Book excerpt: Excerpt from Clinical Surgical Diagnosis for Students and Practitioners When Dieffenbach summarized his views on trephining, in his classical work, some sixty years ago, before the question was definitely settled, he wrote as follows: - "Up to quite recently it was the first urgent duty of the surgeon, immediately after the injury, to make a large crucial incision, and to search for fissures or fractures. Occasionally I found on my arrival that the hairy portion of the head had been ploughed up by incisions, backwards and forwards, crucial and transverse. This was considered to be indispensable in all head injuries. A surgeon who had omitted to make the crucial incision would have incurred the same responsibility as one who failed to open the window in endeavouring to rescue a victim of coal gas suffocation." At that time there was no "cerebral diagnosis"; but nevertheless the surgeon's need for a diagnosis had to find some expression. To leave an injury to Nature, without knowing whether or not a fissure was present, was considered to be an unworthy and culpable piece of surgical inactivity, because the real danger of a fractured skull had not yet been clearly ascertained. Philosophical speculation took the place of observation, and therefore surgery for a long time was guilty of one of its greatest sins, in breaking the law primum nil nocere. We smile at this kind of diagnosis; but posterity will look upon some of our exploratory procedures, involving flaps of half the skull, much in the same way as we regard those meaningless "crucial and transverse incisions." I have long contemplated it, and have finished it after several years' labor. It has taken some time, because it represents almost purely personal work, which has been frequently interrupted. It does not pretend to be based on my personal practice only. In these days of specialized work this would be impossible, though with most of even the rare forms of disease in every section I have had some experience. To fill in the gaps of my own knowledge I have used that of others, but have always sought to make suitable acknowledgment to the proper source, and if this has not been done in any case, it has been a matter of oversight. I had not, at the outset, expected to illustrate the work, but, as it progressed, a certain number of illustrations seemed necessary, not only to explain the text, but also, in a few instances, to render clearer the treatment described. Thus the number of charts and other drawings has grown to nearly a hundred, all of which, it is hoped, will be found useful. In expectation of the ultimate adoption of the metric system for the measuring of doses, these have been indicated throughout the book in the metric and English measures. The general practitioner, representing the unity and connection of the various branches of medical practice, must grasp the practical details of his art in order to be useful at the bedside; and a book to be of value to the family physician should convey clinical experience Without the exhaustive and often purely theoretical details to be found and sought for in monographs. Such a work I have endeavored to write. It has been my desire to reestablish the relations of internal medicine, surgery, and the several specialties; for this reason I have presented special istic methods from the view - point of the general clinician. Disease is neither medical nor surgical nor does it hover on the border lines, but the treatment of disease has become more surgical and the arbitrary division into medical and surgical disease is no longer tenable. This unitarian principle should not be ignored in the presentation of disease, and many methods of diag nosis and treatment originally worked out by the specialist have become, or should become, common property. Drugs no longer dominate our therapeutics; therefore the prominence given to hygienic, prophylactic, dietetic, hydrotherapeutic, and physical methods of treatment. At the same time Well-tried and valuable formulae are distributed throughout the book. In prescription writing the apothecaries' weight and not the metric system has been used. However, simple rules for converting one into the other are given. The special chapters on the Technique of Diagnosis and Laboratory Aids, on Paediatrics, and the various specialties, on diseases of the Osseous, Muscular, and Articular System, on Nutrition and Diet, on the Management of Dropsy and Effusion, on Massage, Vibration, Dry Hot Air Treatment, Poisons and Anaesthesia, it is to be hoped will not be an unwelcome addition to a book on practice.
