of the health care systems with which patients interact. For example, measures are needed to determine how well the system has been organized so that it can be navigated by individuals with different levels of health literacy and how well health organizations are doing at making health information understandable and actionable. To examine what is known about measures of health literacy, the Institute of Medicine convened a workshop. The workshop, summarized in this volume, reviews the current status of measures of health literacy, including those used in the health care setting; discusses possible surrogate measures that might be used to assess health literacy; and explores ways in which health literacy measures can be used to assess patient-centered approaches to care.
