Book excerpt: There are approximately 4,000 fatalities in crashes involving trucks and buses in the United States each year. Though estimates are wide-ranging, possibly 10 to 20 percent of these crashes might have involved fatigued drivers. The stresses associated with their particular jobs (irregular schedules, etc.) and the lifestyle that many truck and bus drivers lead, puts them at substantial risk for insufficient sleep and for developing short- and long-term health problems. Commercial Motor Vehicle Driver Fatigue, Long-Term Health and Highway Safety assesses the state of knowledge about the relationship of such factors as hours of driving, hours on duty, and periods of rest to the fatigue experienced by truck and bus drivers while driving and the implications for the safe operation of their vehicles. This report evaluates the relationship of these factors to drivers' health over the longer term, and identifies improvements in data and research methods that can lead to better understanding in both areas.

Book excerpt: Although road accident deaths have halved between 1958 and 2007 whilst the number of licensed motor vehicles and vehicle mileage covered increased by 400 per cent, the current rate of 3,000 deaths and 250,000 injuries is still an unacceptably high level. Road accidents are the largest single cause of death for people between the ages of 5 and 35 in Britain, and road accidents cost our economy some £18 billion each year. The number of deaths and injuries on roads far outweighs the deaths and injuries in other transport modes, and should be viewed as a major public health problem. The Government should establish a British Road Safety Survey to track overall casualty and safety trends, and review current methods for recording road-traffic injuries. The Committee recommends a systems approach to road safety: ensuring the vehicle, the road infrastructure, regulations and driver training are designed to similar safety and performance standards. Other recommendations include: more 20 mph speed limits; a more proactive approach to determining the safety benefits of new vehicle technologies; action on young drivers - who represent a disproportionate risk to road users - and vulnerable users: motorcyclists, elderly and child pedestrians and cyclists, horse riders; a higher priority given to enforcement of drink-drive and drug-drive offences. The Committee recommends the establishment of an independent Road Safety Commission with powers to work across the whole of government, ensuring that a high priority and adequate resources are given to road safety and that all government departments and agencies give active support. The Government should also establish a road accident investigation branch, like those in aviation, rail and marine.

Book excerpt: One factor that distinguishes older drivers from their younger and middle-aged counterparts is a higher prevalence of medical conditions, and the medications used to treat them. Often, the medical conditions that are more prevalent among older people lead to impairments in visual, cognitive, or psychomotor functions needed to drive safely. Similarly, while some medications restore function and improve mobility for those who would otherwise be unable to drive, an array of potentially driver impairing (PDI) prescriptions and over-the-counter medications have been associated with a statistically significant increase in crash risk.The current understanding of how medical conditions can affect driving is based on the opinions of medical (including rehabilitation) professionals or traffic safety experts, or has been derived from simulation research. Other studies have compared the driving records of drivers whose licenses were restricted as a result of reported medical conditions to those of matched controls with the same conditions who had full driving privilege. However, there is a dearth of empirical data about the relationships between medical conditions common among older adults and either performance or safety outcomes of drivers under realistic driving situations. Further, few studies have explored how people with such conditions may limit their driving exposure.

In 1937, the family moved east to the small town of Bronxville near New York City, when Reverend Arthur Doege became president of Concordia Collegiate Institute, a small, Lutheran Church-affiliated high school and junior college. Attending Bronxville High School and then receiving a scholarship to Oberlin College, Ted graduated in 1950 and briefly was a Burlington Railroad section hand. Drafted into the Army in late 1952, he became a parachutist, after discharge enrolling as a medical student at the University of Rochester School of Medicine and Dentistry. In 1957, Ted married medical school classmate, Ann Elizabeth Edmondson, a graduate of the University of California, Berkeley. At the University of Rochester, Ted also received training as a pathology Fellow. From 1958 to 1967 he held positions at the Centers for Disease Control, Atlanta and the University of Washington, Seattle. In 1967, Doctor Doege was appointed Visiting Associate Professor at the Faculty of Medicine, Chiang Mai, Thailand, after which he returned in 1970 as an associate professor in the College of Medicine, University of Illinois at Chicago. From 1977 until he retired in January, 1996, Doctor Doege directed or advised an American Medical Association science group responsible for environmental, public, and occupational health issues.
